As Practice Teacher, you might not be allowed to accomplish all the forms yet,

however, there might be some instances that you are requested to assist in preparing some

school forms. Make sure that you do it accurately and legibly (if done manually) and encode

properly (if done online). Review all entries/have it checked by your Cooperating Teacher

before you finally submit. It is best to have a receiving copy (if done manually) and save the

file (if done online).

Put a copy of the school form that you were requested to do/ accomplish and have it

signed by your Cooperating Teacher.

Read and analyze the salient provisions of Republic Act No. 10173, otherwise

known as the Data Privacy Act. Cite three (3) implications of these in accomplishing

school forms.

WRITING MY REFLECTION
Completing the forms allows me to gain a deeper understanding of my students'

backgrounds. This process provides insights into their beliefs, culture, and other important

aspects, as well as their school performance, including attendance records. If a student

consistently misses school, I will arrange a meeting with their parents to discuss the

underlying issues affecting their performance. This proactive approach helps me address any

challenges the student may face and provide appropriate support.

Reflecting on the completion of School Forms 1 and 2, I am reminded of the

significant impact of the Data Privacy Act (Republic Act No. 10173) on managing student

information. This legislation underscores the importance of obtaining consent before

collecting personal data, ensuring transparency, and respecting privacy rights. While filling

out these forms, I must comply with the legal requirement to seek consent from students or

their guardians, respecting their autonomy in sharing sensitive information.

Additionally, the law requires strict security measures to protect the integrity and

confidentiality of student data. As I collect and process information for School Forms 1 and

2, I must use encryption, access controls, and other safeguards mandated by the Data

Privacy Act to prevent unauthorized access or data breaches, thereby protecting the privacy

of students and their families.
