JOINT AFFIDAVIT

We, SANY BOY C. SOQUIB and JENNIFER S. PALACIO, Filipino residents, of legal ages,
single and residents of Zone 1, Bugcaon, Lantapan, Bukidnon after having been duly sworn to
in accordance to law hereby depose and say:

That we know spouses CHARLEY C. SOQUIB and JAYCEL JANE Y. LADERA are the
parents of CHANTAL L. SOQUIB who was born out of wedlock on December 15, 2022 at Zone
1, Bugcaon, Lantapan, Bukidnon;

That his/her parents are CHARLEY C. SOQUIB and JAYCEL JANE Y. LADERA;

That we all know this facts because we affiants are friends and close neighbor of the
parents of CHANTAL LADERA SOQUIB.

That, we are executing this affidavit to the facts surrounding the birth of the above-
named child to supportin the delayed registration of his/her birth at the Office of the
Municipal Civil Registrar of Lantapan, Bukidnon.

AFFIANTS FURTHER SAYETH NONE.
