SUBJECT: Request for Fire and Earthquake Safety Lecture & Drill

Greetings!

We are writing your good office to humbly request the conduct of a Fire and Earthquake Safety Lecture &
Drill in our facility located at Libo St., Brgy. Tayud, Consolacion, Cebu.

This is part of our compliance with Occupational Safety and Health “Rule 1940: Fire Protection and
Control" and our semi-annual safety program, as well as to test our site emergency preparedness and
response in the event of a fire or an earthquake. Furthermore, we aim to promote awareness to our
employees on the dangers of fire in our workplace and how to act safely in the event of an incident,
including the selection and use of a fire extinguisher.

We would like to ask for your proposed schedule option(s) for the said activity which we hope you can
