JOINT AFFIDAVIT OF COHABITATION

WE, NOEL C. BONGHANOY AND MARY ANN Z. CASTILLO, both of legal ages, Filipinos,
single (living together) and residents of Brgy. Cruz, San Carlos City, Pangasinan, after having
been duly sworn to in accordance with law, do hereby depose and state:
1. That we have been living together as husband and wife under the same roof,
continuously and without any interruption, since November 2009 or a period of 13
years;
2. During our cohabitation we are blessed with a son named Lord Ivan Lawrence
Castillo Bonghanoy born on February 13, 2011, attached hereto is a copy of the birth
certificate of our son;
3. Up to now, we are living together, we remain both of single status, hence there exists
no legal impediment for us to marry each other;
4. We are executing this affidavit to attest to the foregoing facts and for the purposes of
contracting marriage without the need of securing the marriage license pursuant to
the provisions of Article 34 of the Family Code of the Philippines.
IN WITNESS WHEREOF, WE hereunto set our signatures this 11th of July 2023 at San
