I. Fill-up a PRO-FORMA Credit Application Form based on your assigned type of loan per
group. Use the sample forms you filled-up in your previous assignment as your guide. Use
the topic on Part 2, Chapter 1, Credit Application and Other requirements as your guide in
filling-up the forms.

c. Some “leads” on which to start the credit investigation process
d. Documentation Required for the Processing of the Loan Application

II. Make a video of the Credit Interview based on the Credit Application Form. Use the topics
on Chapter 2, 3 and 4 on Credit Interview, Credit Qualities to Investigate and Sources of
Credit Information as your guide for the video.
a. Identify Credit Qualities to Investigate.
b. Identify Multifarious Sources that a Credit Investigator could find.
c. Include in your video the preparation for the Credit Interview

NOTE: This video will be your Mastery Test score for your Semifinal Grade. Make
sure to give a role to each group members in the video. Make a script of the video.

III. Write a Credit Investigation Report using the guide/topics in Chapter 5 and 6. Use all the
information in the Credit Interview and Sources of Credit Information of the
customer/loan applicant. Include all pertinent information in this report including the
necessary documents as collateral and appraisal of Real and Personal Property. (You can
also research in the internet on how to make a Credit Investigation Report).
 INSTRUCTIONS FOR SUBMISSION:

4. Put in short plastic folder with clip.
