Dear Ms. Erica Manuel

I am writing this referral letter for patient Mr. Deacon Lamprey who was
diagnosed with Skin Cancer of Basal Cell Carcinoma and nodular basal
cell carcinoma on neck and undergone operation in our hospital.

Mr. Lamprey underwent an operation for skin biopsy. He tolerated the

procedure well and no complaints after the surgery. Currently,he is stable
and well upon discharge. He was prescribed to take Panadein forte
500mg for pain relief.

As for the patient’s social background, Mr. Lamprey is a bus driver. Has
a dark-skinned complexion and lives with his wife. He has habit of
drinking alcohol excessively for the past 27 years and a smoker.

I would like to request for continuity of care for Mr. Lamprey living at
235 Euston rd., Fitzrovia, London NW1 2BU for daily observation and
continue taking his medicine for the next seven days upon discharge.
