I. Description of Goods

The exporter confirms having concluded this contract with the importer convering the under
mentioned merchandise on the terms and condition state here under and on the reverse. The
importer is hereby requested to sign and return the original attached here to, and if any
discrepancy found by the importer, the exporter should be informed immediately by cable.

of Lading. T/T To : Bank Mandiri A/C : 111111111 (PT Billion Dollar Ekspor)

IV. Document Required

Billion Dollar Ekspor

The following documents should be rendered by exporter in triplicate which should be
airmailed to the importer

1. Full set clean on board ocean bill of lading and non-negotiable bill of lading
2. Signed commercial invoice in triplicate
3. Certificate of Origin issued by Indonesia Trade Department
4. Signed packing list in duplicate
