AN EXECUTIVE ORDER ESTABLISHING THE BARANGAY REHABILITATION REFERRAL DESK AND

DESIGNATING THE BARANGAY DUTY OFFICER.

WHEREAS, Section 3 (A) of DDB Regulation No. 4, series of 2016 and DILG Memorandum
Circular No. 2017-151 dated November 10, 2017, mandates the barangay to designate a Duty
Officer who’s responsible in the proper documentation of the process of voluntary surrender;

WHEREAS, Section A (7) of DDB Regulation No. 3 series of 2017 provides for the establishment
of a Rehabilitation Referral Desk, designation of duty officer, organization o House Cluster and
designation of cluster leaders;

NOW THEREFORE I, HENRY D. DIONIO, Punong Barangay of Barangay Rawis, by virtue of the
power vested in me by laws, do hereby order:

SECTION 1: ESTABLISHMENT OF BARANGAY REHABILITATION REFERRAL DESK – There shall be

a Rehabilitation Referral Desk Anti-Drug in the barangay with the Duty Officer as the Officer in
Charge.

SECTION 2: DESIGNATION OF BARANGAY DUTY OFFICER – HON. AVARQUES S. ORSAL is

designated as Barangay Duty Officer.

SECTION 3: DUTIES AND FUNCTION OF THE BARANGAY DUTY OFFICERS – Designated duty
officer shall perform the following duties and responsibilities:

1. Man the Rehabilitation Referral Desk;

2. Conduct interview to solicit personal information from the surrenderer;
3. Immediately inform the Punong Barangay in case the surrenderer is included in the
Target List, Wanted List and Watch List Personalities of law enforcement agencies, such
as but not limited to PDEA, PNP, NBI or has any other pending criminal case;
4. Log and maintain a record book containing all personal information of surrenderers;
5. Ensure confidentiality of all information of surrenderers;
6. Monitor progress of surrenderers;and
7. Perform other duties as may be assigned by the BADAC Chairman.

SECTION 3. EFFECTIVITY – This order shall take effect immediately.
