RE: Shirley Decosta, two-week old baby.

Dear Ms. Susanna Bates,

I am writing this referral letter to patient Shirley Decosta for her follow up care. I
am requesting to provide support to her care after discharged.

Mrs. Ritz Decosta delivered Shirley Decosta two weeks ago via normal delivery
without any complications and signs of illness. She weighed 3009 grams at the
time she was delivered. Presently, she is on breastfeeding and weighs 3022 grams
upon discharge.

Mrs. Ritz Decosta is a 28 year old mother. Her husband is a taxi driver and has a 9
year old daughter.

I would like to request for the baby’s support to her care after discharged.
