LEAVE APPLICATION

INSTRUCTIONS:

1. All absences from work MUST be covered by leave application form.

2. In case of personal leave, fill out and submit this form at least one (1) day in advance.

3. In case of emergency or sickness where prior permission could not be obtained, the leave
application MUST be filed on the following day. Employee, however must notify his/her
immediate Supervisor / Department Head of his/her absence before the start of his/her shift.

4. In case of absence from work due to sickness, the company reserves the right to require an
employee to submit a medical certificate before or upon return to work. No medical certificate, no
clearance, no work.

5. Absence without official leave (AWOL) or prior permission is subject to disciplinary action.

6. Notification as well as filing of an application for leave do not automatically mean approval of a
leave of application.
