This form should be used to list identified hazards, controls and responsibilities for each step in the operation.

Keep drill bit square to the surface being drilled
Falls from a ladder Ensure that ladders are safely secured, and at a 4:1 pitch
Do not stand higher than the 3rd top rung
Only carry out light duties, using one hand, when working from
a ladder
Injuries from nail guns, power Never carry power tools with the trigger depressed
