Your Company Name (hereinafter Company), desires to submit a Service Maintenance Agreement proposal to ___________________(hereinafter customer) to provide filter and technical service for ____________________________. Company would like to submit a bid of $_______ for the following work: Air filter replacements on HVAC units serving the interior spaces of the building on a quarterly schedule, based on the following schedule: July, October, January and April. The industry standard fiberglass air filter will be used, unless a situation, temporary or permanent, arises where a higher Merv rated filter is desired, such as dust from construction etc. in nearby area, whereupon Company will contact and apprise customer management and advise concerning options. If a different type of air filter is needed and approved by customer management, the additional cost, if any, will be billed to customer with standard markup. Belt replacement on HVAC units serving the interior spaces of the building, if present on HVAC units, is provided at no additional labor charge. However, customer shall be charged cost of belt with standard markup, with belts being replaced based on the following schedule: Every second contract year, or if not properly rotating blower. Belt changes are documented on Form# W/A AH. Heat Technical on all heating units serving the interior spaces of the building on a yearly schedule, based on the following schedule: October of each year. HVAC units will be logged with Company Form# W/A AH, (attached) with pertinent details observed and recorded by technician. Cooling Technical on all cooling units serving the interior spaces of the building on a yearly schedule, based on the following schedule: April of each year. HVAC units will be logged with Company Form# W/A AH, (attached) with pertinent details observed and recorded by technician. Outdoor condenser unit coils condition will be evaluated using visual observation, and data from the log sheet. This will be used to determine when condenser coils need to be cleaned. When needed, Company will apprise customer management concerning options. Indoor evaporator unit coils will be visually observed, where possible, without cutting sheet metal, etc. If coils are not visible, and data from the log sheet indicates possible evaporator coil blockage, Company will apprise customer management concerning options.

Company will provide the above listed services, upon customer approval, starting on _______________ with an annual rate of $_______, payable quarterly with first payment of $______ beginning on _____________ and every three months thereafter.

General Information Company will provide, at no additional charge, an equipment spreadsheet. This spreadsheet will provide HVAC units location, model and serial numbers, air filters associated with the HVAC units, and information that will be beneficial to customer for future reference, such as unit repair, replacement, competitive bidding, etc. We offer repair and replacement of HVAC units at very reasonable and transparent rates when compared to other service companies. Our technician labor rate is $?? per hour. Drive time to job site is included, with return drive time from job site counted when over twenty-five miles from Companys office. After hours rate on weekdays (before eight am and after four pm) is $?? per hour, and our weekend rates are $??? per hour. Companys Virginia contractors license number is ???? ??????, and a two million dollar liability policy is provided by ???????? Insurance. This proposal is provided for customers use only. The pricing as stated above is valid for thirty days from this date ___/___/____. Either party can terminate this agreement with thirty days written notice to the other party. This proposal becomes a binding agreement after both parties have signed below. This agreement shall continue to renew from year to year unless either party gives a written thirty-day notice.
