Date
30.09.2014 Target [A23-I-2] According to engineer, IDG oil servicing performed before each flight (due to leakage), but no TLB entry for the oil
added since at least 20th August.
Followup item A23 / C01 "Maintenance action not properly reported" and C07 "oil leakage"

I think that CRC does not realize of the impact of following statements on the air safety.:
"According to engineer, IDG oil servicing performed before each flight (due to leakage), but no TLB entry for the oil added
since at least 20th August."

Moreover, the oil leak from spinner has been wrongly recorded into "non airworthiness item" deferred defect list on July 2014.
Further investigations in the on-board documentation showed that this defect was not monitored. I would like also bring your
attention on the
traceability of your technical documentation. Indeed, any technical reference has been used in order to dispatch the leakage,
the reason entered "will be replaced when part available" and the measures implemented by your airline (purchase order is
signed one month later on 16th August 2014) are inappropriate for safety air transportation operations.

Besides, a proof of definitive repair, I kindly appreciate to received a feedback from Cameroon Civil Aviation Authority, as the
entity responsible for the safety oversight of your operations, on this matter. Pleased note that during the SAFA inspection
performed on 02nd MAY 2014, a non permitted dispatch of escape door slide has been highlighted.

30.09.2014 Target [A23-I-2] See item : A01
Followup item A23 / C01 "indication light test"

French NAA has taken due note of corrective action which have been performed before departure to CDG as filled in extract
#103870 from your TLB. However in order to solve this finding, we need further details focused on preventive measures
implemented by CRC to preclude same deviation in the future.

30.09.2014 Target [C07-I-2] Safety markings not applied on both engines.
Followup item C07 "safety marking"

According your statement, safety marking found missing during the inspection would not required by P&W in accordance with
AMM ch 11.
In order to re-assess this finding, could you please forward us the relevant extract from AMM and P&W documentation on this
matter
