AFFIDAVIT OF CORRECTION
I, HAZEL IRIS P. LAFUENTE. Of legal age, Filipino, with office address at 112
Legaspi St., Legaspi Village, Makati City, after having been duly sworn to in accordance with
law, do hereby depose and state:

1. That I am the duly authorized representative of SOLAR PHILIPPINES TARLAC

CORPORATION (SPTRC), a corporation duly organized and existing under the
Philippine laws;

2. Thar SPTRC owns certain parcel of land located in Sta. Rosa, Concepcion, Tarlac
containing an area of Twelve Thousand Two Hundred Seventy-Two (12, 272) square
meters covered by Transfer Certificate Title No. 043-2019014951 a copy of which is
herein attached as “Annex A”;

3. That annotated on the abovementioned title is Entry No. 2019005320 which refers to,
among others, a DAR Order: Issued by Rosalina L. Bistoyong OIC-Secretary of the
Department of Agrarian Reform, Diliman Quezon City;

4. That the abovementioned annotation was erroneously and inadvertently entered and
referred to as DARCO ORDER NO. CON-1711-331, when in fact, the proper reference
should be DARCO ORDER NO. ______________ a copy of which is herein attached
as “Annex C”;

5. That I am executing this affidavit to ask your good office to correct the said annotation;

6. That I am executing this affidavit to attest to the truth of the foregoing facts;

7. That I am further executing this affidavit as a compliance for the requirements of

Registry of Deeds for the Province of Tarlac for the correction of the annotation in the
said Transfer Certificate of Title and for whatever legal purpose, it may serve.
