JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JESSIE M. RESUELLO, ___ years old and MA. VICTORIA P. PARIS, ___ years
old, both Filipinos and residents of Brgy. Palaris, San Carlos City, Pangasinan, after having
been duly sworn to in accordance with law, hereby depose and state:
1. That we both personally know the person of CARLITO POQUIZ LAVARIAS, over
a long period of time because we live in the same barangay, and his family is a
close family friend for several years now;
2. That we also know and we hereby declare that the said CARLITO POQUIZ
LAVARIAS is married to DIPLOMA LAVARIAS;
3. That Carlito P. Lavarias from the record of the GSIS born in the year 1953 and
the Carlito P. Lavarias born on October 30, 1952 is one and the same person;
4. That we know for a fact that the said person by the name of CARLITO
POQUIZ LAVARIAS was born on October 30, 1952;
5. That we are not related in any way, either by consanguinity or affinity, to the
said CARLITO POQUIZ LAVARIAS NOR TO HIS WIFE DIPLOMA LAVARIAS;
6. As such, we execute this Affidavit to certify or attest to the truth of the
foregoing facts and for whatever legal purpose that this Affidavit may serve.
IN WITNESS WHEREOF, we have hereunto set our hands this 23 rd day of August
2023 at San Carlos city, Pangasinan, Philippines.
