The Master has the overriding authority and responsibility in relation to his ship to make decisions with respect to safety of
the Vessel, prevention of Human Injury or Loss of Life and the prevention of property damage and pollution. He may request
the Company’s assistance as necessary.

(1A) VESSEL STANDBY CONDITIONS AND FOLLOW-UP ACTION - STAGE 1


The vessel and related parties should be on standby, under the following conditions:

1. If the radar gives indication of impending squalls approaching the area of operations

2. Wind speed maintain constant of 10 seconds for 20 Knots.


3. Sea state (Waves/swell) height up to 1.5 meters
4. Sea current/wave/swell pushing the vessel towards the offshore installation.
Anchor wire tension reaching 20 tonnes (Anchor pre-tensioning set at 25 Tonnes) without using Main Engines to offset the

6. Weather forecast warning of Adverse Weather in Vessel area of operation.

Maximum Time Limit for Vessel to be ready:30 minutes (Must inform POB and Marine Crew for standby)

Once ANY conditions (1A) above are met, the following actions shall be taken immediately:

1. Main Engines (MEs) running idling


2. Bow Thruster (BT) on standby and ready to operate

3. Mooring stations are to be manned.


Mooring winches on standby and anchor wire drum brake released as vessel may need to be moved to the Approved Anchor

Pattern stand-off position.
5. Inform CSR, Field Supt., OIM of the adverse weather condition. Informed DPA.

(1B) VESSEL PULL-OFF CONDITIONS AND FOLLOW-UP ACTION – STAGE 2


The Master should decide on the actual pull-off action if the following conditions are met with priority given to the anchor
wire tension:
1. Wind speeds exceeding 25 Knots
2. Sea state (waves/swell) height up to 2.5 meters..

Re-tension the anchor wires after pull-off to Anchor pre-tensioning 25 Tonnes.

Regardless of above environmental safety criteria, the Master MUST use his professional judgement in making decisions to pull off to
ensure the safety of vessel, people, pollution prevention and damage to client assets.

Once ANY conditions (1B) above are met, the following actions shall be taken immediately:
Inform related parties (i.e. construction supervisors, platform staff) to disconnect and clear all umbilical hoses/cables through

Retreat vessel to Approved Anchor Pattern Stand-off position as per contingency plan (approximately 100 metres away from

any part of the offshore installation)..
Mooring winches on standby and anchor wire drum brake released as vessel may need to be moved to the Approved Anchor

4. Ascertain propeller is well clear (in event where engine is to be used).
All down weather anchors should be slackened when necessary to allow vessel to move as free as possible to prevailing

weather direction with the engine thrust (main engines & bow thruster) as required to minimize effect of the adverse weather.
6 Continuously maintain radar watch, monitor all weather forecast, sea state and anchor wires tension
7 Advise Office & DPA.

(1C) VESSEL PULL-OUT [RELEASE ANCHOR WIRES] AND FOLLOW-UP ACTION – Any One Condition Met – STAGE 3
(Vessel Currently at “Stand Off” Position – Approximately 100M from Offshore Platform)
Should the weather deteriorate further and the anchor wire tension increase and any one of the below parameters is met,
Master will release the anchors immediately and proceed to shelter:
Shelter Position Bintulu Anchorage

1. Wind speeds exceeding 35 Knots

2. Sea state (wave/swell) height exceed/more than 2.5 meters

3. Excessive motion of the vessel affecting crew safety and resulting inability to carry out work operations.

4. Anchor found dragging or any of the anchor wire parted.

5. When Master believe that the vessel can be damaged by the weather conditions eg. sea water flooding on deck.

6. Any of anchor wire tension exceeded 25MT.

Irrespective, if the above parameter are not met, Master MUST release the anchor wires and move out when (i) the Bow
Thruster operation exceeded 1 hour and (ii) the main engines propulsion exceeding 45% in keeping / maintaining vessel in
position

Regardless of above environmental and safety criteria, the Master MUST use his professional judgement in making decisions
to pull off to ensure the safety of vessel, people, pollution prevention and damage to client assets.

Once ANY conditions (1C) above are met, the following actions shall be taken immediately:

1. Release anchor wires securing at the mooring winch.

2. Initiate the release of anchor wires.

3. Navigate vessel safely away from platform to allow anchor wire to slip off safely

4. Vessel steaming off offshore location or proceeds to shelter point as advised by CSR.
Ensure entries made in Deck (Chief Officer) Logbook. Make an official marine note of protest and entry into the Official

Logbook at the earliest opportunity if Master believed that the vessel sustained damages due to the weather conditions.

(2A) OFFSHORE INSTALLATION FIRE BREAKOUT


In the event that a fire breakout occurs at an offshore installation and the Accommodation Vessel is required to render
assistance, the following action need to be taken:
1. The Master is to make an announcement over the PA system to alert all personnel onboard the vessel

2. Main Engines, Bow Thruster and Anchor Winches are to be started, with relevant crew manning their stations.

3. Emergency Fire Pump is to be started and ready to assist in fighting the fire
Marine Emergency Response Team (ERT) is to be on standby and ready to man their fire-fighting stations should assistance

(2B) OFFSHORE INSTALLATION FIRE ESCALATION


If the fire escalates beyond control and poses a threat to the safety of the vessel and her crew/passengers, the following
actions should be taken:
1. Inform all Key Personnel (i.e., CSR, Field Supt., OIM etc.) and DPA

2. Inform CSR/OIM the decision for the vessel to stand-by up-wind within the vicinity to render assistance, whenever needed.

3. Initiate vessel pull-off (release anchors) from location.

4. Should assistance not required, proceed towards the agreed holding or shelter.
Should the vessel be withdrawn to a standing-by mode up-wind within the vicinity of the platform installation, Marine
5. Emergency Response Team is to man their “Man Overboard” stations to assist in any search and rescue operation, to
retrieve survivors from the offshore installation.

(2C) FIRE ON SEA SURFACE INSTALLATION


If vessel located up-wind of the fire;

1. Master/OOW announce over PA to alert all personnel onboard

2. Standby Main Engines and Bow Thruster

3. Ensure all attachments are cleared from platform

4. Start all anchor winches motor, pull off vessel to a safe distance up to maximum anchor wires spread.

5. FFA ready to assist as require.

7 Marine Emergency Response Team on standby to assist as require.

If vessel located down-wind of the fire (Unsafe Position);

1. Master/OOW announce over PA to alert all personnel onboard

2. Ensure all attachments are cleared from platform

3. Start all anchor winches motor, Main Engines and Bow Thruster

4. Pull off vessel to a safe distance up to maximum anchor wires spread

5. Release all anchor wires and withdraw to a safe position.

(3A) OFFSHORE INSTALLATION BLOWOUT OR GAS LEAK


If the fire escalates beyond control and poses a threat to the safety of the vessel and her crew/passengers, the following
actions should be taken:
1. The Master is to make an announcement over the PA system to alert all personnel onboard the vessel

2. Main Engines, Bow Thruster and Anchor Winches are to be started, with relevant crew manning their stations.

3. All umbilical hoses/cables are to be disconnected between the vessel and the offshore installation
Master shall pull off the vessel to her designated stand-off position, or to the limits of the anchor spread, whichever is

deemed appropriate as per (1B)
5. Inform CSR/OIM the decision for the vessel to stand-by up-wind within the vicinity to render assistance, whenever needed

6. Should the situation required for the vessel to pull out, Master shall initiate the pull out procedure as per section 1C above

7. Render assistance as required by the SAR Team as required

8. Vessel shall seek shelter at agreed shelter point should her presence is no longer required at site and as advised by CSR.
