AFFIDAVIT OF COHABITATION

I, KRISTINE JOY G. SANTIAGO, of legal age, Filipino, with address at 8 Libya Street,
Greenheights Subdivision, Brgy Nangka, Marikina City, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. I am the affiant in this affidavit and have personal knowledge of the facts stated herein.

2. That I have been cohabitating with my partner, Daniel Garret Villareal Macaraig, also

of legal age, Filipino, single, and residing at 8 Libya Street, Greenheights Subdivision,

Brgy Nangka, Marikina City, since February 20, 2021.

3. That during the said period of cohabitation, we have lived together as partners in a

relationship akin to that of spouses.

4. That the purpose of this affidavit is to attest to the fact of our cohabitation for legal

and/or documentary purposes as may be necessary.
