This workplace project assessment requires you to develop a plan to communicate your personal wellbeing
in the workplace to your trainer/supervisor.

You will also be required to identify access at least one formal and at least one informal resource to support
your wellbeing.

This assessment is divided into five parts:


 Part A: Reflect on Personal Wellbeing
 Part B: Communication Plan
 Part C: Communicate with Supervisor
 Part D: Review Effectiveness of Communication
Part E: Formal and Informal Wellbeing Resources

This project requires you to complete the assessment tasks in a real/simulated workplace environment.
The simulated business, Bounce Fitness, can be accessed by going to

Login to Bounce Fitness using the username and password provided by your trainer.

Each task comes with a set of instructions. You are to follow and perform these instructions while being
observed by the assessor and/or submit any required documentation as evidence of task completion.
Before starting this assessment, your assessor will discuss with you these tasks, including instructions,
resources, and guidance for satisfactorily completing them.

ASSESSMENT INSTRUCTIONS

This task will require you to conduct a self-reflection to recognise factors that may impact your wellbeing in
the workplace.

Use the Reflective Journal Sheet (Appendix 1) provided at the Bounce Fitness site to complete this task.

 Answer preliminary questions about your work role.
 Think of at least three personal factors that can impact your overall health and happiness.
 Think of at least three workplace factors that can impact your overall health and happiness.

i. Due to having the family problem makes me difficult to focus. So, it impacts my wellbeing.

ii. Due to not having enough hours of sleep, it makes me hard to do work and serve as a receptionist. It
impacts my wellbeing.

iii. Due to financial problems in my life, my stress level and panic level high so it makes it difficult to
focus on my work and it impacts my wellbeing.

Responses can relate to how each specific factor affects you physically, mentally, emotionally, or
socially.
i. Having a family problem in my life directly affects my mental health and it also affects me
emotionally, so I am not able to focus on my workplace and it affects my wellbeing and makes me
feel low day by day.
ii. I Have late night work in the workplace. When I reach home it's already midnight and I am not able
to get enough sleep in my nighttime it makes me feel stress and fatigue. So, having this directly
affects my physical and mental health.

about your personal wellbeing in the workplace.
Use your workplace/organisation’s template to complete this task, or use the Generic Communication Plan
(Appendix 2) template provided at the Bounce Fitness site.
To complete this task, you must:
 Identify at least two workplace factors that affect your overall wellbeing in the workplace. For each
factor identified, briefly describe its:
o Positive impact for you in the workplace
o Negative impact for you in the workplace
You may refer to your responses from the Workplace Project Part A, or refer to current situations or
challenges you experience in line with your work role.
 Plan to discuss your wellbeing with your trainer/supervisor based on each of the factors identified.
Complete Part A of the Communication Plan with the following:
o Detail to communicate (Details can be issues or concerns that need to be resolved or
feedback or suggestions to further improve your personal wellbeing in the workplace).

Part C: Communicate with Supervisor
ASSESSMENT INSTRUCTIONS

Your assessor will observe you as you communicate your personal wellbeing with your supervisor (played by
another student) based on the communication plan developed in Workplace Task 2, Part B.
For the purpose of this assessment, the communication method to be used must be verbal.
You will be assessed on your practical skills to:
 Arrange and conduct communication with your supervisor based on the plan
 Use clear and appropriate language and features to discuss with your supervisor
 Demonstrate active listening and questioning techniques to:
o Request assistance
o Confirm understanding
 Establish relationship with the supervisor through honest and open discussion about your personal
wellbeing
 Use problem-solving skills to seek advice on personal wellbeing
To complete the task, you must:
 Send an email to your supervisor to arrange a session to discuss your personal wellbeing.
 Participate in role play with your classmate who will play the role of your supervisor to communicate
your personal wellbeing.
 Complete Part B of the Communication Plan after discussing with your supervisor, include the
following:

This must be consistent with Part A of the communication plan.

ASSESSMENT INSTRUCTIONS

This task will require you to identify, review, and access at least one formal and at least one informal
wellbeing resource as applicable to your workplace.
To complete this task, you must:
 Identify and review the following resources which you can access to support your personal wellbeing
in the workplace:
o At least two must be formal resources
o At least two must be informal resources
You can seek assistance from relevant personnel in your workplace or conduct your own research to identify
formal wellbeing resources.
 Based on your review, select at least one formal and at least one informal wellbeing resource that you
will access.
 Outline the steps you will undertake to access each of these selected resources.
 Provide your responses to the above steps within this workbook.
 Undertake the steps to access each of the required resources and submit evidence to demonstrate this.

Submit your responses to this workbook to your assessor. You must also submit evidence that demonstrates
when you accessed at least one formal wellbeing resource and at least one informal wellbeing resource.

Evidence can include any of the following:


 Formal wellbeing resources, such as:
o Researched information relevant to concerns you discussed with your supervisor about your
wellbeing
o Email correspondence or meeting minutes if contact was done with the agencies or
institutions with programs relating to personal wellbeing in the workplace
 Informal wellbeing resources, such as:
o Email correspondence, meeting minutes, or personal notes with personal relationships

Personal files and/or relevant documents accessed or created to support own personal wellbeing, e.g.
journals or portfolios
