ABSTRACT

Our study deals with to comply the data of accident happened in chemical industries and analyze
the data to assess causes of accident, highly hazardous area or highly accident-prone equipment
and machinery in an industry.
This project is based on simple approach to collect the data from various sources either it is a
government report, newspaper article, individual agency report, NGO report etc. This project
report covered all the accident happened in a chemical industry since 2000. It consists the
detailed report and assessment that which is most accident prone or hazardous equipment in an
industry. One hundred and twenty-four accidents involve major fires or explosion which
occurred across the India in tanks, warehouses, storage yards etc. have been studied.

Introduction-
India is one of the most developing or growing country in the world, some good path is taken by
Indian government and chemical industries in developing progress of the country. But in these
chemical industries growth of modernization of processes has not been quite good it generates
lots of accident in industries due to negligence of safety measure or over exploitation of
machines without proper maintenance and services. Due to this more people or skilled
personnel now live dangerously close to some hazard in these industries.
As per a study done by International Labour Office which revealed that a total of 586 major
accident hazard unit and 75 hazardous chemicals. In a report of “The Guardian” it states that
54000 people killed or injured in factory accident between 2014-2016 and many other industrial
accidents remain unknown or unreported. The biggest mishap had been written in dark words in
the history of India when Bhopal Gas Tragedy occurred in 1984 in which 4000 people dies
within days and over 16,000 claimed. In this approx. 5.5 lakh people suffered of non-fatal
injuries. This report explains a portion of accidents and fire in chemical process industries which
occurred during (2000-2020) in India. This report considered the examination of process
industries, and due to these accidents in the industries many employees or workers were affected
which led to loss of skilled personnel, loss of money and fatality.

Definition of accident-
Accident is an unfortunate or unexpected or unintentionally event under a circumstantial
situation which outcomes is damage to the property, environment and fatality of human being. It
can be averted by achieving the environmental condition, periodic inspection of equipment and
implementation of engineering controls.
It is of mainly three types-1. Major accident
2. Minor accident
3. Near-miss incident
But we mainly explain here about the minor and major accident in chemical industries.
Major accident- In this accident outcome is damage to property, environment, injury or fatality.
Minor accident- In this accident outcome does not in damage to property, environment, injury
or fatality.
Accident can further be classified on the following basis- (Depend on the work and damage)

Degree of avoidability
Degree of property loss
Degree of environmental damage etc.
Reason of accidents- As briefed above it is an unfortunate or unexpected event which can take
place at anywhere but accident occur due to unsafe working condition, human error, mechanical
fault etc. It is a point to take up that why the condition are unsafe, why the cause of human error
and the simple answer is due to increase in demand with growing economy, population and huge
demand result in the increase in pressure on industries of fulfilling demand, branding their name,
competency etc. are the root cause behind the accidents. By increasing of mechanization,
chemicalization and other processes have made industrial jobs more and more complex and
intricate, this has led to various accidents and injuries as well as damage to both property and
people working there.
Some of the main reasons of accidents are-
● Unsafe conditions
● Unsafe acts
● Mechanical faults
● Overload
● Human errors
● Fire
Major Process Hazard-
Hazard is a physical or chemical situation which has the potential to cause harm to property,
environment and life of human under different circumstances. Chemical process industries are
concerned due to some specific hazards are fire, explosion, blast, leakage, rupturing etc. These
possible hazards can do some serious damage to the industries which create a huge loss of
property, lives, environment and also cause impact on economy. Accident of these type of hazard
directly impact individual or societal profit.
As per the data collected of chemical industry accident take place between 2000-2020 in which
explosion is the main reason behind the 30 percent industry accident which caused a huge
damage to the property and threat to human life via injury or fatality. Secondly fire is the main
cause of industry accident. On third leakage of toxic gases is the cause of industry accident
which had proportion of 18.60 %. The leakage of toxic gas releases has perhaps the greatest
potential to impact large number of populations in a very less time which can cause area toxified
for several months to several years. Large toxic release or leakage is very rare case but as the LG
polymer gas case it causes huge impact on nearby community result in death of 12 people, at
least 32 animals get killed and movement of thousands of people out of toxified area.
Overall, there is no distinction is made between fire, explosion, leakage, flash fire, blast, human
error etc.
As per globally increase in chemical industries accident, companies should take safety as priority
in their working environment and in workers behavior. Also, it’s a job of industries to provide
proper training and shift break to decrease human error while for other causes need to safety
tools of engineering either it is safety valve, relief valve, control system etc. and proper waste
disposal to prevent environment.
LG Polymer Gas Leakage-
Bhopal gas tragedy still running in mind since 1984 which caused a disaster resulted in the death
of approx. 16,000 people and long-term injury of approx. 5 lakh people whose traces are still
available in Bhopal. This accident had taken place due to negligence of safety measures by the
industry.
The same type of mishap had taken place on 7 May 2020 in LG Polymer firm in Vizag or
Vishakhapatnam in the state of Andhra Pradesh. According to a report, gas started leaking
around 2:30 a.m. which was identified by two residents living just about 200 meters from the
plant. Kiran is the first who smelt the gas and immediately dialed his friend Suresh living on the
other end of the village. Around thousands of people live in the nearby region of the plant. Due
to vapors of styrene gas around 12 people and 32 animals got killed in this mishap. After hearing
the gas leakage news, the local department started the process to empty the people from villages
and also NDRF started its work to assess the situation of gas present in the environment. In the
escaping over 800 people had suffered injuries which were then hospitalized. It had been
reported that around 15,000 people have been vacated from their homes in panic. There also
were sufficient indications that the environment through which vapors traversed has been
adversely affected. Even a week after the tragedy people living in the nearby area of LG polymer
plant feared to go back because the tragedy due to gas leak engulfed their mind with fear. Many
complaints have come to the local hospitals of having headaches, vomiting, itching and burning
sensation in eyes, breathing problems, dizziness and skin irritations.

There was every possibility that the death-toll would have been far higher if the highly
inflammable styrene vapors had ignited and the huge quantity of styrene monomer stored in two
large tanks at the plant had exploded. Miraculously that did not occur and the vast majority of
people in the Vizag metropolitan region survived due to disaster unscathed.

The reason behind the least effect of gas leakage is that styrene gas is not that much toxic but it is
highly inflammable. Styrene gas has a high Threshold Limit value which is 215 mg/m3 whether
in Bhopal gas leakage methyl isocyanate has a high Threshold Limit Value which is 0.05mg/m3.
The National Green Tribunal put a finger on the plant behind this accident which is LG Polymer
India Pvt. Ltd. is owned by LG Chem Ltd. an affiliate of LG corps - the giant South Korean
multinational company. LG polymer plant had employed about 500 workers and had a
production capacity of about 650 tons of styrene-based product every day.

Styrene gas which was imported from Singapore was probably stored in a 5000 KL capacity
tank. At the time of the accident one of the storage tanks in the plant had reportedly contained
about 1900 KL of styrene monomer, was the one that had leaked. As per report, the company
worker first detected the gas leak but did not alert authorities or the nearby residents. As per the
authorities first call of gas leakage came around 3:25 am by the local resident, not the company.
Kiran and Suresh are the people from residents who woke the others and evacuated the villagers
to safety for the next one hour. Due to protest by the people, the company had to transport the
remaining styrene monomer around 13,000 KL by ship back to Seoul. The latest disaster at vizag
is yet another example of utterly irresponsible attitude of at least some sections within decision
makers in the industry whose priority is anything but health and safety and who are averse to
upholding principles and in following best practices. Despite of many accident’s lessons have not
been learned and the reoccurrence of Styrene related runaway incidents continues.

Lack of environment clearance-


The South Korean parent company, LG Chem, said in its May 2019 affidavit, a part of an
application for environment clearance, that the company did not have a legitimate environmental
clearance issued by the Ministry of Environment, Forest and Climate Change (MoEFCC), after
receiving an Environmental impact assessment (EIA), substantiating the produced quantity and
for continuing operations. According to the EIA notification (amendment) of 2006 under the
Environment Protection Act of 1986, LG Chemicals India, which is part of the petrochemical
industry, falls into the category 'A' and should get clearance from the MoEFCC every time they
expanded their plant or brought a change to their manufactured product after November 2006.
LG Chem expanded its operations at LG Polymers plant five times between 2006 and 2018
without such clearance. According to the May 2019 affidavit, since 1997, it was instead
operating with state permits required for starting a new business with renewals every five years.

LG Polymers had never asked for a federal clearance until 2017 and as per the minutes of a
meeting between the company and the Andhra Pradesh Pollution Control Board, the latter denied
the former's request of producing engineering plastics at its plant. However, a member of the
state pollution board said that there was no information available regarding any order by the state
government to stop the operation of the plant. In 2018, the company applied for an
environmental clearance for the first time, to expand its manufacturing capacity of polystyrene, a
plastic used to make bottles and lids. The Environment ministry sent the application for a review
citing that the company didn't have a clearance for the chemicals it was already manufacturing.
The company withdrew the application while applying for a retroactive clearance that the
ministry offered to companies in 2018 as a one-time amnesty, which remained pending until the
fatal leak occurred.

According to the AP, officials and legal experts indicated that the plant seemed to be operating in
a legal grey area, with the environmental clearance required under central regulations while the
state executives are to look after the enforcement.

Relief and Rescue-

Nearly 200-250 families were evacuated from villages in a five-kilometer (3.1 mi) radius around
the plant. About 300 people were hospitalized, according to a media report. The chief minister of
Andhra Pradesh announced a grant of Rs. 1 crore for the kin of those deceased in the accident.
He further announced ₹25,000 for those who received primary treatment, RS 1 lakh for those
who received longer treatment, and ₹1 lakh for those on ventilator support. To neutralize the gas,
around 500 kilograms (1,100 lb.) of the antioxidant 4-tert-butylcatechol (PTBC) will be airlifted
by the Government of Andhra Pradesh and sent to the crippled factory. The Central Government
flew in a specialized CBRN (chemical, biological, radiological and nuclear) team of the National
Disaster Response force (NDRF) from Pune to the site as well.

Health effects of Styrene gas-

● Harmful to aquatic life
● High exposure lead to brain and liver functions
● Poisonous gas produced when it is ignited

Views of experts on the cause of gas leakage-


● Some experts suspected that corrosion to be responsible of leak.
● Some experts suspected that overheating and corrosion is the reason of gas leak.
● As per initial investigation reports the cause of gas leakage is maintenance failure,
operational error and improper storage.
● Some experts suspected that clogged cooling system was the cause of temperature surge
in a storage tank which lead to gas leakage.
Oil India Baghjan fire-
Before this mishap occurred in 2016 after the proposal of this project several NGOs and
environmental activists protested against the extraction from oil wells due to lack of experience
and machinery for deep extraction. Oil India brought the machinery from a Gujarat firm for this
project and got a nod from MOEFCC after a 2-year break. Fire has been started in the oil well
under the Oil India limited in Baghjan Oilfield. First blowout took place on 27 may 2020 and the
cause behind this is machinery failure leading to uncontrolled leakage of gas/oil from the well.
Gas is leaking uncontrollably despite the continuously pumping of water in the well. Leaking gas
catches fire on 9 June which does not doused upto 50 days. After the leakage local communities
residing near the oil well transferred to the relief camp 12 km from the site. It creates a huge
impact on the environment and the biodiversity in the nearby national park. As per a report, NGT
ordered Oil India to pay the fine of 25 crores for the damage to the environment. Oil India passed
a statement to pay the 30,000 as a compensation for the damage. As per studies done by OP
Singh faculty at the Department of Environmental Studies, North Eastern Hill university is that
oil spill on the ground contains hundreds of toxic and carcinogenic chemical compounds
contaminating the soil and environment. It affects the fertility of soil and breathable air that gets
poisonous for long periods which shows the high damage to health and property of people. Two
firefighters got killed during the extinguish of fire. Oil India calls the Singapore experts to
control the blowout which got injured in the controlling process.

Effect on environment and biodiversity-


 An endangered gangetic dolphin was recovered in critical condition after a blowout.
 Dibru Saikhowa National Park and Maguri Motapung Beel which are a unique
biodiversity hotspot for 36 mammals and 382 species of birds got affected because of
blowout and its sound upto 12 km.
 As per experts due to this blowout very less birds are seen in the area, either they have
died or flown away.
 A bird guide Jiban Dutta said that on 6 June locals rescued a king quail covered in oil.
 Environment impact assessment report shall be prepared by NEERI.
 Environment remains contaminated for a long period of time and upto then the site
remains prohibited for living.
 As per survey 1383 families affected by the fire and 1610 families evacuated.
 Fertility of land got affected due to spillage of oil in the field.

● Blast- blast is one of the main reasons for the accidents in industries occur due to some
materials which are energetically unstable and has explosives properties, materials are
sensitive to shock, friction, heat etc. and may dictate special procedures for safe handling.
Blast cause around 9% of the accidents in chemical process industries
● Explosions- An explosion is a rapid increase in volume and release of energy in an
extreme manner, usually with the generation of high temperatures and the release of
gases. Explosion causes around 30% of the accidents in chemical process industries in
India.
● Fire-The fire is a process of burning that produces heat, light, and often smokes and
flames. the effects of fire on the people takes the form of skin burns due to exposure to
thermal radiations. Fire causes around 27% of accidents in chemical industries
● Flash fire-A flash fire occurs when a cloud of flammable gas and air is ignited. the speed
of burning is function of the concentration of the flammable component in the cloud and
also the wind speed. Within a few second of ignition the flame spreads both upwind and
downwind of the ignition source. Flash fire causes almost 6% of accidents in chemical
industries in India.
● Human error- It has been defined as any improper decision or behavior which may have a
negative impact on the effectiveness, safety or performance system. human error caused
around 2.5% of accidents in chemical industries in India
● Overheating and high pressure- It occurred due to failure of pressure relief system and
temperature control system of a process line in an industry. In chemical industry
accidents it contributes about 2.32 %.
● Spillage- In industrial production spillage is the loss of production output due to
production of a series of defective and unacceptable products which must be rejected.
spillage can lead to various explosions and blast in oil and chemical industries. Spillage
caused about 2.3% of accidents in India since 2000
● Blowing- In this blowing up of chemical reactor which mainly contributes around 1.16%
accidents in the chemical industries.
● Leakage- a leak is a way for a fluid or a gas to escape a container or tank, through which
the contents of the container can escape or outside mater can enter which can cause the
accidents. types of leak openings include puncture, rust or other corrosion hole, crack etc.
leakage causes around 18% of accidents in industries.

● Storage- this is the area or tanks where all the flammable substance or chemicals are
stored in a chemical process industry. So, in this area safety is an important factor but due
to some reasons some mishappening occurs in these areas which lead to various accidents
in these industries. accident in storage area in India is about 4.54%
● Laboratory- this is a place equipped for experimental study in a science or for testing and
analysis a research in some chemical process industries. these laboratories might catch
some fire due to some reaction in them or due to some leakage. accidents in laboratories
in India is around 6%
● Boiler- A boiler is a closed vessel in which fluid is heated. The heated or vaporized fluid
exits the boiler for use in various processes or heating applications. some accidents also
happen due to less safety of boiler or not so proper safety precautions while working on
boiler. around 13.63% of accidents in chemical process industries is because of boiler
accidents.
● Storage tanks- storage tanks are containers that hold liquids, compressed gases there are
various types of tanks which used by chemical process industry like transport tank, boiler
tank etc. In India about 16% of the accidents in chemical process industry is because of
some defect or leakage or fire in these storage tanks only.
● Warehouse-A warehouse is a building for storing goods. Warehouses are used by
manufacturers, importers, exporters, wholesalers, transport etc. so as we observed in
many chemical industries accidents warehouse fire or some storing goods or flammable
product caught fire in warehouse which lead to an accident, about 3.03% of the accidents
in India are because of warehouse lack of safety.
● Chemical reactor- A chemical reactor is an enclosed volume in which a chemical reaction
takes place. In chemical process industries there are various hazards related to chemical
reactors if safety is not proper according to these hazards it can lead to various accidents
as in our report, about 18.18% of accidents in chemical process industry in India are
because of chemical reactors.
● Disposal area- this is the area where waste product from chemical process industry is
being disposed so if there is some flammable product which is going to be disposed or
any other product which can caught fire or can be reacted easily should be disposed by
proper precautions but in India about 4.54% of the accidents is because of the disposed
material as it was reactive or flammable.
● Pipeline- Pipeline is used for the transportation of liquid and gases to various places and
in chemical and oil industrial flammable liquid and gases are being transferred so there
are chances of accidents because of the hazards, about 1.51% of the accidents caused by
pipeline accidents.
● Electric circuit fault-this is one of the most common faults in any kind of industry but it
can lead to various accidents causing damaged to property as well as loss of life, in
chemical process industries electric circuit fault cause around 4.54% of the accidents
● Drums- these are the vessel in which any type of liquid is stored maybe flammable and
any type of chemical as per chemical process industry, Drums caused about 1.51% of the
accident in chemical and oil industries.
● Cylinders- in cylinders flammable or non -flammable gases and liquid is being kept. but
cylinders can cause various accident if not handled or kept properly at proper places.
these cause about3.03% of the accidents in industries.
● Vessel- this is a solid hollow container used to contain liquid, flammable or non-
flammable liquid could be there so it can lead to any accidents in chemical process
industry. about 1.51% of accidents in these industries is because of carelessly handling of
vessel.
● Valve- A valve is a device or natural object that regulates, directs or controls the flow of
a fluid (gases, liquids, fluidized solids, or slurries) by opening, closing, or partially
obstructing various passageways. so due to some failure in valve lead to an accidents so
proper maintenance of valve should be there. in chemical process industries there are
around 1.5% of accidents which caused by valve.
● Heat exchanger- A heat exchanger is a system used to transfer heat between two or more
fluids. Heat exchangers are used in both cooling and heating processes. So, if there is any
wrong regulating or transferring of heat from one liquid to another which can lead to
an accident. In chemical industries 1.5% of accidents are because of the wrong handling
of heat exchangers.
● Unknown- There are some of the reasons for the accidents in chemical process industry
which left undetected by the accident surveyors or other safety professionals this factor is
about 9% of the total accidents in India in chemical and oil industries.
● Compressor Shed- Compressor shed contributes around 1.51% in chemical industry
accidents.
● Vacuum dryer- It is an equipment used to remove moisture from the solid materials,
usually a wet solid has been removed by creating vacuum. It contributes around 1.51% in
accident.
● Cooling tower- It is a specialized heat exchanger in which air and water are brought in
contact to reduce water temperature. It contributes around 1.51% in chemical industry
accidents.
● Storm water drainage- It is a well-planned infrastructure designed to drain excess storm
water or rain water to prevent the standby of water at one place. It contributes around
1.51% in accidents in chemical industry.
● LD converter- LD converter is a refined version of Bessemeter converter in which
blowing of air is replaced by blowing of oxygen. It reduced the cost of plant, time of
smelting and increase in labour productivity. It contributes around 1.51% in accident in
chemical industries.
● Bottling Chamber- In this bottle has been filled with the fluid materials in the contained
chamber. It contributes around 1.51% in accident in chemical industries.

As per CAIRS (govt. agency) and news reports we found about 124 accident in which 65
accident record had been found in news reporting’s and about 59 accident remained unreported.
It is a very small number in reporting as per “The guardian” from 2014-16 around 54000 people
get killed in industries accident which is a huge number and many of them remained unreported
and unrecorded which results in exploitation of workers and employees by the industries to
remain prevented from paying the compensation wages to them. Many cases go unreported
because for securing their name, brand, accident accusation charges, external investigation,
prevention of fine by government, environment exploitation charges, safety measures
investigation and from several other reasons.
Detailed report of some Chemical Industries accident-
Chemical manufacturing industry
Boisar Factory Blast
The death toll reported in the blast at a chemical factory in Palghar district rose to 8. As
per the local administration blast occurred at an under construction pharmaceutical
factory. Authorities are unable to point out what had caused the boiler explosion.
Explosion was so huge that it was heard upto 15 km radius and windows of some houses
got shattered.

Metropolitan Eximchem pvt. ltd


On 18 February 2020 an accident occurred in the chemical company in Dombivali led to
the fire. The fire was extinguished around 4 am, 15 hrs. after it started. The blaze at
Metropolitan Eximchem Pvt. Ltd. in Maharashtra Industrial Development corporation led
to over 100 explosions. An inquiry has been set up by Thane collector to investigate
whether the factory was following fire safety regulation or not.

RIL rubber plant


3 people were killed in a fire that broke out in RIL rubber plant on 29 November 2018.
The fire was restricted by the effort of the RIL firefighting team in one section. An
investigation has been started to point out the cause of the fire. (RIL rubber plant, 2018)

Balaji Chemicals
On 14 June 2019 an explosion broke out in the boiler at Balaji chemicals. 3 people have
been killed in this accident. Thick layer of smoke from the explosion engulfed the area.
Firefighting team rushed to the spot and the collector ordered an inquiry of the accident.

Alkyl Amines plant


A fire broke out in a chemical plant in MIDC area Pune. Fire broke out at plant Alkyl
Amines around 10:15 pm. No casualties occurred due to the accident. Fire broke out in
the storage area of solvents.

GIDC chemical plant


On 10 October 2019 a major fire broke out at a chemical factory in an industrial area in
Gujarat. There were no reports of any casualties in this accident.

Balaji Chemicals
13 people killed and 72 injured in a fire at a chemical factory in Dhule. As per police, fire
started about 9:30 in a factory. Fire was followed by several explosions and it takes 2.5
hrs. to bring under control. Workers were caught in blaze and died due to burn, others
died of suffocation and the collapsing by the portion of factory.
Sajjan India ltd
On 7 July 2019 fire broke out in the warehouse of a chemical company in Ankleshwar.
No casualties were reported in this accident. Manufacturing unit located in the vicinity
has been shut by administration as a precautionary measure.

Kandla chemical factory


A blast has taken place in a chemical warehouse close to Indian Oil refinery near Kandla
port in Gujarat. 3 people are reported dead in the accident. As per reports, a storage tank
caught fire and was a reason behind the blast.

LG Polymer
On 7 may 2020 a tragic accident took place in the chemical plant of LG polymers. 11
people including a child reported died after toxic gas released from a chemical plant.
More than 200 people have been admitted to hospital due to severe problems with
respiration.

Shakti Paper Mill


This incident took place at Shakti paper mill in Tetla village in Chhattisgarh on 7 may
2020. There were some workers who were cleaning an open tank filled with paper waste.
The mill had remained shut due to which there was poisonous gas in that tank. At least 7
workers of a paper mill fell ill after inhaling poisonous gas.

Bharat Dynamics
There is an explosion in Bharat Dynamics Limited, a defense laboratory in Hyderabad.
The accident occurred in BDL premises where employees were disposing of waste
explosives in a pit. Small quantity of waste explosive exploded while being disposed of
as a routine practice at a burning pit. Four workers were injured in that accident.

Bhagiratha chemicals
A major fire accident occurred at production block 3 at a factory site of Bhagiratha
chemicals located at ongole, Andhra Pradesh. In this accident 2 people died and caused
major loss to the property.

Gujarat Narmada valley fertilizer and chemicals ltd


An accident took place when there was a gas leakage at Toluene Di-Isocyanate TDI unit
at Dahej. This was due to mechanical fault in the heat exchanger of the company’s TDI
plant. 6 workers died due to the inhalation of gas and many more were also affected.
United Phosphorus
An accident took place in which a major fire broke out in the scrapyard of united
phosphorus limited plant number 1 at Ankleshwar. The smoke columns which were
formed could be seen from 2 km in that fire. No casualties took place but some damage to
the property happened.

GSP cropscience
A major accident took place when a blast happened which led to a huge fire at the
premises of a chemical plant of GSP crop sciences. 4 workers were killed and 9 others
injured in that accident. Machinery and equipment at two plants of the company were
damaged.

Hindustan insecticides ltd


An explosion in a tanker following leak of hazardous carbon disulfide during an
unloading session at the premises of the Hindustan Insecticides Ltd. This incident
happened when the CS2 in liquid form was being transferred from a tanker to a
manufacturing plant at the Margozeb factory. The volatile liquid compound catches fire
spontaneously upon coming into contact with air. A leak in the tanker valve has resulted
in release of the toxic vapor and caused at least 3 explosions which deflated tanker tires.
16 people were injured in that explosion.

Vizag steel plant


This was the worst ever accident in the history of Visakhapatnam steel plant. In this
accident 16 workers including some officers were burnt alive following a huge explosion
in the oxygen plant in the steel melting shop 3 unit. The explosion occurred when
officials were conducting a trial run of the oxygen plant. The oxygen plant was
completely gutted as huge flames swept through the unit.

Pune Chemical factory


A major fire broke out at a chemical manufacturing unit in Pune. No casualty was
reported in the incident as the plant, located in Kurkumbh, MIDC. The blaze erupted
around 10.30 am. Water tankers were rushed to the site and the fire was doused. He
added that drums containing acetone and ethanol were kept at the plant. As the fire
spread, those drums caught fire and exploded. A blanket of thick black smoke engulfed
the area after the blaze.
Asiatic Gases
This accident took place in Asiatic gases limited in Orissa. At least two people were
killed and two injured today, when oxygen cylinders exploded in a plant.

United Phosphorus ltd.


Two laborers were killed when a reactor exploded in a united phosphorus limited
chemical unit at Ankleshwar. At least six people were injured, one of them critically
injured in that blast, and the fire that broke out. eight people were working in the unit
when the accident took place, blast caused due to the overheating of the reactor.
Fertilizers and Chemicals Travancore Ltd
This accident happened when ammonia gas was being transferred by a lorry going to
Ambalamedu division. This Leakage of Ammonia gas has been reported at Fertilizers and
Chemicals Travancore Ltd.

Hindustan insecticides ltd


This accident took place at Hindustan insecticides ltd (HIL) at Udyog Mandal near
velour. This accident occurred when technicians were draining the chemicals from a
bullet tanker. At least 12 persons were injured in this accident.

Gharda Chemicals ltd


Two persons were killed and three injured in a fire at the Gharda Chemicals ltd, MIDC,
in Dombivli. Thirty workers were in the pesticide manufacturing unit's plant 4 when there
was a huge explosion. Primary investigations show that the fire was due to a mechanical
fault.

BHEL
This accident took place when a major fire burst out in the Bharat Heavy Electricals
Limited's (BHEL) plant. The fire started from BHEL's transformer block number three.
No casualties or injuries were reported in the incident.

Gulf Oil
This incident took place when a major fire broke out on the premises of the Gulf Oil
Corporation Limited, formerly the Indian Detonators Industries (IDL), at Kukatpally.
When the employees had gone out for lunch. The loss of property was not estimated. The
fire mishap, coming after last year's blast in the company which claimed eight lives,
created panic in Kukatpally and surrounding areas. According to the police, the fire
started after a cylinder blast in the R&D lab.

Petrochemical/ Power generation industry-


Indian oil corporation limited
On 29 October 2009 an accident took place in Indian Oil Corporation fuel depot in
Sitapura. Due to the explosion 12 people were killed and 200 injured in this accident.
Smoke has been scattered, leaving the sky dark which causes the movement of people of
nearby villages by administration. As per media reports, 30 fire tenders reached to spot
on time.
National thermal power corporation
On 1 November 2017 an accident had been occurred in NTPC power plant in Raebareli.
Accident occurred in the ash-handling section of the plant. Explosion takes place inside
the boiler causing cinder and steam to gush out thereby fatally injuring the workers. The
explosion kills at least 20 people and around 100 injured. As per NTPC statement a
sudden abnormal sound was heard around 3:30 pm at a new 500 MW unit. The Central
government sent an NDRF team at the spot to handle the situation.

ONGC
On 3 September 2019 a fire broke out at the oil and gas processing plant in Navi
Mumbai. 3 people of CISF fire and safety wing and plant supervisor were killed in this
accident. Fire has been doused by the fire services within 2 hrs. Gas has been diverted to
Hazira plant in Gujarat having no impact on processing plant.

NLC thermal plant


The incident took place due to the boiler explosion at an NLC thermal power plant at
Neyveli. The fire broke out at the TS 2 power plant resulting in burn injuries to two
regular employees and six contract workers. The boiler is 84 m high and when the
accident occurred, the workers and technicians were at a height of about 32 m.

Gulf Oil
The explosion occurred when staff were trying to dispose of chemicals at Gulf oil
corporation Ltd., which makes detonators. 2 workers died and 13 others were injured.
When they were disposing of rejected detonators at an open place adjacent to a shed.
Apparently, they were deactivating the detonators when suddenly the blast occurred.

Indian oil corporation limited


A fire broke out in a storage tank of the Indian oil corporation terminal at Hazira. 5
people were killed in that accident. The tank which caught fire had almost 5000 KL of
petrol in it, half of its capacity when it caught fire. The fire was brought under control
after more than 24 hrs.

BPCL
This was a tragic accident of explosion at the oil refinery at Kochi operated by Bharat
Petroleum Corporation Limited. The mishap happened at the electricity generation plant
at the oil refinery. It was suspected that an electric fire triggered the explosion as the
plant works on LPG. In this a contract worker lost his life in an explosion.
BPCL
A fire broke out at state-run Bharat Petroleum Corporation’s Mehul refinery in Mumbai
in which 40 people were injured. The accident took place at the compressor shed of the
hydrocracker plant, triggering an explosion. Eight people got seriously injured.

Cochin Shipyard
This accident took place when an explosion occurred in the forward area of a drillship
owned by ONGC which killed five people and injured seven others at the Cochin
Shipyard Limited (CSL). This happened shortly after a gas leakage was reported near the
AC compartment area of the ship. The explosion took place when the workers were
involved in cutting and welding work around the portable water tank of the ship.

Haldia petrochemicals
An unexplained accident at Haldia petrochemicals cracker complex at West Bengal state
in eastern India has left three people dead. This happened due to a gas leak in that
industry.

HPCL
In this accident at least 22 people were killed and 15 others injured at the 4.5 million ton
of crude in Hindustan Petroleum refinery in Visakhapatnam. The fire ignited after an
LPG pipeline leakage results in an explosion in the storage area of the refinery. About
70,000 people in the vicinity of the plant are said to have fled their homes in search of
safer places.

HPCL
A major accident occurred at state-owned HPCL refinery-cum-petrochemical complex
here Friday evening, killing 4 persons and injuring 39 people. The fire broke out due to a
blast in the cooling tower due to a short circuit. HPCL has shut down its daily production
to 166,000 barrels per day (bpd) Vizag refinery after the fire in the cooling tower.

HPCL
This accident happened when fire broke out in an LPG plant of Hindustan petroleum
corporation limited at Unnao district UP. 3 persons got injured in that accident. The
injured workers said the fire was caused by leakage of gas from a valve and soon it
assumed alarming proportion.

Pharmaceutical industry
Suven life sciences
Fire incidence occurred in the Suven pharmaceuticals on 26 April 2020. No one has been
killed or injured in this incident. At the time of the accident there were fewer staff
available on the site. People on site contained the fire in one portion until fire tender
arrived.

Pusilin Biotechnology
3 people killed including two Chinese nationals burnt to death when a boiler exploded.
Nuh SP said the incident took place around 3:30 pm while doing repair work on the
boiler. As per preliminary investigation the explosion occurred due to lack of proper
maintenance of the boiler.

Seutic Pharma
A major fire broke out in a pharma chemical company in the industrial area. 7 people got
injured in this accident. Hazardous material apparatus rescue vehicle has been reached on
the spot and saved the 20 workers from fire. Regional fire officers send the 8 fire tenders
at spot after receiving a call.

Dashmesh Medicare pvt. ltd


On 1 may 2012 a blast occurred in a pharma factory in Jalandhar. 4 people killed and 15
injured in a chemical reactor. A case has been filed against the MD of the company. Fire
was brought down under control by 2 dozen fire tender after 2.5 hrs.

Dr. Reddy's Laboratory


A major disaster was averted at Dr. Reddy’s Laboratories after a fire broke out at one of
its production units at Bachupally. Fire happened at a US FDA approved facility that
manufacturers finished drugs for exports. This accident took place due to a chemical
solvent leak sparked off a fire. No casualties in this accident.

Aurobindo Pharma
There was a fire accident in Aurobindo pharma Ltd. A flash fire accident occurred in the
company’s active pharmaceutical ingredients unit located in Andhra Pradesh. In this
accident 2 people died and 1 critically injured.

Smilax limited
2 people died and 2 others were injured in the accident in smilax labs in pharma city.
When 2 workers entered a room to clean it as there was a leakage of Methylene
Dichloride; which they inhaled and fell unconscious. Oxygen levels were also down in
that room and they died due to the MDC fumes and low oxygen.

Aurobindo Pharmaceuticals
This accident took place in which three workers sustained injuries in a fire which broke
out in the unit of Aurobindo Pharmaceuticals at Pydibheemavaram in Srikakulam district.
The fire was caused by an explosion in G-block due to high pressure in the vacuum dryer.
Fireworks industry
Sivakasi Factory explosion
On 5 September 2009 in the Sivakasi town of Tamil Nadu a blaze of accidents broke out
at a fireworks factory. Sivakasi town is known as the fireworks capital of India. As per
media reports several mishaps occurred in the past. 40 people died in 2009, 30 in 2001
and more than 60 in 1991. Every year 20-25 workers die each year in fire accidents. An
estimated annual turnover of the firework industry is more than $365m. In these factories
employees work in hazardous conditions resulting in the various health problems.

Regenesis Industries
This is an explosive industry and is a manufacturing unit where a chemical is being used
for manufacturing detonators exploded in one of the storage points at the unit at
Bommalaramaram. In this a worker died when he picked up the bottles containing
chemical (nickel hydrazine nitrate) used to manufacture detonators.
