A. SOURCE OF FUNDING ALLOCATION.

The project Fund (PF) shall be mainly from the Special Project Fund of the
Directorate for Intelligence.

B. DISBURSEMENT/BREAKDOWN OF OPERATION COST.

COPLAN “MANGUILLA” PS-8 2012-01 needs a monthly budget allocation of Php.

54,000 per month or a total of Php. 162,000.00 for the three (3) months.

Case Director (CD):

a.) Shall provide command and management of the entire intelligence operation
of this COPLAN “MANGUILLA“ PS-8-2012-01.

Case Supervisor (CS):

a.) Shall assist the Case Director and supervise the Implementation of the
project.

Case Officer (CO):

a.) Shall responsible for the operational control and supervision of the entire case

am working with CONFIDENTIAL INFORMANT including its agents, employees,
officers, directors. Affiliates, successors and assigns (hereinafter, the “Released
Parties”), of and from any and all claims, demands, debts, contracts, expenses, causes
of action, lawsuits, damages, and liabilities, of every kind and nature, whether known or
unknown, in law or equity, that I ever had or may have, arising from or in any way
related to my participation in any of the events or activities conducted by, on the
premises of, or for the benefit of, provided that this waiver of liability does not apply
acts of gross negligence, or intentional, willful or wanton misconduct.

I understand that the activities and functions in which I participate maybe

considered (but not have to be) of a volunteer nature. On behalf of myself, my heirs,
assigns and net of kin, I waive all claims for damages, injuries and death sustained to
me or my property, that I may have against the aforementioned Released Parties.

By this Waiver, I assume any risk, and take full responsibility and waive any
claims of personal injury, death or damage to personal property associated with my as
an agent/informant on behalf of the Released Parties including but not limited to any
professional or volunteer activities, community events, using PNP facilities or
