1 INTENDED USE
Immunoenzymetric assay for the in vitro quantitative measurement of human Growth Hormone (hGH) in serum and
plasma.
2 CLINICAL BACKGROUND
2.1 Biological activities
hGH is a polypeptide hormone (molecular weight 21,500 Da) produced by the acidophil cells of the anterior pituitary
under the control of two main substances from the median eminence : Growth-hormone Releasing Factor (GRF) and an
inhibitory agent, somatostatin. Dopaminergic, adrenergic and serotoninergic neuroendocrine pathways also play an
important role in the control of hGH secretion. Excitatory stimuli of hGH secretion include hypoglycemia, exercise,
fasting, meals with a high protein content, deep sleep, stress, glucagon, L Dopa, amino acids, etc. Inhibitory stimuli
include glucose, cortisol, hGH and free fatty acids. Because of its short plasma half life (±25 minutes) and of the frequent
excitatory or inhibitory stimuli, hGH displays frequent and large variations of concentration in serum.
One of the main physiological functions of hGH is to act on the liver and other tissues to produce somatomedins, which in
turn induce growth by direct action on target tissues. In contrast to hGH, the concentration of somatomedins in serum is
kept stable by virtue of being largely bound to circulating plasma proteins.

6 REAGENT PREPARATION
1. Calibrators:
Reconstitute the zero Calibrator with 2.0 mL distilled water and the other Calibrators with 1.0 mL distilled water.
2. Controls:
Reconstitute the controls with 1.0 mL distilled water.
3. Working anti hGH-HRP conjugate:
Prepare an adequate volume of conjugate solution by adding for example 25 µL of the concentrated hGH-HRP
conjugate to 1 mL of conjugate buffer. Use a vortex to homogenize.
Prepare the working solution just before use.
4. Working Wash Solution:
Prepare an adequate volume of Working Wash Solution by adding 199 volumes of distilled water to 1 volume of
Wash Solution (200x). Use a magnetic stirrer to homogenize.
Discard unused Working Wash Solution at the end of the day.
9 PROCEDURE
9.1 Handling notes
Do not use the kit or components beyond expiry date.
Do not mix materials from different kit lots.
Bring all the reagents to room temperature prior to use.
Thoroughly mix all reagents and samples by gentle agitation or swirling.
Perform Calibrators, controls and samples in duplicate. Vertical alignment is recommended.
Use a clean plastic container to prepare the Wash Solution.
In order to avoid cross-contamination, use a clean disposable pipette tip for the addition of each reagent and sample.
For the dispensing of the Chromogenic Solution and the Stop Solution avoid pipettes with metal parts.
High precision pipettes or automated pipetting equipment will improve the precision.
Respect the incubation times.
To avoid drift, the time between pipetting of the first Calibrator and the last sample must be limited to the time mentioned
in section 12 paragraph 12.5 (Time delay).
Prepare a calibration curve for each run, do not use data from previous runs.
Dispense the Chromogenic Solution within 15 minutes following the washing of the microtiter plate.
During incubation with Chromogenic Solution, avoid direct sunlight on the microtiter plate.
9.2 Procedure
1. Select the required number of wells for the run. The unused wells should be resealed in the bag with a desiccant and
stored at 2 °C - 8 °C.
2. Secure the wells into the holding frame.
3. Pipette 50 µL of each Calibrator, Control and Sample into the appropriate wells.
4. Pipette 50 µL of working anti-hGH-HRP conjugate into all the wells.
5. Incubate for 30 minutes at room temperature. It is mandatory to respect the 30 minute-duration of this
incubation.
6. Aspirate the liquid from each well.
7. Wash the plate 3 times by:
Dispensing 0.4 mL of Wash Solution into each well
Aspirating the content of each well
8. Pipette 100 µL of the Chromogenic Solution into each well within 15 minutes following the washing step.
9. Incubate the microtiter plate for 30 minutes at room temperature, avoid direct sunlight.
10. Pipette 100 µL of Stop Solution into each well.
11. Read the absorbencies at 450 nm (reference filter 630 nm or 650 nm) within 1 hour and calculate the results as
described in the next section.
10 CALCULATION OF RESULTS
1. Read the plate at 450 nm against a reference filter set at 650 nm (or 630 nm).
2. Calculate the mean of duplicate determinations.
3. On semi-logarithmic or linear graph paper plot the OD values (ordinate) for each Calibrator against the corresponding
concentration of hGH (abscissa) and draw a calibration curve through the Calibrator points by connecting the plotted
points with straight lines.
4. Read the concentration for each control and sample by interpolation on the calibration curve.
5. Computer assisted data reduction will simplify these calculations. If automatic result processing is used, a
4-parameter logistic function curve fitting is recommended.

14 REFERENCE INTERVALS
These values are given only for guidance; each laboratory should establish its own normal range of values.
In normal subjects, growth hormone (hGH) secretion is pulsatile. During day-time, hGH concentrations range from
< 0.2 – 10 μIU/mL. During sleep, hGH concentrations increase consistently (± 30 μIU/mL).
hGH secretion is greatly stimulated by exercise and stress (venous puncture, hypoglycemia, ...), but is decreased by
hyperglycemia.
In normal subjects (n=34), two hours after oral glucose load (75 g in adults), hGH levels were lower than 10 μIU/mL and
the hGH response to stimulation tests (insulin, arginine, glucagon administration) exceeded 20 μIU/mL.
hGH levels were elevated (even after glucose load) in acromegaly (> 10 μIU/mL).
In hGH deficiency, the response to stimulation test is absent or blunted (short stature by hGH deficiency; hypopituitarism
from various origins).

2 KLINISCHER HINTERGRUND
2.1 Biologische Aktivität
hGH ist ein Polypeptid mit einem Molekulargewicht von 21.500 Da. Synthetisiert in den azidophilen Zellen des
Hypophysenvorderlappens (HVL) erfolgt die Ausschüttung unter der Kontrolle von Somatotropin-Releasing-Faktor (GRF)
und Somatostatin als inhibierendem Agens. Neurotransmitter wie Dopamin, Adrenalin und Serotonin spielen ebenfalls
eine wichtige Rolle in der hGH-Sekretion. Stimulierend auf die hGH-Ausschüttung wirken Hypoglykämie, Sport, Fasten,
Nahrung mit hohem Proteingehalt, Schlaf, Stress, Glucagon, L-Dopa, Aminosäuren, usw. Glucose, Cortisol, hGH und
freie Fettsäuren hingegen hemmen die Freisetzung von hGH. Neben der kurzen Halbwertszeit von hGH (± 25 Minuten)
ist die Vielzahl der die Ausschüttung beeinflussenden Faktoren für die häufige und große Variationsbreite der hGH-
Spiegel im Serum verantwortlich.
Eine der Hauptwirkungen von hGH ist die Induktion der Somatomedin-Produktion in Leber und anderen Geweben.
Somatomedine beeinflussen das Wachstum durch direkte Wirkung auf die entsprechenden Zielorgane. Im Gegensatz zu
hGH bleibt die Somatomedin-Konzentration im Serum durch die weit gehende Bindung an zirkulierende Plasmaproteine
stabil.

9 DURCHFÜHRUNG
9.1 Bemerkungen zur Durchführung
Verwenden Sie den Kit oder dessen Komponenten nicht nach Ablaufdatum.
Vermischen Sie Materialien von unterschiedlichen Kit-Chargen nicht. Bringen Sie alle Reagenzien vor der Verwendung
auf Raumtemperatur.
Mischen Sie alle Reagenzien und Proben gründlich durch sanftes Schütteln oder Rühren.
Führen Sie Kalibratoren, Kontrollen und Proben doppelt aus. Vertikale Ausrichtung wird empfohlen.
Verwenden Sie zur Zubereitung der Waschlösung reinen Kunststoffbehälter.
Verwenden Sie saubere Einwegpipettenspitzen, um Kreuzkontamination zu vermeiden.
Verwenden Sie zur Pipettierung der chromogene Lösung und der Stopplösung keine Pipetten mit Metallteilen.
Präzisionspipetten oder ein automatisches Pipettiersystem erhöhen die Präzision.
Achten Sie auf die Einhaltung der Inkubationszeiten.
Zur Vermeidung von Drift muss die Zeit zwischen dem Pipettieren des ersten Kalibrators und der letzten Probe auf die
Zeit beschränkt werden, die in Abschnitt „Zeitverzögerung“ erwähnt wird.
Erstellen Sie für jeden Durchlauf eine Kalibrationskurve, verwenden Sie nicht die Daten von früheren Durchläufen.
Pipettieren Sie die chromogene Lösung innerhalb von 15 Minuten nach dem Waschen der Mikrotiterplatte.
Während der Inkubation mit der chromogene Lösung ist die Mikrotiterplatte vor direktem Sonnenlicht zu schützen.
9.2 Durchführung
1. Wählen Sie die erforderliche Anzahl der Streifen für den Lauf aus. Nicht verwendete Mikrotiterstreifen sollten wieder
dicht in der Folie verschlossen mit Trockenmittel bei 2 °C bis 8 °C gelagert werden.
2. Befestigen Sie die Streifen im Halterahmen.
3. Pipettieren Sie jeweils 50 µL Kalibrator, Kontrolle und Probe in die entsprechenden Wells.
4. Pipettieren Sie 50 µL Anti-hGH-MRP-Konjugat (Arbeitslösung) in alle Wells.
5. Inkubieren Sie 30 Minuten bei Raumtemperatur. Es ist zwingend notwendig, die Inkubationszeit von 30 Minuten
einzuhalten.
6. Saugen Sie die Flüssigkeit aus jedem Well ab.
7. Waschen Sie die Platte dreimal:
pipettieren Sie 0,4 mL Waschlösung in jedes Well
saugen Sie den Inhalt jedes Wells ab
8. Pipettieren Sie 100 µL der chromogene Lösung innerhalb von 15 Minuten nach dem Waschvorgang in jeden Well.
9. Inkubieren Sie die Mikrotiterplatte 30 Minuten bei Raumtemperatur. Vermeiden Sie direktes Sonnenlicht.
10. Pipettieren Sie 100 µL der Stopplösung in jedes Well.
11. Werten Sie die Absorptionen bei 450 nm (Referenzfilter 630 nm oder 650 nm) innerhalb 1 Stunde aus und berechnen
Sie die Resultate wie in Abschnitt 10 beschrieben.
11 TYPISCHE WERTE

12.6 Hook-Effekt
Eine Probe mit hGH bis zu 4000 μIU/mL liefert höhere Messwerte als der letzte Kalibratormesswert.
13 INTERNE QUALITÄTSKONTROLLE
Entsprechen die Ist-Werte nicht den auf den Fläschchen angegebenen Soll-Werten, können die Werte, ohne treffende
Erklärung der Abweichungen, nicht weiterverarbeitet werden.
Falls zusätzliche Kontrollen erwünscht sind, kann jedes Labor seinen eigenen Pool herstellen, der in Aliquots eingefroren
werden sollte. Kontrollen mit erhöhten Azidkonzentrationen stören die Enzymreaktion und können nicht verwendet
werden.
Die Akzeptanzkriterien für die Abweichung einer Doppelbestimmung müssen auf Guter Laborpraxis beruhen.
Es wird empfohlen, Kontrollen im Assay routinemäßig wie unbekannte Proben zu behandeln, um die Assayvarianz zu
messen. Die Leistung des Assays muss mit den Qualitätskontrollkarten der Kontrollen überprüft werden.
Es hat sich bewährt, die durch den Computer ausgewählte Kurvenanpassung visuell zu überprüfen.
14 REFERENZ INTERVALLE
Diese Werte sind nur Richtwerte; jedes Labor muss seinen eigenen Normalwertbereich ermitteln.
Bei gesunden Personen erfolgt die hGH-Ausschüttung pulsatil. Tagsüber variiert die hGH-Konzentration von < 0,2 bis
10 µIU/mL. Im Schlaf steigt die hGH-Konzentration kontinuierlich an (± 30 µIU/mL).
Nach sportlichen Aktivitäten oder Stress (Blutentnahme, Hypoglykämie) werden hohe Werte, bei Hyperglykämie niedrige
hGH-Spiegel gefunden.
Zwei Stunden nach Glucose-Belastung (75 g bei Erwachsenen) wurden bei gesunden Personen (n = 34) hGH-Spiegel
< 10 µIU/mL, nach einem Stimulationstest (Insulin, Arginin, Glucagon) hGH-Spiegel > 20 µIU/mL gefunden.
Patienten mit Akromegalie zeigten selbst nach Glucose-Gabe erhöhte hGH-Werte (> 10 µIU/mL).
Bei hGH-Mangel kann keine oder nur verminderte Erhöhung der hGH-Spiegel nach Stimulation detektiert werden
(Minderwuchs durch hGH-Mangel; Hypopituitarismus verschiedener Ursachen).
2 INFORMAZIONI CLINICHE
2.1 Attività biologiche
L’hGH è un ormone polipeptidico (peso molecolare 21.500 Da) la cui produzione da parte delle celule acidofile della
ghiandola pituitaria anteriore è controllata da due importanti sostanze prodotte dall’eminenza mediana: il Fattore di
Rilascio dell’Ormone della Crescita (GRF) e la somatostatina, un agente dall’effetto inibente. Un ruolo non meno
importante nella secrezione dell’hGH è quello esercitato dalle vie dopaminergica, adrenergica e serotoninergica. La
secrezione di hGH viene stimolata anche da altri fattori quali ipoglicemia, esercizio fisico, digiuno, assunzione di pasti ad
alto contenuto proteico, sonno profondo, stress, glucagone, L-Dopa, aminoacidi e altro, mentre risulta inibita da
glucosio, cortisolo, hGH, e acidi grassi liberi. La sua breve emivita plasmatica (+/-25 minuti) e i vari stimoli eccitatori o
inibitori determinano ampie e frequenti variazioni del suo livello nel siero. L’azione dell’hGH a livello epatico e tissutale,
una delle sue principali funzioni fisiologiche, induce la produzione di somatomedina che stimola la crescita attraverso
un’azione diretta sui tessuti target. Al contrario di quanto accade per l’hGH, il livello di somatomedina nel siero si
mantiene stabile grazie al legame di questa con le proteine circolanti nel plasma

14 INTERVALLI DI RIFERIMENTO
Questi valori vengono dati solo come guida; ogni laboratorio deve stabilire i propri intervalli normali di valori.
In soggetti normali la secrezione dell’ormone della crescita (hGH) è pulsatile. Durante il giorno, le concentrazioni di hGH
variano da < 0,2 - 10 µUI/mL, mentre durante il sonno aumentano sensibilmente (+/- 30 µUI/mL)
La secrezione di hGH viene fortemente stimolata dall’esercizio fisico e dallo stress (iniezione endovenosa, ipoglicemia…)
e viene al contrario inibita dall’iperglicemia. In soggetti normali (n=34), a due ore dal test da carico di glucosio (75g. negli
adulti), i livelli di hGH sono risultati inferiori a 10 µUI/mL, e la risposta dell’hGH ai test di stimolazione (insulina, arginina e
glucagone) ha superato il livello di 20 µUI/mL.
Nei soggetti con acromegalia, i livelli di hGH sono risultati elevati anche dopo carico di glucosio (>10 µUI/mL).
Nei casi di deficit di hGH, la risposta ai test di stimolazione risulta assente o fortemente attenuata (bassa statura dovuta a
deficit di hGH, ipopituitarismo di varia origine).
1 INSTRUCCIONES DE USO
Ensayo inmunoenzimático para la medición cuantitativa in vitro de la Hormona de Crecimiento Humana (hGH) en suero o
plasma.
2 INFORMACIÓN CLÍNICA
2.1 Actividades biológicas
La hGH es una hormona polipeptídica (peso molecular 21,500 Da) producida por las células acidófilas de la
adenohipófisis bajo el control de dos sustancias principales de la eminencia media: factor de liberación de la hormona de
crecimiento (GRF) y un agente inhibidor, somatostatina. Las vías dopaminérgica, adrenérgica serotoninérgica y
neuroendocrina también juegan un papel importante en el control de la secreción de hGH. Estímulos excitatorios de la
secreción de hGH incluyen hipoglicemia, ejercicio, ayuno, alimentos con un contenido proteico alto, sueño profundo,
estrés, glucagón, L Dopa, aminoácidos etc. Estímulos inhibitorios incluyen glucosa, cortisol, hGH y ácidos grasos.
Debido a su corta vida media plasmática (±25 minutos) y los frecuentes estímulos excitatorios e inhibitorios, la hGH
presenta variaciones frecuentes y prolongadas de concentración en el suero.
Una de las funciones fisiológicas principales de la hGH es de actuar en el hígado y otros tejidos para producir
somatomedinas, la que a su vez induce el crecimiento por acción directa en tejidos diana. En contraste con la hGH, la
concentración de somatomedinas séricas se mantiene estable gracias a que en su mayoría se encuentra unida a
proteínas plasmáticas circulantes.

5 MATERIALES NO SUMINISTRADOS
El siguiente material es necesario pero no está suministrado en el kit:
1. Agua destilada de alta calidad
2. Pipetas para dispensar : 50 µL, 200 µL, 500 µL y 2 mL (se recomienda el uso de una pipeta precisa con puntas
plásticas desechables)

6. Lector de microplacas capaz de leer a 450 nm y 650 nm (lectura bicromática)
6 PREPARACIÓN DE REACTIVOS
1. Calibradores:
Reconstituya el calibrador cero con 2,0 mL de agua destilada y otros calibradores con 1 mL de agua destilada.
2. Controles:
Reconstituya los controles con 1 mL de agua destilada.
3. Conjugado anti-hGH-HRP de trabajo:
Prepare un volumen adecuado de solución de conjugado añadiendo 25 µL del conjugado anti-hGH-HRP concentrado
a 1 mL de tampón de conjugado. Use un Vortex para homogenizar.
Se recomienda preparar inmediatamente antes de utilizar.
4. Solución de lavado de trabajo:
Prepare un volumen adecuado de solución de lavado de trabajo añadiendo 199 volúmenes de agua destilada a
1 volumen de solución de lavado (200x). Utilice un agitador magnético para homogenizar.
Elimine la solución de lavado de trabajo no utilizada al final del día.
9 PROTOCOLO
9.1 Notas de manejo
No utilice el kit o sus componentes después de la fecha de caducidad.
No mezcle materiales de kits con números de lote diferentes.
Antes de utilizar deje que todos los reactivos alcancen temperatura ambiente.
Mezcle exhaustivamente todos los reactivos y muestras agitándolos suavemente o girándolos.
Analice los calibradores, controles y muestras en duplicado. Se recomienda la alineación vertical
Utilice un envase de plástico limpio para preparar la solución de lavado.
Para evitar contaminación cruzada, utilice una punta de pipeta desechable, limpia para añadir cada reactivo y muestra.
Para dispensar la solución cromogénica y la solución de parada evite las pipetas con partes metálicas.
Pipetas de alta precisión o equipo de pipeteo automático mejorará la precisión.
Respete los tiempos de incubación.
Para evitar la demora, el tiempo entre el pipeteo del primer calibrador y la última muestra debe limitarse al tiempo
mencionado en la sección 12 párrafo 5 (Demora en el tiempo).
Prepare una curva de calibración para cada serie, no utilice datos de series anteriores.
Dispense la solución cromogénica dentro de 15 minutos después del lavado de la microplaca.
Durante la incubación con la solución cromogénica, evite la luz solar directa sobre la microplaca.
9.2 Procedimiento
1. Seleccione el número requerido de pocillos para la serie. Los pocillos no utilizados deben volver a almacenarse en la
bolsa sellada con el desecante a 2 °C - 8 °C.
2. Fije las tiras de pocillos en el marco de sostén.
3. Pipetee 50 µL de cada calibrador, control y muestra en los pocillos pertinentes.
4. Pipetee 50 µL de conjugado anti-hGH-HRP de trabajo en todos los pocillos.
5. Incube por 30 minutos a temperatura ambiente. Es obligatorio respetar los 30 minutos que dura esta
incubación.
6. Aspire el líquido de cada pocillo.
7. Lave la placa 3 veces:
Dispensando 0,4 mL de solución de lavado en cada pocillo
Aspirando el contenido de cada pocillo
8. Pipetee 100 µL de solución cromogénica en cada pocillo dentro de los 15 minutos después del paso de lavado.
9. Incube la microplaca por 30 minutos a temperatura ambiente, evitando la luz solar directa.
10. Pipetee 100 µL de solución de parada en cada pocillo.
11. Lea las absorbancias a 450 nm (filtro de referencia 630 nm o 650 nm) dentro de 1 hora y calcule los resultados como
se describe en la sección 10.
10 CALCULO DE RESULTADOS
1. Lea la placa a 450 nm contra un filtro de referencia a 650 nm (o 630 nm).
2. Calcule la media de las determinaciones en duplicado.
3. En papel semi logarítmico o lineal dibuje los valores de DO (ordenada) para cada calibrador contra la concentración
de hGH correspondiente (abscisa) y dibuje una curva de calibración a través de los puntos de los calibradores
conectando los puntos con líneas rectas.
4. Lea la concentración de cada control y muestra por interpolación en la curva de calibración.
5. La reducción de datos asistida por un ordenador simplificará estos cálculos. Si utiliza un procesamiento automático
de los resultados, se recomienda un ajuste de la curva con una función logística de 4 parámetros.
11 EJEMPLO DE RESULTADOS
Los siguientes datos son solo a modo de ilustración y nunca deben utilizarse en vez de la curva de calibración en tiempo
real.

12 REALIZACIÓN Y LIMITACIONES
12.1 Límite de detección
Se analizaron 20 calibradores cero en conjunto con un grupo de otros calibradores. El límite de detección, definido como
la concentración aparente dos desviaciones estándar sobre la DO promedio cuando hay cero unión, fue 0,17 μIU/mL.
12.2 Especificidad
Se añadieron hormonas con reacción cruzada a una muestra de control con hGH alto y otro bajo. Se midió la respuesta
hGH aparente.

14 INTERVALOS DE REFERENCIA
Estos valores son solo una guia; cada laboratorio debe establecer su propio rango normal de valores.
En sujetos normales, la secreción de la hormona de crecimiento (hGH) es pulsátil. Durante el día, las concentraciones de
hGH varían entre < 0,2 - 10 μUI/mL. Durante el sueño, las concentraciones de hGH aumentan constantemente
(± 30 μUI/mL).
La secreción de hGH se estimula en gran medida por el ejercicio y el estrés (punción venosa, hipoglucemia, ...), pero
disminuye por la hiperglucemia.
En sujetos normales (n = 34), dos horas después de la carga de glucosa oral (75 g en adultos), los niveles de hGH
fueron inferiores a 10 μUI y la respuesta de la hGH a pruebas de estimulación (insulina, arginina, administración de
glucagón) aumentaron por sobre 20 μUI/mL.
Los niveles de hGH estaban elevados (incluso después de carga de glucosa) en la acromegalia (> 10 μUI/mL).
En la deficiencia de hGH, la respuesta a la prueba de estimulación está ausente o disminuida (baja estatura por la
deficiencia de hGH; hipopituitarismo de diversos orígenes)
15 PRECAUCIONES Y ADVERTENCIAS
Seguridad
Solo para uso de diagnóstico in vitro.
Los componentes de sangre humana incluidos en este kit han sido analizados con métodos aprobados en Europa y/o
por la FDA y fueron negativos para HBsAg, anti-VHC, anti-VIH-1 y 2. Ningún método conocido puede garantizar
totalmente que los derivados de la sangre humana no transmitan hepatitis, SIDA u otras infecciones. Por lo tanto, el
manejo de reactivos, suero o plasma deben cumplir con los procedimientos de seguridad locales.
Todos los productos de origen animal y derivados son de animales sanos. Los componentes bovinos son originarios de
países donde no se ha informado la presencia de EEB. Sin embargo, los componentes que contienen sustancias
animales deben ser tratados como potencialmente infecciosos.
