1 Original Valid and recent NBI Clearance
2 Medical exam (Package A)
3 2 pcs. 2 x 2 ID picture( white background )
4 4 copies of Photocopy of Birth Certificate (PSA )
5 4 copies of Photocopy of Birth Certificate of Dependents (PSA )
6 4 copies of Photocopy of Marriage Contract (PSA ) if applicable
7 Photocopy of SSS-E1 or Employee Static Information
Photocopy of SSS loan balance print out (Salary Loan, Calamity Loan, Loan Restructuring
8 Program) - Regardless of no loans, you still need to submit the loan verification.
Copy of Pag-ibig loan details -Certificate of no Short Term Loan (STL) or Statement of Account

(SOA) regardless if you do not have any loans
10 Photocopy of Pag-ibig ID/number or Pag-ibig Loyalty Card
11 Photocopy of Philhealth ID/number or MDR
12 Photocopy of Tax Identification Number (TIN) , ID or 1902 or 2305
13 Photocopy of ITR BIR 2316 (for those with previous employers) within the same year
14 Copy of Transcript of Records/ DIPLOMA
15 Certificate of Employment and clearance from previous employer/s (Latest)
Google Map of permanent and provincial residence

All requirements should be submitted to HR on or before June 3, 2024. Kindly contact HR if you need some
clarifications/concerns regarding processing of the requirements. Thank you.

Once submitted completely to HR, your orientation will be scheduled on June 3, 2024.
