CONTRACTOR'S AFFIDAVIT

1. I ALVIN D. VALES, of legal age, Filipino, residing at Diversion Road, Brgy. Macabog, Sorsogon
City, Sorsogon after having duly sworn to in accordance with law , hereby depose and state that:

2. That I am the owner of Araphil Construction and Development Corporation was awarded to
undertake the SUPPLY OF LABOR AND MATERIALS FOR THE REPLACEMENT OF BURNT WIRE,
PANELBOARD AND CIRCUITE BREAKERS OF SORSU DORMTEL AT SORSOGON CITY CAMPUS.

3. That I am the contractor as to this date for the said project is requesting for progress billing;

1. That I have paid partially all laborers employed in this project; paid partially all supplies and
materials;

2. That I have paid all government taxes due for this project;

3. That I did not give or intend to give gifts to any official or employees who intervene directly or
otherwise taken part this contract;

4. That I am executing this affidavit for the purpose of collecting payment for the work I have
accomplished.
