covered by an OCT/TCT F-21795 containing an area of THREE THOUSAND AND
FOUR HUNDRED TWENTY-FOUR SQUARE METERS (3424 sq. m.) more or less.

2. That on ____________________, a DEED OF DONATION has been executed over the

said parcel of land in favor of Brgy. Bay-ang for a portion of TWO HUNDRED EIGHTY
SQUARE METERS ONLY (280 sq.m.) executed by our Attorney-in-Fact, Ma. Nita B.
Subong;

3. That we hereby affirm and confirm the execution of the said instrument of the above-
described parcel of land in favor of Brgy. Bay-ang, Ajuy, Iloilo before Notary Public
Atty. _________________________ on _________________ under Doc No. _________,

This Affidavit is being executed to attest to the foregoing facts and in compliance with the
requirements prescribed by the concerned agencies of the Government.
