CHECKLIST OF DOCUMENTARY REQUIREMENTS


FOR ORIGINAL APPOINTMENT OR RE-EMPLOYMENT IN THE LOWER COURTS

(x) Favorable recommendation of the Presiding Judge (for vacant positions in the
branch)

(x) Indorsement of the Executive Judge (for vacant positions in the branch)

() Favorable recommendation of the Executive Judge (for vacant positions in the Office
of the Clerk of Court of a multiple sala court)

() Comment of the Clerk of Court on the application (for vacant positions in the Office
of the Clerk of Court of a multiple sala court)

(x) Three (3) copies of properly, completely and recently accomplished Personal Data
Sheet (CSC Form 212, revised 2005) with latest 3.5 cm x. 4.5. cm. (passport size)
pictures

(x) Authenticated copy of Eligibility from the Professional Regulation Commission and
official receipt of payment for authentication (2 copies) or authenticated copies of
Eligibility authenticated by the Professional Regulation Commission
(for positions requiring civil service eligibility)

(x) Certificate of Marriage issued by the Philippine Statistics Authority (PSA), 2 copies
(for married women)

(x) Birth Certificate issued by the Philippine Statistics Authority (PSA), Original copy

(x) Certification under oath by the Clerk of Court (COC in the OCC for vacancies in the
OCC or COC in the branch for vacancies in the branch) that the position to be filled
up has been posted in three (3) conspicuous places in the court’s premises and other
public places for a period of at least ten (10) consecutive working days pursuant to
RA No. 7041, indicating the date when the notice was posted and the public places
where it was posted, pursuant to OCA Circular No. 74-2010

() Written manifestation of the applicant's willingness to accept and be appointed


under a temporary appointment if he/she does not meet the experience and training
requirements (for positions requiring work experience and/or training)

(x) Proof of completion of Secretarial/Stenography course or units in Stenography for


Stenographer position

(x) Report on the result of the competency examination together with a copy of the
shorthand notes and transcription thereof, pursuant to OCA Circular No. 124-2004
(for stenographer positions only)

(x) Transcript of School Records authenticated by the School Registrar or other


authorized official.
(x) Sworn Affidavit executed by the applicant that he/she has no relative(s) within the
third civil degree of consanguinity or affinity who is an incumbent employee in the
branch/office

(x) Certification under oath by the Presiding Judge/Acting Presiding Judge that the list
submitted contains the name/s of all the applicant/s who applied for the position.

() Certification issued by the Clerk of Court duly certified by the Presiding


Judge/Executive Judge as to the applicant's knowledge and proficiency in the use and
operation of computer, for Data Entry Machine Operator II position

() Bar Rating and Certificate of Membership in the Philippine Bar (for Clerks of Court
requiring lawyer positions)

(x) Latest Medical Certificate (CSC Form No. 211) with attached original copy of
laboratory results; in case the same could not be submitted yet, the applicant should
submit for the meantime an affidavit that he/she is fit to work, subject to the
submission of the medical certificate and laboratory results if appointed)

(x) Latest NBI Clearance

FOR APPLICATION FOR PROMOTION WITHIN THE SAME BRANCH/OFFICE IN THE LOWER
COURTS

(x) Favorable recommendation of the Presiding Judge (for vacant positions in the branch)

(x) Indorsement of the Executive Judge (for vacant positions in the branch)

() Favorable recommendation of the Executive Judge (for vacant positions in the Office of the
Clerk of Court of a multiple sala court)

Comment of the Clerk of Court on the application (for vacant positions in the Office of the
() Clerk of Court of a multiple sala court)

(x) Three (3) copies of properly, completely and recently accomplished Personal Data Sheet (CSC
Form 212, revised 2005) with latest 3.5 cm x. 4.5. cm. (passport size) pictures

(x) Authenticated copy of Civil Service Eligibility from the CSC Regional Office and official receipt
of payment for authentication (2 copies) or authenticated copies of Board Rating and
Professional Regulation Commission (PRC) License from the PRC (2 copies each) (for positions
requiring civil service eligibility and the applicant has not yet submitted a certified copy of
her/his civil service eligibility to the OAS, OCA)

(X) Certification under oath by the Clerk of Court (COC in the OCC for vacancies in the OCC or
COC in the branch for vacancies in the branch) that the position to be filled up has been
posted in three (3) conspicuous places in the court’s premises and other public places for a
period of at least ten (10) consecutive working days pursuant to RA No. 7041, indicating the
date when the notice was posted and the public places where it was posted, pursuant to OCA
Circular No. 74-2010

() Properly accomplished Position Description Form (CSC Form No. 1)

(x) Certificate of Marriage issued by the National Statistics Office (NSO), 2 copies (for married
women)

(x) Proof of completion of Secretarial/Stenography course or units in Stenography for


Stenographer positions

(x) Report on the result of the competency examination together with a copy of the shorthand
notes and transcription thereof, pursuant to OCA Circular No. 124-2004. (for stenographer
positions only)

(x) Latest performance rating.

(x) Transcript of School Records authenticated by the School Registrar.

(x) Certification under oath by the Presiding Judge the the list submitted contain the name/s of
all the applicant/s who applied for the position

() Certified photocopy of proof of completion of relevant training/seminar in a training institution


accredited by the Civil Service Commission, 2 copies (if applicable)

(x) Sworn Affidavit executed by the applicant that he/she has no relative(s) within the third civil
degree of consanguinity or affinity who is an incumbent employee in the branch/office
(x) Certification under oath by the Presiding Judge the list submitted contain the name/s of all the
applicant/s who applied for the position

() Bar Rating and Certificate of Membership in the Philippine Bar (for Clerks of Court requiring
lawyer positions)

(x) Certification from present office on pending transcription of stenographic notes (for court
stenographers)

(x) Certification from present office as to money and property accountabilities and as to pending
criminal and/or administrative cases

( ) Written manifestation of the applicant's willingness to accept the authorized salary rate of the
position applied for, in cases of demotion in salary or in rank

(x) Latest copy of Service Record duly authenticated by the authorized officer of the issuing agency

(x) Latest Performance Rating duly certified by the authorized officer of the issuing agency.

(x) Copy of last approved appointment duly certified by the authorized officer of the issuing agency
