Community Level e-OPT Tool

For Children 0 - 59 Months Old
Based on the WHO Child Growth Standards

This tool is for use by nutrition workers in the community to help them record, interpret, and
summarize results of the OPT Plus -- an annual activity that is done in barangays across the country
to measure the nutritional status of children 0-59 months old. The e-OPT Tool runs on MS Excel
version 2007 or later. Full functionality of the tool requires a licensed version of MS Excel.

GETTING STARTED

If you are using the e-OPT Tool for the first time

Go to the 'Nut_StatusTool' worksheet. Fill up the necessary information in the headings. These cells are
highlighted in green.

Before entering child data, familiarize yourself with some important reminders for data entry (see
below).
Refer to the paper forms (i.e. NNC Form 1A) that you used to record details of preschool children from
your last OPT Plus activity. Type in all the required data for each child into the 'Nut_StatusTool'
worksheet one at a time. One child, one row.

If your barangay has used the e-OPT Tool previously and you wish to update and complete your e-OPT master
list

Open the e-OPT Tool file that contains the entries from the most recent OPT Plus done in your
barangay.

Go to the 'NewEntry&Update' worksheet from your last OPT Plus. Delete any duplicate entries and the
names of children older than 59 months. These are the rows that are highlighted in red. A step-by-step
description is outlined in the 'Tips for Using the 'Clean&Update' worksheet shown below. Note: the
'NewEntry&Update' worksheet in earlier versions of the e-OPT Tool has been renamed to
'Clean&Update'.

Get the latest version of the e-OPT Tool. Into a blank 'Nut_StatusTool' worksheet, copy and paste
(starting from Row # 10) the names and details of age-eligible children from the cleaned
'Clean&Update' worksheet. Always use 'Paste>>Values' and not the simple 'Paste' option. This copy and
paste step saves you from having to re-type in most of the names and details of children that have
already been encoded previously.

At the end of the list that you copied from existing e-OPT files, type in the names and details of new or
previously unlisted children in the barangay. This should include infants born in the past year, new
residents in the barangay, and those who were not included in past OPT Plus.

For each child in the list, note any missing or incomplete details, so that the needed information can be
obtained by the nutrition worker from mothers or caregivers during the next house visit.

If your barangay already has e-OPT records and you are preparing for an upcoming e-OPT Plus activity

Open your barangay's most recent e-OPT Plus file that contains the list of preschool children and their
details.

Go to the 'BNS_Printout worksheet in that file. Type in the date of the upcoming OPT Plus or house visit
in cell highlighted in green (Cell J2).

Make a printout of the 'BNS_Printout' sheet. This serves as your pre-printed list of preschool children in
your barangay which you can use to find age-eligible children as you make the rounds in the
community. Names of preschoolers are alphabetically arranged to help you match children and names.
You should also write down the names of new or previously unlisted children at the end of this list.

Key Features of the Tool


NEW IN THIS UPDATE:

This update now includes auto-fill of population data in the Summary worksheet, as well as the auto-labelling of
the Philippine Standard Geographic Code (PSGC) for each barangay. Population data (i.e. total barangay
population, 0-59 months children) are from DOH - Epidemiology Bureau estimates for 2021.


The counts of mothers/caregivers of children classified as having undernutrition as well as overweight and obesity
have been included in the Summary. This counts the number of mothers/caregivers who have at least one child
whose nutritional status falls below the lower cut-offs for normal WFA, HFA, or WFL/H and those who have
children classified as overweight or obese (WFA or WFL/H). These counts are intended to help LGUs in nutrition
program planning.

Note: if a mother/caregiver has more than one child affected, she/he is counted only once.

A count of the number of children with either the weight or height measurement missing or those with
incomplete information has been included in the Summary. For 'incomplete information', this reflects the number
of children who are missing any of the following: home address, name of mother/caregiver, IP status, sex, date of
birth, or date of measurement. To help users in spotting the children with missing details in the 'Nut_StatusTool',
the name is flagged with a gray colored cell in the 'Child Seq.' column (Column A). Also,iIf any of these information
(except for home address) are not filled in, this will result in non-appearance of the child's nutritional status.

• Additional columns have been added for children 0-23 months old in the Summary table to show the total
number and prevalence in each of the three nutritional status indices WFA, HFA, and WFL/H.

• The tally of children belonging to indigenous peoples (IP) groups according to their nutritional status classification
in each of the three indices has been added to the summary table. The total presented is also disaggregated by
sex.

Nutritional Status Calculator. By typing in the child's weight and height, as well as the date of birth and the date
of measurement, the tool quickly calculates a child's age in months and auto-generates the nutritional status. The
classification is based on the WHO Child Growth Standards for the three indices weight for age, height for age,
and weight for length/height. After all the children's measurements are taken, these are recorded then later fed
into the tool by the nutrition worker or others in a local facility with access to a computer. This automatic feature
is intended to make it quicker for nutrition workers to collect and interpret the measurements of many children in
the community. This could also lessen the chances of errors compared with the previous approach where -- after
each child's measurements are taken -- the nutrition worker had to manually look up each child's nutritional
status from three kinds of printed reference tables, write down each result in a recording sheet, and repeat the
process for all children visited.

Updatable Database. The need for re-typing all the names of preschool children in the barangay and entering
their details after every annual OPT Plus activity can be avoided, as the e-OPT Tool Template can just be updated,
exported, renamed, and saved into a new file every year, or as often as needed.

Lists of affected children are auto-generated. The tool was designed to make tabulation and report preparation
by nutrition workers less tedious and more timely. This feature is expected to significantly lessen the amount of
time previously spent by nutrition workers in preparing OPT Plus reports using the old paper-and-pen method. It
also lessens the chances for manual calculation errors and discrepancies during the tabulation of summary
Lists of affected children are auto-generated. The tool was designed to make tabulation and report preparation

time previously spent by nutrition workers in preparing OPT Plus reports using the old paper-and-pen method. It
also lessens the chances for manual calculation errors and discrepancies during the tabulation of summary
results. With faster turnaround time, children found to have moderate and severe malnutrition in any of the
three indices are can now be identified and listed separately so that these children can be prioritized, visited or
referred, and given appropriate treatment or support by the LGU. The lists of affected children generated by the
tool can also be copied and transferred to another MS Excel worksheet. This allows users to create more detailed
or customized monitoring formats depending on the needs of health and nutrition workers in the locality.

Adaptable for use in small or large barangays. The tool has a capacity of 1000 child entries. For barangays with
more than 1000 preschool children (i.e. 0-59 months old), users can also apply this tool for smaller parts of the
barangay such as Puroks, Blocks, or Areas. The data from the sub-part of the barangay can then be aggregated
and summarized as a single file using another e-OPT Tool that consolidates the barangay data (e-OPT Tool 1A - 'e-
OPT Tool for Large Barangays').

Avoids double-counting of children. During community surveys, names of children can be inadvertantly included
in the OPT list more than once. To avoid errors in tallying and reporting the data, the tool has been set up to
automatically exclude duplicate names and the numbers from these repeated entries are not included in the
totals in the summary results. Once these names are validated to be from one individual child, users can then just
delete the repeated names later when they clean or update the master list of children in their community.

Before you Begin - Important Reminders for Use of the Nut_StatusTool Worksheet:
As a first step, press CTRL + F1 or CTRL Fn + F1. Doinhg this will hide the ribbon above the worksheet and will give
you more working space as you enter data. You can toggle this on or off. Use CTRL + Fn + F1 for keyboards or
laptops that have a Fn key.

The Nut_StatusTool is the main data entry worksheet. You can enter information one child at a time into each row
or you can do batch entry for many children (if you already have previous worksheets) by using 'copy>>paste
values' from another file.

• DATE FORMAT TO USE IN ENTERING DATES. First determine the date format settings on your computer at the
bottom right corner of your screen on your taskbar (or you can look at the date format in cell K1 in
'Nut_StatusTool'). For example, if your computer's date format is MM-DD-YYYY, "January 21, 2021" would appear
as "01/21/2021"; if it is set as DD-MM-YYYY, it would appear as "21-01-2021".

IMPORTANT: The way you type in the date format into the tool for the child's date of
birth and the date of measurement must be the same as the way the date format in
your computer's operating system appears (e.g. MM/DD/YYYY or DD/MM/YYYY).
Always type in any date using the format consistent with your computer's settings.
When you type in the numeric date, make sure that the number for the month and the
date are not interchanged, otherwise, this will lead to incorrect calculation of the
child's age in months. After you have typed in the numeric date, it will then appear in
the cell as MMM-DD-YYY (for example: Jan-21-2021), regardless of your computer's
date format settings. This is to help users in reading the correct dates after the date
has been typed in. Please also check that the date and time settings of your computer
are always correct.

• FILL IN ALL THE REQUIRED INFORMATION IN THE HEADINGS SECTION. Before you start entering children's data,
first fill up all cells that are highlighted in green. The data entry worksheet will not appear if there are any green-
highlighted cells left blank. Filling up these cells ensures that the headings in all tables and reports in other parts
of this tool are filled in.

Type in the YEAR (cell O1). Choose first the PROVINCE/NCR LGU, then the CITY/MUNICIPALITY, and
lastly the name of the BARANGAY from the dropdown lists in each corresponding cell. Please fill these
up in sequence.

If the message "Note: Large Barangay" appears in Cell C3, you should use this file for a Purok or smaller
part of the barangay. In Cell A3, choose the "Purok/Area/Block" option. Please then type in the name of
the Purok or sub-part of the barangay in Cell C6.

• ENTER DATA FOR EACH CHILD. You can then begin entering information for each child in the main table -- one
child at a time -- starting from the first blank row (Row # 10). Or, if you already have an encoded worksheet with
e-OPT data from previous use of the tool, you can batch copy the list of multiple children's names and details into
the blank Nut_StatusTool using the "Copy>>Paste>>Values" function of MS Excel. Please just note that the
children's names and other information from the source worksheet should be arranged in exactly the same way
the columns in the tool are set up. Also make sure that the source data are pasted into the blank tool starting
from Row #10 downwards.
• ENTER DATA FOR EACH CHILD. You can then begin entering information for each child in the main table -- one
child at a time -- starting from the first blank row (Row # 10). Or, if you already have an encoded worksheet with
e-OPT data from previous use of the tool, you can batch copy the list of multiple children's names and details into
the blank Nut_StatusTool using the "Copy>>Paste>>Values" function of MS Excel. Please just note that the
children's names and other information from the source worksheet should be arranged in exactly the same way
the columns in the tool are set up. Also make sure that the source data are pasted into the blank tool starting
from Row #10 downwards.

• ENSURE ALL CELLS OR INFORMATION ARE FILLED IN FOR EACH CHILD. To auto-generate each child's nutritional
status for all three indices, the following information must be complete: the name of the mother/caregiver, the
child's name, IP status (whether or not the child belongs to an indigenous group), child's sex, date of birth, date
measured, weight (in kg), and height (in cm). To facilitate follow-up visits by nutrition workers, the address or the
location in the barangay where the child lives should also be entered. This can be the name of the purok, block, or
name of the part of the barangay. If the child is missing any essential information (e.g. name of
mother/caregiver), type in "TBD", and make sure to obtain the needed informationduring the next house visit.
Otherwise if any details are left blank in the tool, one or all nutritional status indices for that child might not
appear. To help users of the tool to quickly see if a child has no weight and/or height entered in the
Nut_StatusTool, the cell for the corresponding measurement that is missing will be highlighted in yellow. If other
child information is missing, the cell opposite the child's name in Column A will be highlighted with a gray color.

• IMPORTANT: Always start entering child data in Row # 10. After you have started data entry in
the Nut_StatusTool, do not skip rows or leave any rows blank -- this will cause your BNS Printout
and the Clean&Update worksheets to not work properly. It will also result in sorting problems in
your other worksheets and your reports will not appear correctly.
• Reminder: When copying from another worksheet into the e-OPT Tool, always use the option "Paste>>Values".
Doing this avoids errors that might happen as underlying formulas and formats in cells from the source will also
be copied if just "Paste" is used.

Instructions for Using the 'Clean&Update' Worksheet:

This worksheet enables you to delete names from the list of children in your OPT Plus database, based on the
most recent house-to-house survey. By deleting duplicate names and children who are already older than 59
months as of the current date, you can "clean" and update the list of preschool children in your barangay or
purok/ Street/Block# (depending on how this tool is being used).

• Scroll down the list of children. Children whose names are highlighted in red across the whole row are those
that have been entered more than once. Once you verify that the names highlighted in red are duplicates of the
same child, you can delete the extra names to ensure that the name only appears once in the list. However, a new
feature of this tool is that it does not count names of children that appear more than once and excludes them
from the total, as an added precaution against duplicate counts.

• Children who are older than 59 months are also highlighted in red. You can also remove these names from the
list as you clean and update your list of 0-59 months old children.

HOW TO REMOVE NAMES OF CHILDREN THAT ARE DUPLICATES OR THOSE MORE THAN 59 MONTHS OLD

1. Point your cursor to the row number on the leftmost side of the screen to the left of column A. Once you see a
black arrow pointing to the right, click on the mouse once to highlight the entire row you wish to delete.

2. Right click on your mouse to see the menu of options. Go to 'Delete' and click on your left mouse button to
delete the row. DO NOT use the 'Delete' key on you keyboard.

Once there are no more remaining rows highlighted in red, you can then proceed to copy the contents of the
entire worksheet under columns B through G.

If you have already encoded the names of the children in your barangay into the e-OPT Tool previously, this should
save you from having to re-type in the names of most children in your barangay again the next time you do
another OPT Plus activity. Subsequently, only the names and details of new children will need to be added and
typed in after each annual OPT Plus.

COPYING THE CLEANED MASTERLIST TO YOUR CLIPBOARD

1. Hold down your left mouse button to highlight all the content of the rows and columns with children's names
and details. Copy all rows, columns, and cells that are highlighted in yellow. After highlighting all the cells for
copying, click on your right mouse button to expand the menu of options.

2. After right-clicking on your mouse, go to the 'Copy' option to put the contents of the worksheets cells you wish
to copy onto your clipboard.
TRANSFERRING THE CLEANED LIST INTO A NEW BLANK e-OPT TOOL

3. Get the latest version of the e-OPT Tool from the NNC website. Open a blank copy of the e-OPT Tool and
transfer the contents of what you copied in the previous step into the blank 'Nut_StatusTool' worksheet.

4. First place the mouse cursor in cell B10. Then paste the contents of the cells copied from the "Clean&Update'
(formerly 'NewEntry&Update') source worksheet.

Note: To paste, right click on your mouse and always choose the "PASTE VALUES" option. (Caution: if you use just
the plain "Paste" option, you will be copying not only the content of the cells but also any underlying formulas
and formatting that came from the source worksheet. This often leads to copying errors and incorrect data.)

ADDING NEW NAMES, DETAILS, AND MEASUREMENTS

5. Type in the names of new children (those born during the past year or those who are new residents in the
barangay) at the end of the list of old names in the 'Nut_StatusTool' worksheet.

6. Determine the actual date when the child was measured and type this into the 'Date Measured' cell (column
H).

7. Type in the latest weight and height measurements of children. Make sure to enter these under the
appropriate columns and spaces in the 'Nut_StatusTool'.
Printing Instructions
There are 10 print-ready worksheets in this tool. These are:

• Summary • BNS Printout • OPT Form 1A • OPT Form 1B • List UW (Underweight) • List St (Stunted)

• List W (Wasted) • List SUW (Severely Underweight) • List SSt (Severely Stunted) • List SW (Severely Wasted)

The printable worksheets have been formatted for A4 size paper, Landscape orientation. However, you can also
use other paper sizes, depending on what is available in your LGU. You will just need to adjust the page margins
and scaling settings as described below.

PAGE SETUP

1 - Click on the Page Layout tab on your menu. Then click on the small arrow at the bottom right corner of the
Page Setup group. The Page Setup Dialog Box with 4 tabs (Page, Margins, Header/Footer, and Sheet) should
appear.
SET PAGE ORIENTATION

2 - Click on the 'Page' tab. Under 'Orientation', choose 'Landscape'. At the bottom part of the 'Page' tab, set your
paper size by choosing from the dropdown list.
SET PAGE MARGINS

3 - Click on the 'Margins' tab. Set the margins according to the paper size you are using. Apply the following
margin settings listed below:
Note: These margin settings allow for placement of fasteners or binders for the printed sheets on top for Letter
and A4 paper sizes and on the left for Long and Legal size paper .

SCALING

4 - Click on the Page tab once again. Here you can adjust the scaling for your printout. Depending on your printer
settings and the paper size you are using, set the scaling to any value between 80 - 100%. Click on Print Preview to
see if the scaling settings you selected will make your printout fit properly in your chosen paper size. If not, adjust
the scaling accordingly.
4 - Click on the Page tab once again. Here you can adjust the scaling for your printout. Depending on your printer
settings and the paper size you are using, set the scaling to any value between 80 - 100%. Click on Print Preview to
see if the scaling settings you selected will make your printout fit properly in your chosen paper size. If not, adjust
the scaling accordingly.

SPECIFY THE NUMBER OF PAGES TO PRINT


6 - Click on File >> Print. The Printer Dialog Box/Settings should appear. Set the number of page to print in Page
Range and select Active Sheet(s) under 'Print what'. (E.g. Pages ___1__ to __xx__).

TIP: Take note of the total number of pages to print as indicated in the worksheet you are printing. For example, in
the lists of children listed as UW,St,W, SUW, St, and SW, this number is shown in cell J5. For the BNS_Printout, the
number of pages for printing is shown in cell J4. The Summary worksheet is only 1 page.
Note: Specifying the total number of pages to be printed avoids pages without names being printed inadvertently
and paper wastage.

7 - Click on the 'OK' or 'Print' button when ready.
