WHAT HAPPENED ? PHOTO/SKETCH:

 On the day of incident, Dressing of the


string cable was planned.

 One worker was engaged for the activity.

 A lot of bushes were available in the work Snake Bite location


area.

 Worker started dressing of the string cable,


which was below the module.

 After working for sometime, I/P noticed that


snake was crossed over his left leg.

 He was shifted to the near by hospital


Immediately for treatment.

WHAT WENT WRONG?


 Bushes were not removed from the work area before starting the activity.

 Snake alert stick was not used by the worker.

 Worker was not aware of the biological hazards.

 Effective supervision of the work was not ensured.
