1. SCOPE
This Method Statement describes the preparation prior to Installation of Underground and
Aboveground Fuel Piping located at Lot PT 12165, Cyber Point 4 Road, Cyber 8, Cyberjaya, Mukim
Dengkil, Daerah Sepang, Selangor Darul Ehsan. This method of statement is developed to serve as a
guide and cover the activity and work related to the fuel piping installation.

Scope of work as listed below: -

1. Survey, setting-out & preparation.


2. Installation of Underground Fuel Pipe (HDPE).
3. Installation of Aboveground Fuel Pipe (Stainless Steel).
4. Check correctness installation of pipe according to approved shop drawing.

Remarks: These installations are applicable to all the related works in KUL14 Substation only. Refer to
drawings for the actual installation location.

5.2 Weather Protection
5.2.1 Adverse 1. Works will commence in a hot

3. No outdoor work shall be carried

works supervisor.
5. Follow Leighton’s safety procedure
during inclement weather. Workers
to be briefed on safety procedures
on a regular basis.
6. All workers must gather in the
designated. area only (e.g. rest
shelter) during adverse weather
7. Workers should remain in the
shelter until further notice from
Safety personnel.
8. Upon lightning detection, all
operation will be halted if lightning
is detected near the site.

during bad weather condition.

5.4.2 Transport, 1. The materials will be transported by
handling & lorry.
storage 2. Materials with large batches will be
unloaded using cranes/forklift. A
lifting permit will be obtained and
the lifting plan to be brief to all
relevant parties.
3. The outriggers shall be extended
before the unloading. Shall inspect
the ground and make sure of the
stability of the ground.
4. Place the outriggers pads and they
shall stay firm on the ground. The
outriggers pads size shall follow
Leighton PSS.
5. The lifting supervisor shall monitor
the lifting work and be alert to signs
of instability. If anything is
suspicious, stop work immediately
and correct it before resuming the
work.
6. The other lightweight materials
including accessories will be
unloaded using manpower.

HOLD POINT: The storage area for the


materials shall be discussed with LCM.
The materials will be covered to prevent
damage by weather

5.4.3 Material 1. Materials accessories will be


transferring transferred to the site by manual
to site handling.
2. Buddy system will be implemented
for carrying all the materials.

HOLD POINT: Heavy material shall be


carried by 2 people at the same time

5.5.1 Site 1. Construction equipment and
preparation & materials should be removed from
pipe the piping area
2. Pipe trenches should be dug out to
arrangement

3. Trenches should then be prepared
with bottom layer of 10-15 cm of
bedding material on which pipe are
placed
4. For minor adjustments of gradient,
bag with backfill material used to
placed under the pipe at minimum
1 meter intervals.
5. A minimum of two people needed
to uncoil the pipe.
6. Pipe up to 63mm in diameter can
be cut using pipe scissors. Keep the
bottom handle still and use the
upper handle to cut.
7. Pipe between 48mm to 125mm can
use rotating cutter to cut the pipes.

ACTIVITY ILLUSTRATIONS NOTES

5.5.2 Installation 1. The connection technique for


underground HDPE pipes involves a socket
fuel piping fusion joint achieved through the
use of a self-regulating
(HDPE pipe)
electrofusion handheld welding
machine, competently operated by
a qualified individual.
2. The fusion joint is carried out at
temperature approximately 200°C
3. Connect the machine to the
electrical power supply source.
When the green light starts to blink,
the welding temperature has been
reached and can proceed to start
the work
4. Ensure that the surface of the pipe
and fitting are clean and free of
grease and dirt.
5. Heat the pipe to the insertion
depth and the fitting to the socket
depth on the welding tool. When
the heating time elapsed, push the
pipe and fitting together smoothly
and evenly. The result is a very
strong and homogenous joint.
6. The position of the fitting can be
adjusted for a few seconds
immediately after the pipe and
fitting have been joined. Within a
short period of time the joint is
capable of withstanding a full load

wall first, then we will punch the C-
Channel to the wall and then use
bolts, nuts, and washer to support
the C-Channel, next will punch
holes the C-Channel to install U-Bolt
to further strenghthen the position
of the pipe for the vertical
installation.

6. CONSTRUCTION RISK ASSESSMENT


6.1. Risk Assessment
The following elements will be checked for defects or other risks on a daily basis:

1. Develop a comprehensive risk assessment and management plan that addresses potential
hazards and outlines procedures to mitigate those risks.
2. Conduct regular safety training sessions for workers and provide them with appropriate PPE.
3. Monitor delivery material and implement measures to prevent damage to the structure or
surrounding property.
4. Implement effective waste disposal and containment measures to prevent environmental
damage.
5. Conduct regular inspections and maintenance of equipment to prevent equipment failure

6.2. Fire-Fighting Measures


1. Implement fire prevention measures, including the provision of fire extinguishers, fire blankets,
and adequate ventilation in the work area.
2. Prohibit smoking and open flames near the fuel pipe installation site

6.3. Site Access Point


1. Site Assessment:

 Evaluate the layout of the construction site to identify potential access points for
delivery vehicles.
 Consider factors such as distance from the main road, terrain conditions, existing
infrastructure, and any obstacles (e.g., trees, buildings, utility lines).

2. Road Conditions:

 Assess the condition of roads leading to the site to ensure they can support the weight
and size of delivery vehicles.
 Identify any narrow or steep sections that may pose challenges for large trucks.

3. Temporary Access Roads:

 If necessary, consider creating temporary access roads or pathways to facilitate the


delivery.
 Ensure these temporary routes are properly graded, compacted, and stabilized to
prevent erosion or damage.

4. Traffic Management:

 Develop a traffic management plan to minimize disruptions to ongoing construction


activities and ensure the safety of workers and visitors.

 Establish designated areas for loading and unloading the underground tank to keep
traffic flow organized.

5. Safety Precautions:

 Implement safety measures such as signage, barricades, and flaggers to alert workers
and visitors to the presence of delivery vehicles.
 Ensure delivery personnel are trained in safe driving practices and are aware of site-
specific hazards.

7. ENGINEERING
All relevant drawings for these works are as stated in item 2 (Reference).

8. STORAGE AND HANDLING


1. Most of equipment/material to be stored off site. Equipment/material to be delivered to site
upon respective installation works ready to carry out.
2. Ensure the materials/equipment delivered to site are properly placed (e.g elevated from the
ground and stagnant water, protected from weather and possibility of damages, etc).
3. Shall stacking of materials/equipment required, not store in excess 1.5m in height, stopper
requirements etc.
4. Avoid the materials from excess moisture. Oxidation of the materials will cause the materials to
become sharp and cause injury.
5. Appropriate engineering controls: N/A
6. Individual protection measures, such as personal protective equipment (PPE)
7. Eye protection: Safety Goggles.
8. Skin protection: N/A
9. Hand protection: Cut Resistance Gloves.
10. Respiratory protection: N/A

8.1. Non-Hazardous Substances/Materials


All non-hazardous materials should be stored within site boundary and barricaded properly with
signages. Contractor shall appoint registered contractor for waste disposal management where
applicable. The Disposal will be disposed at contractor’s designated waste disposal area.

8.2. Hazardous Substances/Materials

9.1. Inspection and Testing


 To refer attached Approved Inspection Test Plan (ITP) for Quality Assurance & Quality Control.

9.2. Measuring Equipment


 To refer attached Approved Inspection Test Plan (ITP) for related inspection and testing on/off
site.
 All equipment which is being used should be in good condition & well-functioning.
 To ensure calibration certification validity and match with the equipment use.

9.3. Materials Control


 To refer project specification for related material handling & storage.
 To make sure storage for related material should be well organized, well maintained, and
signage for each material are well clear shown.
 Submission of material certificates, test certificates etc.

10. ENVIRONMENTAL PROTECTION


 Material Control
All materials shall be delivered, stored, consumed and disposed of without adverse impact to
the environmental and surrounding of the site.

 Storage Condition
1. Work activities will mainly be conducted during the nighttime. However, daytime work
will also be conducted if required.
2. Adequate measures will be taken on handling and storage to prevent potential safety
and health hazard.

 Waste Control
1. Ensure that the waste generated at the project site is managed efficiently and in
accordance with the current rules and regulations of the federal and state government
and local councils.
2. Ensure that any waste created during the work are collected, segregated, and disposed
of appropriately at approved dumpsite.
3. The waste shall be separately stored in the following categories:
 Domestic waste such as food leftovers, which create smell when rot.
 Biodegradable wastes like timber, paper and paper packaging box.

SYARAT - SYARAT PERAKUAN PEMATUHAN STANDARD (PPS)

1. Syarat-syarat

a) Perakuan ini tidak boleh dipindah milik.

b) Lembaga berhak mengenakan apa-apa arahan berkenaan bahan binaan dan standard yang didaftarkan dalam PPS ini dari
semasa ke semasa.

c) Lembaga berhak pada bila-bila masa menjalankan pemeriksaan terhadap mana-mana pihak yang berurusan dengan
mana-mana bahan binaan yang terkandung dalam Jadual Keempat.

2. Tanggungjawab Pemilik Sijil PPS

a) Memahami dan mematuhi peruntukan-peruntukan Akta Lembaga Pembangunan Industri Pembinaan Malaysia 1994 Akta 520
(Pindaan 2011), peraturan-peraturan yang dibuat di bawahnya dan apa-apa terma, syarat-syarat atau sekatan yang
dikenakan oleh Lembaga dari semasa ke semasa.

b) Tidak boleh berurusan atau mengaku janji untuk berurusan, sama ada secara langsung atau tidak langsung, dengan bahan
binaan yang dinyatakan dalam Jadual Keempat, melainkan jika bahan binaan itu telah diperakukan dan memperolehi PPS yang
masih sah (belum tamat tempoh).

c) Memastikan Sijil Produk/ Keputusan Pengujian Penuh yang dikeluarkan oleh Badan Pensijilan yang diiktiraf bagi bahan binaan
yang diperakukan dalam perakuan ini sentiasa dalam tempoh sah laku.

d) Mengemukakan Salinan sijil PPS ini kepada pembeli semasa urusan pembekalan, pemasaran, pemindahan dan penjualan
sebagai bukti bahan binaan ini telah diperakui oleh Lembaga.

e) Sentiasa memberi kerjasama dalam mengemukakan apa-apa maklumat yang dikehendaki oleh Lembaga dari semasa ke
semasa.

f) Memaklumkan kepada Lembaga jika ingin menamatkan, memindahkan atau apa-apa perubahan berkaitan operasi dan lokasi.

g) Memahami dan bertanggungjawab terhadap produk-produk yang memerlukan pematuhan standard yang berterusan
(pembaharuan) terutama yang melibatkan kualiti dan keselamatan kerja-kerja pembinaan, pekerja dan orang awam (Contoh;
perancah, konkrit siap bancuh atau lain-lain yang berkaitan)

3. Tindakan Atas Perlanggaran Akta 520 CIDB

a) Lembaga boleh mengambil tindakan pendakwaan mahkamah kepada mana-mana orang yang melanggar mana-mana
peruntukan dalam Akta 520 dan apabila disabitkan, didenda tidak kurang daripada sepuluh ribu ringgit (RM10,000) tetapi tidak
melebihi lima ratus ribu ringgit (RM500,000).

b) Lembaga juga boleh mengenakan tindakan tatatertib terhadap mana-mana ketidakpatuhan yang dilakukan seperti berikut:-

2 Memalsu atau mengubahsuai sijil PPS, Pensijilan Produk (PC), Keputusan Pengujian Penuh dikenakan penalti denda
(FTTR) atau apa-apa dokumen berkaitan bagi maksud memperakui bahan binaan tersebut tidak lebih RM10,000 atau
3 Melakukan apa-apa ketidakpatuhan terhadap bahan binaan yang diperiksa atau mana-mana tindakan
berdasarkan keputusan ujian atau kedua-duanya gantung atau batal dan juga
4 Melanggar mana-mana arahan Lembaga, akujanji atau akuan tanggungjawab yang dikeluarkan dikenakan penalti denda
tidak lebih RM10,000
5 Berurusan dengan mana-mana bahan binaan yang tiada di dalam senarai sijil PPS
6 Walau bagaimanapun bagi PC yang telah digantung, dibatal atau lain-lain tindakan oleh PPS tersebut akan
Badan Persijilan (CB) digantung atau dibatalkan
7 Pemohon membatalkan atau menggantung PPS secara sukarela

c) Lembaga tidak akan bertanggungjawab terhadap apa-apa kerugian atau kos-kos yang terlibat hasil dari tindakan
ketidakpatuhan yang dilakukan dan ianya merupakan tanggungjawab pengeluar, pengilang atau mana-mana orang yang
berurusan sepenuhnya.

Catatan : PPS yang telah diputuskan dan dibatalkan oleh Jawatankuasa tatatertib tidak boleh diperbaharui sehingga tempoh maksima 2
tahun. Perlanggaran arahan tersebut dan masih berurusan dengan bahan yang tidak diperakukan akan mengakibatkan pemohon
disenarai hitam dari memohon PPS selamanya dan tindakan pendakwaan mahkamah dengan denda maksima RM500,000.

1. SCOPE

This work instruction serves as guidance to project personnel in carrying out working at
height (WAH) activities safely; taking into consideration for every factor affecting the
safety of personnel while work is being carried out. This work instruction covers all SKA
Technology Sdn Bhd works where the company has its operations for Vantage Data
Centre Main Building at a site located at Lot PT 12165, Cyber Point 4 Road, Cyber 8,
Cyberjaya, Mukim Dengkil, Daerah Sepang, SelangorDarul Ehsan. This is also to ensure
the work will be executed on site entirely complying with LCM standard. EI Power
Technologies Sdn Bhd has been appointed as the subcontractor for the project, and the
safety measures will be given the utmost priority to ensure the safety and security of the work.

While these guidelines are primarily aimed at the building operation and work of for
engineering construction in relation to the design, building, and maintenance &
demolition of structures; it alsohas application to a wide range of work situations where
workers are placed in a position from which fall from height are possible.

Wherever a fall from any height could result in harm, some sort of fall protection should be
used tobreak the fall and minimize the damage and risk of injury. Fall protection shall be
supplied and used on site at risk of a fall of 1.8 meters or more. The employer / main
contractor can select the fall protection method that is most compatible with the type of
work being carried out.

These guidelines apply to all workplaces in Malaysia covered by the Factories and
Machinery Act1967 and Occupational Safety and Health Amendment Act 2022. The
aforementioned document provides detailed guidance on the safe working at heights.
However, it is not possible to deal withevery situation that may be found in the workplace
where there is a potential for personnel / materials to fall from, through or into any area,
person, equipment, machinery, etc.

2. RESPONSIBILITY

An Emergency Response Team (ERT) should be established. ERT is the internal


team to be at fore front handling the emergency situation and they should be
trained in fire-fighting, first aid and other related aspects to make sure they are
well equipped to execute the ERP procedures in the event of emergencies.

The Proposed ERT for the Project is conceptually presented below. The
roles of ERT include:

 To control or limit any effect that an emergency or potential emergency


may have on site or on neighboring areas;
 To facilitate emergency response and to provide such assistance on the
site as isappropriate to the occasion;
 To provide a communication channel with local, regional and federal
emergencyresponse networks;
 To ensure timely communication of all vital information;
 To facilitate the reorganization and reconstruction activities so that normal
operationscan be resumed;
 To provide training so that a high level of emergency
preparedness can becontinuously maintained; and
To provide a basis for updating and reviewing emergency procedures.

3. PROCEDURES

the roof of structures or any related work at risk of a fall of 1.8 meters or more in
compliance to LCM Standard.
