Bring this form fully completed to either the Sports Office in the City Sports Centre in the Students Union
Building (City Site) or Lee Westwood Sports Centre main reception (Clifton Campus) and pay the £10 entry fee.
The completed form can also be emailed to football@ntu.ac.uk

This entry fee also covers your first fixture. Weekly fees are £10 per team, payable by the end of the Tuesday
prior to the forthcoming week’s fixture. Entries will be accepted on a first-come-first-served basis.
