I am excited to apply for the position of Field Monitor/Data Collector for AHS 2024
at Particip GmbH. With my background in health and social sciences, I am confident
in my ability to effectively collect and manage data for the Afghanistan Household
Health Survey 2024.
As a couple, my partner and I meet the Islamic Sharia law requirement of being
Mahram, and we are both residents of the province, which aligns with the job
requirements. Our experience in past surveys such as AHS, DHS, MICS, SMART,
LQAS, HMIS verification, and Covid-19 surveys will be valuable in this role.
I am proficient in both Dari and Pashto, which will enable me to communicate
effectively with the local population. My excellent written and spoken skills will also
facilitate the completion of the Household Questionnaire and other data collection
forms accurately.
I am committed to maintaining the integrity and confidentiality of the data collected,
adhering to the ethical standards of Particip GmbH and the Ministry of Public Health.
I am also willing to travel across the province to visit households, ensuring that all
data is collected efficiently.

 Attended and actively participated in training on data collection procedures


and measures, maintaining reliability
 Participated in pre-training and post-training assessments, passing the post-
training assessment threshold
 Collected data from selected households in selected clusters in accordance
with established protocols
 Located the compounds that had been selected for household surveys
 Ensured consent forms were completed for every person interviewed
 Completed the Household Questionnaire for the selected household
 Identified all eligible women in the selected household and interviewed them
using the Female and Child Health Questionnaire
 Checked completed interviews to ensure all questions were asked and
responses were neatly and legibly recorded
 Identified child and women deaths within the surveyed household and alerted
the supervisor for subsequent verbal autopsy interviews
 Carried out other data collection activities as assigned by the field
managers/office
 Reviewed all collected data on a daily basis to identify and correct mistakes or
missing information
 Communicated regularly with the Field Managers to update them on the status
of the field work and all field-related problems
 Submitted all completed data collection forms (or scanned copies) to the office
after final check
 Maintained the safety of instruments and confidentiality of the data
 Maintained supplies of material needed for data collection and communicated
needs for additional supplies to Field Managers in a timely manner
 Demonstrated integrity with the values and ethical standards of Particip GmbH
and the Ministry of Public Health
 Promoted the vision, mission, and strategic goals of Particip GmbH
 Displayed cultural, gender, religion, race, nationality, and age sensitivity and
