I, the appointee of hereby appoint

ensure that all stages of set up, event and strike are run in terms of the Occupational Health and Safety act and the

SASARE act, and is properly managed and controlled.

In terms of this appointment you are required to ensure that all work and occurrences is performed in accordance

with the requirements of the Occupational Health and Safety Act and in particular with the Safety at Sports and

Recreational Act in terms of which you must ensure, amongst others, the following:-
1) That all work is performed by suitably trained and competent persons;

2) That all statutory appointments have been completed and all statutory requirements are met;

That, where required, a VOC is established, and that meetings are accordingly held and that the minutes of

such meetings are kept and properly filed;

4) That all persons are aware of the hazards attached to the work being carried out and are fully conversant

regarding the precautionary measures to be taken or observed to obviate such dangers;

5) That the required risk assessments are carried out;

That discipline is enforced at the site at all times; and

7) That any other interests, in terms of health and safety with respect to the responsible area, are met.

You are required to report any deviations of the above-mentioned instructions, and which cannot be rectified by
TE

yourself, to me immediately and include any such deviations in the written weekly report which must be submitted
to reach me not later than close of business every Friday.
