We received your request to review your case for payments you made directly to KATHERINE
CAMPBELL. In order to process your request, write the date, the amount of the payment, and how the
payment was made (check, money order, cash, etc.) on the attached worksheet.

1. Complete the affidavit and total up all the payments

2. Sign and date the affidavit
3. Attach, proof of payments made (copy of cashed checks, western union receipts, statements,
etc.)
4. The proof must contain the name of the person the payments were made to

Send all the information back to our office within the next 20 days. Once we received the required
information, your case will be reviewed. We will confirm with the other party on the case if direct payments
were received. The other party on the case must agree in order to proceed with your request. We will
provide you with the outcome of our review within 30 days. We do not give credit toward any delinquencies
or arrears that are due to the State of Florida. There’s no guarantee credit for direct payments will be granted.
This process does not cease enforcement actions on your case.

We must be able to accurately monitor the payments on your case. If you continue to pay any child
support directly to the other party, you would need to get help outside our office for any future credits.
