AFFIDAVIT OF LOSS

I, ELLNER ORDOÑEZ y De La Cruz, of legal age, married, Filipino, with

residence address at Munting Tubig, Balayan, Batangas, after having been duly
sworn in accordance with law do hereby depose and state:

1. That on April 8, 2000, I completed high school and graduated from

Balayan National High School;

2. That as proof of said completion, I was issued a High School Diploma by

the said school;

3. That I placed the original copy of my High School Diploma inside a

brown envelope and kept it inside our house at the abovementioned
address;

4. That on April 22, 2024, while I was tidying up my files and records, I
noticed that my High School Diploma is missing;

5. That despite diligent search and efforts to locate my High School

Diploma, I could not find the same such that I now have reason to
believe that it is lost and could not be recovered;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 8 th day of May

2024 at San Juan City.
