AFFIDAVIT OF ONE AND THE SAME PERSON

I, JIMMY BOY S. SEEZ, of legal age, Filipino and a resident of #23 Quintong,
San Carlos City, Pangasinan, after having been sworn in accordance with do
hereby depose and say:

1. That my Complete and True name is JIMMY BOY S. SEEZ and as proof,
hereto attached is my Birth Certificate as Annex “A” and made an
integral part of this Affidavit;
2. I have been using the name JIMMY BOY S. SEEZ in all of my transactions
and dealings such as Government Identifications, some of which are my
Passport and Driver’s License, as proof, hereto attached are my Passport
and Driver’s License Annexes “B” and “C” and made an integral part of
this Affidavit;
3. It was only during the time that I need to secure a School Diploma that I
discovered that my recorded full name is JIMMY BOY S. SAEZ instead of
JIMMY BOY S. SEEZ as appearing in my Birth Certificate which I am using
since childhood;
4. I am executing this Affidavit to attest that JIMMY BOY S. SEEZ as
appearing on my Birth Certificate AND JIMMY BOY S. SAEZ as appearing
on my School Diploma are ONE AND THE SAME PERSON.
5. I am executing this affidavit to attest to the truthfulness and honesty of
the foregoing facts and for all intents and purposes it may serve.
