A RESOLUTION FAVORABLE REQUESTING THE LAND BANK OF THE PHILIPPINES (LBP)

MAMBAJAO BRANCH, POBLACION, MAMBAJAO CAMIGUIN TO CLOSE OUR COMMUNITY
BANK ACCOUNT WITH THE ACCOUNT NAME DSWD KC: KKB CFW TAGDO, MAMBAJAO AND
WITH THE ACCOUNT NUMBER 0992-1092-89

WHEREAS, the Barangay Tagdo, Mambajao, Camiguin was chosen as one of the beneficiaries of
the said Sub – Project of KALAHI – CIDSS KKB CFW.

WHEREAS, the fund released to Barangay Tagdo, Mambajao, Camiguin Community account already
disburse as payment to obligations and no other transactions to be done.

WHEREAS, there’s a need to close the Community bank account and turn over officially the
remaining unused check booklet to official depository bank.
NOWTHEREFORE, be resolved that the people of Barangay Tagdo, Mambajao, Camiguin together
with the Barangay Officials DSWD KALAHI CIDSS KKB CFW BARANGAY TAGDO, MAMBAJAO,
CAMIGUIN humbly Requesting LBP Mambajao Branch, Poblacion, Mambajao, Camiguin to close the
Community bank account of DSWD KC: KKB CFW.

RESOLVED FURTHER, let the copies of this resolution be furnished to the office of LBP Mambajao
Branch, Poblacion, Mambajao, Camiguin, office of DSWD Region X, Mayor’s office, and Barangay
Tagdo, Mambajao, Camiguin.
