Pre-employment Medical Examination slip:

Claim the PEME Slip from your recruiter. Kindly follow the indicated date above. Remember that your
start date is dependent on the release of your medical clearance so we advise you to undergo the
necessary exams as soon as possible.

Internship Agreement.
Kindly print this, edit the highlighted fields, and submit the signed copy to us. Afterwards, have it notarized
personally or thru your school.
We will also sign your school's Memorandum of Agreement (MOA) if needed. Simply photocopy it and
submit the two copies to your recruiter for signing.

Security Bank and Payroll enrollment

Get your SSS number either online or at the closest SSS branch from you

Answer completely so you can receive your allowance

Intern Schedule Template form

Download the attached form, and color your availability so your internship schedule can be plotted by
your supervisor.
