E. Equipment Rental (Boom truck)
F. Occupational safety & Health Protection

The Approved Budget for the Contract is Eight Hundred One Thousand Three Hundred Seventy Five
and 29/100 Pesos Only (P801, 375.29)

Please attach with this proposal your valid documentary requirements (PhilGEPS Registration, PCAB
License – Specialty Electrical Works (SP-EE), DTI/SEC Registration, Latest Income/Business Tax
Return, Mayor’s/Business Permit and Omnibus Sworn Statement).

Contract duration: Forty Five (45) calendar days.
