DATA PRIVACY AND CONFIDENTIALTY AGREEMENT

In compliance with Republic Act No. 10173 the Data Privacy Act of 2012 and other relevant laws
and regulations, the Social Welfare Officer of Regional Trial Court of Angeles City, seeks your consent to
collect and use personal information upon order of the Honorable Court for the purposes of social case
study report.

Purposes and Collection

The Social Welfare Officer will collect through office interview, home visitation and collateral
interview and process your personal information solely for the purpose of the social case study report
ordered by the Honorable Court with recommendation for its guidance.

Processing and Data Sharing

In furtherance of above stated purpose, your personal information will be disclosed only to the
Honorable Court. The following data that you own may be processed solely for the above purposes.

1. Name, age, date and place of birth, residential and provincial address, height and weight, civil
status, and other contact details
2. Citizenship, educational records and background, employment history
3. Family composition and background
4. Government clearance records such as barangay, NBI, police, prosecutors, MTCC and RTC
5. Physical and psychological evaluation/clearance, if required
6. Financial status such as bank statements
7. Online user identity and information collected during interviews. As part of the interview process,
the Social Welfare Officer may maintain audio/video recordings.
8. Other information collected through collateral interviews from non-relative individuals
