Presented by Colleen LeBlanc
Meet My Patient!
My case study patient is a 31-year-old woman who recently relocated to the United States
from Oaxaca Mexico after discovering she was born in San Diego. Accompanied by her
estranged partner, she sought shelter with a kind Samaritan, amid suspicions of intimate
partner violence. Her two children, aged 12 and 14, remain in Mexico under their
grandparents’ care.

Despite uncertainties, Erika does her best to take care of her oral health, including regular
cleanings in Mexico, though comprehensive radiographs were not part of her prior dental
care.

This initial dental encounter in the U.S. presents an opportunity to establish a dental home,
marking the beginning of her oral health journey in her new home!
Medical History

Medical & Dental Indications
Depression can impact oral health in various ways:
Neglect of oral hygiene: Depression may lead to neglect of
oral hygiene, increasing the risk of cavities and gum disease.
Bruxism: Stress-related bruxism can cause tooth wear and
jaw pain. Pt had very stron masseters
Dry mouth: Some depression medications can cause dry
mouth, raising the risk of tooth decay and gum disease.
Poor diet: Changes in diet associated with depression can
contribute to tooth decay and erosion.
Avoidance of dental care: Individuals with depression may
avoid seeking dental treatment, allowing dental problems to
worsen.

many x-rays” when the FMX was being
performed. She stated “They only did x-rays if
there was a problem with the tooth”

Summary of Risk Assessments

The patient is at moderate risk for caries due to her

medication, FLUoxetine (PROZAC) which puts her at risk
for xerostomia, diagnosed depression, and heavy levels
of calculus
The patient states that she does not typically snack
between meals and does not consume coffee, juice or
sodas
Strong masseters and malocclusion present the risk of
attrition
Patient brushes her teeth 2x daily and tries to floss daily
Perio: Gen 3-4 mm pockets with localized 5
mm pockets on her posterior teeth

Treatment Revisions
The patient completed 3 out of 4 quadrants of comprehensive
non-surgical periodontal therapy (NSPT). Despite multiple
attempts to reschedule, she cancelled her final appointment
twice and did not respond to follow-up efforts.

Although significant progress was made in improving her oral


hygiene skills and disrupting the calculus formation cycle, the
incomplete treatment poses a risk for disease recurrence and
compromised long-term periodontal stability.
