attachments are required per CADT/CALT application
(including the original copy);

▪ ICCs/IPs or clan/family/individual – one
(1) original copy
▪ Regional Office – one (1) original copy
and one (1) duplicate copy
▪ The rest of the duplicate copies shall be
given to the PO and CSC.
Ø Photocopied documents must be certified that it
came from the original copy.
Ø All thumb-marks (both left and right) that are
executed by the IPs in lieu of their signatures should
be attested or verified by the NCIP.
Ø Sources of data/information should be cited and/or
acknowledged.
