Supply, Delivery and Catering Services For The Various Activities of Cgecs For The 1st Quarter and 2nd Quarter of Calendar Cy 2024
CTJ BANQUET, a business entity duly organizes and existing by operation of law and represented herein
by its duly authorized representative; MS. CARLA CAMILLE GAY-YA, hereinafter called as the
CATERER.
AND
PHILIPPINE COAST GUARD a government entity duly organizes and existing by operation law and with
office address at Farola Compound, Binondo, Manila, represented herein by its authorized representative;
CG COMMO LOWIE ANACTA PALINES hereinafter called as the CLIENT.
The above-mentioned parties bind themselves to comply with all the terms and conditions stipulated
hereunder as follows:

: Chips
: Drinks
1. A contract of agreement shall be served, by the CATERER at least a week or two (2) days after
reservation has been made provided that information vital to contract have been furnished i.e., food choices,
color motif and others.
2. The CLIENT must sign the contract of agreement and return it to the CATERER after two (2) days.
3. The CLIENT further aggresses to pay the sum of FIVE HUNDRED FIVE THOUSAND SIX
HUNRRED SIXTY PESOS & 00/100 (PHP 505,660.00) ONLY as full Contract Price.
4. The CLIENT further agrees to pay the full contract price within 15 days after the function.
5. In case of postponement of function date, the caterer shall not guarantee the same contract price as stated,
should there be price increase implemented by the company
6. The Client further agrees that if it fails to pay the full amount of the contract on or before the 15 th day after
the function, the CATERER will charge an additional 5% on the total amount of the Contract Price.
8. The CATERER shall have the discretion to refuse serving of food that would be given or brought in by
the CLIENT or his/her representative based on the following:
a. If the quantity of food would not be enough for all the expected guests.
b. If the food was not properly handles and no longer in good condition.
9. The CATERER should be informed by the CLIENT or and his representative, prior to the event,
bringing in of additional beverages, wines, liquor, and spirits. The caterer shall not be obliged to serve such
drinks unless informed.
10. The CATERER shall not be responsible for the shortage of foods and beverages in the event of having
unexpected or excess guests from what was in the contract.
11. The CLIENT shall pay for the cost of broken, damaged or lost equipment, furniture and fixtures,
glassware and utensils caused by the guests and other visitors.
12. In cases/occasions, where there are gifts, the CLIENT is responsible in taking care of the gifts. The
CATERER is not liable for any loss or damage of said items.
IN WITNESS WHEREOF, the parties hereto have signed this BANQUET AND CATERING
AGREEMENT on this ____ day of ________ 2024 at the City of Manila.
Before me, this day of ____ of _______________ personally appeared CARLA CAMILLE GAY-YA
C.T.C No. 20223763 issued at Manila on 15 JANUARY 2024, and CG COMMO LOWIE ANACTA
PALINES with CTC No. _______________ issued at __________________ on ______________, known
to me to be the same persons who executed the foregoing instrument and acknowledged to me that the same
is their free and voluntary act and deed.
This instrument consisting of nine (9) pages including on which this acknowledgement is written has been
signed on the left margin of each and every page thereof by the contracting parties and their witnesses, and
seal with my notarial seal.
