1 SCOPE

This document covers the Health, Safety, Environment (HSE) management programs and procedures
applied to the construction Site.
The Site Manager and the Site HSE Manager shall establish Site HSE programs to enhance all site
personnel HSE awareness, sense of responsibility and their accountability to create a Safety culture that can
deliver the Project HSE objectives and improve their HSE culture. Site HSE activities and programs are
shown in Appendix-2.

2 HSE OBJECTIVES AND TARGETS

Our Project HSE Objectives are:


“Nobody gets hurt, No harm to the Environment and No damage to the Assets through a proactive approach”

The Monthly HSE Objectives and Targets on the construction Site are established by the EPCC
CONTRACTOR Site Manager and the EPCC CONTRACTOR Site HSE Manager, reflecting the construction
progress, HSE status and any major hazardous work anticipated during that month.

5 HSE ORGANIZATION AND RESPONSIBILITIES

5.1 SITE ORGANIZATION

The organization chart (refer to 9952T-000-PP-0802) shows the line of responsibilities between the EPCC
CONTRACTOR Site Manager and the other persons mentioned below.

5.2 HSE ORGANIZATION

Prior to the start of the construction work, the EPCC CONTRACTOR Site Manager shall establish the Site
HSE organization and the Site HSE committee.

5.3 INDIVIDUAL RESPONSIBILITIES

(1) EPCC CONTRACTOR Site Manager


EPCC CONTRACTOR Site Manager has primary responsibility for Site HSE management and
performance.
His major roles and responsibilities are to:

• Establish Site HSE objectives and targets, and visibly demonstrate to the organization that
personal commitment to HSE is his number one priority.
• Ensure the present site HSE management plan is implemented at all stages, by Contractor &

Subcontractors.
• Chair the Site HSE Steering Committee,
• Chair the weekly safety walk and safety critics
• Patrol the Site as frequently as possible to ascertain status of the work and compliance with HSE
requirements and to demonstrate an active and visible attitude toward HSE,
• Direct the Construction Manager, Site HSE Manager and other Managers in carrying out their
roles and responsibilities.
• Ensure that competent resources are timely mobilize in sufficient number to support Project HSE
objectives
• Establish and direct an Accident Investigation Team when an accident occurs and chair the
weekly incident review committee.
• Evaluate HSE reports on HSE performance, HSE topics, and any accidents or near misses
prepared by the Site HSE Manager, and report them to the Project Director and the Project HSE
Manager in a regular and timely manner.
• Communicate daily on HSE issues with OWNER site representative
• Stop any unsafe activity or acts

(2) EPCC CONTRACTOR Site HSE Manager


EPCC CONTRACTOR Site HSE Manager shall assist the Site Manager with HSE matters in
promoting the HSE programs and to ensuring that all HSE requirements are satisfied. The Site HSE
Manager is responsible for monitoring and administering a proactive overall project HSE program
designed to provide assistance in recognizing, evaluating, and subsequently eliminating or controlling
hazardous acts or conditions.
His major roles and responsibilities are to:
• Set up a Site HSE organization and assign Site HSE Supervisors and Officers, including
Equipment/Scaffold Inspectors, and define their roles and responsibilities,
• Set up Site HSE standards, procedures and routine HSE programs including HSE training
program, inspection/audit program, etc.
• Develop specifics training programs that suits subcontractor labour force and charge hand and
foreman.
• Set up the work permit system that will be used during the construction, pre-commissioning and
commissioning periods
• Organize fire prevention and fire fighting activities.
• Organize and regularly test the emergency response program
• Set up Site environment control measures to protect the environment and mitigate adverse
environmental impact of the works
• Advise the Construction Manager and Line Supervisors on HSE matters,
• Conduct HSE training by himself or by using competent HSE Supervisors, or by others assigned
by him, according to necessity and specialty,
• Patrol the Site daily and audit Sub-contractors‘ activities, equipment and facilities to determine if
they are in compliance with HSE requirements,
• Participate / Assist the Accident Investigation Team,
• Chair a weekly HSE Meeting attended by EPCC CONTRACTOR and Sub-contractors’ Site HSE
Staff,
• Report daily to the Site Manager on major HSE activities and topics, and prepare a “ weekly and
Monthly HSE Report” and an “Industrial Accident Status Report” for the Site Manager,
• Submit accident reports to the OWNER and Local Labor Office,
• Continuously evaluate project working conditions and safe work practices, and if warranted,

develop positive & innovative recommendations for project management.


• Monitor the behaviour of the people and provide coaching towards working safety and instill “think
before acting “and “value of human life”.
• Develop meaningful leading indicators
• Communicate daily on all HSE issues with OWNER Site HSE Manager.
• Stop any unsafe activity or act.
• Communicate corporate HSE safety alert to OWNER, EPCC CONTRACTOR &
SUBCONTRACTORS staff
• Prepare regular safety focus flyer for display. And distribution to all project participant

(3) EPCC CONTRACTOR HSE Coordinator


The HSE Supervisors and equipment/scaffolds Inspectors are responsible for ensuring that safe work
practices and equipment standards are being implemented in the areas where they have been
assigned.
Their major roles and responsibilities are to:

• Assist EPCC CONTRACTOR and OWNER Site HSE Managers and subcontractor HSE
personnel.
• Patrol Site as frequently as possible, and inspect current activities and working conditions,
• Stop any unsafe activity or act
• Advise Construction Managers and Discipline Supervisors of any repetitive unsafe conditions or
violations and remedial action .
• Report daily to the Site HSE Manager on any HSE issues.
• Review, approve JSA.
• Inspect construction equipment, tools, lifting gears and maintain inspection logs.
• Liase with respective Area Construction Managers for any HSE concerns
• Assist site personnel during emergency evacuation or drills
• Ensure SUBCONTRACTORS implement Starrt meetings & inspections
• Ensure relevant PTW issued and verify their correct implementation.

(4) EPCC CONTRACTOR Construction Manager


The Construction Manager shall ensure that all the construction work progresses smoothly, safely and
in an environmentally sound manner. This Manager is essential in the implementation of the safety
accountability program.
His major roles and responsibilities are to:

• Ensure that Site HSE plan programs are fully understood and implemented by his staff and
subcontractors.
• Define requirements for additional specific HSE training programs.
• Plan and coordinate the works to ensure that conflicts of interest between Sub-contractors do not
occur,
• Lead weekly TBT for his staff in his area
• Lead incident investigation team for incident occurring in his area and present incident report to
weekly incident review committee
• Guide discipline supervisors in ensuring their roles and responsibilities for HSE,

• Set-up and chair weekly progress meetings, which should include HSE topics,
• Ensure that Sub-contractors analyze all routine and non-routine works and identify any associated
hazards,
• Ensure that Method Statement and Job safety Analysis have been provided for all works, ,
• Patrol the Site daily and ascertain the status of the work and compliance with HSE requirements,
• Promote housekeeping and waste disposal at the highest standard,
• Report to Site Manager on any anticipated HSE matters of concern.
• Participate to TBT, Starrt meeting and Safety Hurdle as frequently as possible
• Ensure that his staff participate to TBT and Starrt and Safety hurdle.
• Participate to HSE incentive program implementation
• Stop any unsafe activity or act
• Develop HSE initiatives to recover from unacceptable safety performance or to continuously
improve site safety climate and standards.
• Identify changes in construction method and risks and required immediate corrective action or
revision of MS and JSA as required.

(5) EPCC CONTRACTOR Discipline Supervisors


The Discipline supervisors organize the works in their scope, and lead their subordinates to ensure
that the work is carried out effectively, safely and in an environmentally sound manner.
Their major roles and responsibilities for HSE activities are to:
• Stop any unsafe activity or act
• Review MS and JSA.
• Approved PTW as per PP 9952T-000 PP 0848, for activities they are supervising
• Participate to incident investigation and assist CM in the presentatrion of incident report.
• Coordinate with other discipline supervisors to ensure that all the Sub-contractors activities are
being carried out in compliance with HSE requirements and standards,
• Ensure that hazards identification, risk analysis and evaluation (risk assessment) are
implemented, and Method Statement are provided incorporating risk control measures for all
hazardous work,
• Ensure that the Sub-contractors obtain any necessary Work Permits,
• Verify that the Sub-contractors work is carried out strictly in accordance with the approved
MethoStatement , associated Job Safety Analysis & Work Permit,
• Ensure that all tools and equipment used meet minimum safety standards,
• Ensure that workers properly wear and use personal protective equipment and safety devices
according to requirements and the manufacturer‘s instructions,
• Ensure that housekeeping standards are maintained, and that a waste disposal program for waste
and scrap materials is set up and enforced,
• Regularly take part in the Tool Box Meetings (TBM) and ensure that all employees actively
participate and receive the necessary safety instructions,
• Report to the Construction Manager on anticipated HSE matters of concern,
• Establish and maintain a motivational environment that includes positive recognition as well as
enforcement.
• Ensure that materiel is delivered to site just in time to reduce site congestion
• Verify that construction aids have been inspected and logs is maintained
• Ensure that their workers are trained and/or certified for the activities they have to perform

6 CONTROL OF SUB-CONTRACTORS

In order to maintain close communication with the Sub-contractors, EPCC CONTRACTOR will hold various
HSE meetings and conduct HSE training as stated in the following Sections.
Sub-contractors’ personnel shall be entrusted with similar roles and responsibilities for HSE management
and practices as described in Section 3, “HSE Organization and Responsibilities”, according to their
organization and the scope of work.

6.1 RESPONSIBILITIES OF SUBCONTRACTORS

Sub-contractors SITE Manager is fully responsible to ensure HSE management and performance of his
scope of work and in the concerned area.
Sub-contractors major roles and responsibilities on HSE management are to:

¾ Submit their HSE organization and HSE management plan to EPCC CONTRACTOR for review and
approval prior to start any activities on site (This plan and organization shall fully comply with the
requirements detailed in EPCC CONTRACTOR Site HSE Execution Plan )

26. Transport safety.

¾ Initiate and chair their own internal HSE meetings,

¾ Assign their own competent and experienced HSE Manager and Supervisors. CV’s to be approved by
EPCC CONTRACTOR

¾ Participate to Site HSE Committee Meetings, Weekly HSE Meeting, & weekly safety walks and safety
critics and inform their subordinates of all-important matters deliberated in these meetings.

¾ Participate to incident investigation and preparation of report. Participate to weekly incident review
committee.

¾ Attend the HSE Training courses organized by EPCC CONTRACTOR HSE staff.

¾ Provide HSE training as deems necessary in addition to EPCC CONTRACTOR Training and
specially dedicated to management, and supervisory level personnel including foremen and for labour
force including charge hand and workers

¾ Provides craft training as required including as a minimum scaffolding, rigging lifting, works at height,
manual handling, as well as 1/3 party certification for crane drivers and plant equipments operators

¾ Provide new or well-maintained tools/equipment and materials, construction aids to be used for the
work and inspect them on regular basis. Maintain log.

¾ Provide all necessary, adequate and task specifics Personal Protective Equipment (PPE) to their
employees and enforce them to wear and use it at all time during the work.

¾ Assign a sufficient number of competent supervisors, foremen and workers to the work as per the
requirements of the project procedure

¾ Assign a sufficient number of safety officers (1:50 workers) as per the requirement of OWNER 6897-
00-19-43-0001 rev 1 to support the implementation of the EPCC CONTRACTOR and subcontractor
HSE management plan programs. Safety personnel have to be approved by EPCC CONTRACTOR
HSE Site Manager prior to assignment, must be fluent in the Arabic and French language and must
hold a First Aid Certification 3rd party organisation. When Local Regulation is more restrictive than
above requirement, Local Regulation must be applied.

¾ Report any accidents, property damage, environmental incidents and near misses immediately to
EPCC CONTRACTOR Site HSE Manager. As per EPCC CONTRACTOR Incident Investigation and
Reporting procedure reference 9952T-000 PP 0856.

¾ Provide a detailed plan of approach concerning his own company safety promotion and incentive
program.

¾ Issue reports and statistic on regular basis, as requested by EPCC CONTRACTOR

¾ Conduct regular inspection of workers and camp welfare

and safety initiatives development program, as well as the implementation of Pulse HSE training
program.

¾ Participate actively in the implementation of EPCC CONTRACTOR Pulse program.

6.2 RESPONSIBILITIES OF FOREMEN AND WORKERS

Sub-contractors’ personnel including foremen and workers shall fulfil their roles and responsibilities
regarding HSE requirements according to their positions and work roles

6.2.1 Responsibilities of foremen and charge-hand

The Sub-contractors foremen and charge-hand major roles and responsibilities are to:

• Incorporate safety instructions in routine orders.


• Understand MS and JSA and insure that workers understand risks and risks mitigation measures
and implement them
• Listen to workers observations.
• Ensure that all new employees understand OWNER and EPCC CONTRACTOR HSE regulations
and standards for work, as well as all concerned Local regulations and laws.
• Discourage horseplay and reprimand those who fail to consider their own well-being and that of
others around them.
• Report promptly to their Supervisor all defects in plant and equipment.
• Ensure strict adherence to HSE Plans and Procedures.
• Ensure that tasks are allocated to workers only in accordance with their capabilities and
qualifications.
• Ensure that all necessary work permits are at the work site
• Conduct the STARRT meetings with their crew.
• Insure that construction equipment, tools, lifting gears and construction aids have a valid
inspection certificates
• Ensure strict compliance with PPE requirements.

6.2.2 Responsibilities of workers

The Sub-contractors personnel’s major roles and responsibilities are to:

• Be aware that HSE is everyone‘s responsibility,


• Attend the HSE Induction Training Course prior to starting their field work, and any specific training

course according to project requirements,


• Strictly follow instructions and directions given by management, supervisors and safety staff,
• Attend the STARRT meetings held every morning,
• Wear and use proper personal protective equipment, safety tools and devices, and keep them in
good condition,
• Always keep their work location neat and tidy.
• Stop the works when unsafe and report unsafe situation to charge hand or foreman.

7 LAWS AND REGULATIONS

SUBCONTRACTOR shall comply with all applicable safety regulations during the transportation of
equipment and materials, construction. In the absence of local governmental regulations, the guidelines of
Occupational Safety and Health Administration (OSHA) shall be adhered to.

SUBCONTRACTOR shall comply and ensure compliance by any of his Subcontractors with OWNER HSE
and EPCC Contractor project HSE plan

SUBCONTRACTOR shall be liable for any breach of these requirements by his personnel and his
subcontractor’s personnel.

SUBCONTRACTOR shall comply and ensure compliance by any of his lower tier Subcontractors with the
latest version of the applicable industry Codes, Standards, regulations and good.

8 HSE MEETINGS AND COMMUNICATION

All meetings and communication will be done in the French language. If international subcontractor
management do not understand French, in this case meeting shall be in English.
Sub-contractor is responsible for further translation to guarantee all members of their workforce will receive
this information in a language understandable.
The results of the meetings shall be made known to all concerned parties and all employees by means of
briefing, notices or posters placed on the HSE bulletin boards, etc.
The minutes of all HSE meetings shall be recorded and retained throughout the entire construction period.
The following HSE meetings will be held to maintain close communication among all the parties and
individuals working on the Site.
All meeting attendees must be registered and registers available at EPCC CONTRACTOR HSE office

The Site HSE Committee that is chaired by the EPCC CONTRACTOR SITE Manager, attendees are:
EPCC CONTRACTOR Construction Managers, HSE Manager and Sub-contractors SITE Managers and
HSE Representatives, and the OWNER Site and HSE Representatives. Project director as well as workers
representatives could be required.
The HSE Committee Meeting is held at least once a month. The minutes of meeting are prepared by
EPCC CONTRACTOR HSE Manager and sent to relevant personnel of OWNER, EPCC CONTRACTOR
and all Sub-contractors.
The major objectives of the Site HSE Committee Meeting are to:
• Confirm the results following the strategic decisions and tactics decided in previous meetings for
continuous improvement or repetitive unsafe situation correction
• Ensure the HSE training programs are implemented and properly carried out to enhance employees‘
HSE consciousness and knowledge,
• Report on recent accidents and near-misses and corrective/preventive measures taken.
• Report on the results of the Behaviour Based Safety program.
• Address area of concern
• Convey major information and instructions from the OWNER and/or the concerned authorities
• Debate and confirm Monthly HSE Objectives and Targets proposed by EPCC CONTRACTOR SITE
Manager.
• Respective site Managers present their presentation of last Month and coming Month HSE issues
• Recommend groups and individuals for HSE incentive awards in accordance with the HSE incentive
award scheme.
• Verify that HSE information has been cascaded throughout the work force.
• Discuss and fix new ways of amelioration of the system for continuous improvement from: medical
statistics, repetitive site observations, and unsafe observations statistics.
• Review HSE 3 month look ahead planning
• Review HSE action register

8.2 WEEKLY HSE MEETING

The weekly HSE Meeting is chaired by the EPCC CONTRACTOR HSE Manager, and attended by the
EPCC CONTRACTOR HSE staff, and OWNER and Sub-contractors HSE Representatives. The HSE
Meeting shall be held weekly and as required
The major objectives of the meeting are to:
• Discuss HSE topics and any problems observed or anticipated,
• Have each Sub-contractors report on the status and performance of HSE activities including HSE
training,
• Report on recent accidents and near-misses, and on follow-up action;
• Report on the results of the Behavior Based Safety program.
• Discuss HSE outstanding action items
• Discuss special construction, PC/C activities that could impact several SUBCONTRACTORS
• Discuss result of STARRT observations

The Progress Meetings held weekly are chaired by EPCC CONTRACTOR Construction Manager and
attended by the relevant EPCC CONTRACTOR Discipline/Area Managers, EPCC CONTRACTOR Site
HSE Manager, Sub-contractors SITE Managers and HSE Representatives.
These meeting are held separately for each subcontractor.
The major objectives of the meeting are to:
• Resolve any issues related to HSE, engineering, procurement and construction interfaces,
• Coordinate the progress and interfaces of each Sub-contractors work to use the working areas most
safely and effectively and to eliminate conflicts of interest among Sub-contractors,
• Review progress with subcontractor, discuss recovery action required and adjust the work schedule,
for the current and following weeks accordingly,
• Ensure that Method Statement (including Job Safety Analysis) are timely provided, reviewed, and
available for execution of the work.

8.4 SAFETY WALKS AND SAFETY CRITICS

The weekly Safety Walk (site inspection) and Safety Critics (debriefing of HSE matters encountered during
Safety Walk) are chaired by the EPCC CONTRACTOR Site Manager, and attended by the EPCC
CONTRACTOR Construction Manager & HSE staff, OWNER and Sub-contractors HSE & Construction
Representatives.
As per schedule, every week, EPCC CONTRACTOR lead supervisors will be assigned to inspect a
defined area. During inspection, they will highlight and record all HSE concerns observed.
Safety Walk is including whole construction site, TSF and lay-down areas
Following the Safety Walk, Safety critics will take place with all participants stipulated here above and
designated lead supervisors will expose their HSE observations.
The major objectives of the meeting are to:
• Involve Construction Supervision in HSE matters
• Highlight all repetitive HSE deviations observed during inspection to OWNER, EPCC CONTRACTOR,
SUBCONTRACTORS
• Expose HSE improvement and safe behaviour observed
• Define Action Plan to correct unsafe observations
• Discuss HSE concerns in presence of all companies
• Critics are recorded by HSE dept and communicated to group leader for action. smiley faces will be
implemented and displayed

8.5 WEEKLY INCIDENT REVIEW COMMITTEE MEETING

The weekly Incident Review Committee meeting is chaired by the EPCC CONTRACTOR HSE Manager,
and attended by OWNER, EPCC CONTRACTOR Site Manager and Sub-contractors HSE
Representatives, and any other persons at EPCC CONTRACTOR Site Manager proposal.

The major objectives of the meeting are to:


• Present incident investigation reports for HiPo, FAC, MTC, RWC, LTI, …

• Follow the correct implementation of the actions decided the past weeks and correct if required

• Avoid reoccurrence of incident already observed during the project

8.6 SAFETY TASK ANALYSIS RISK REDUCTION TALK (STARRT)

The STARRT process is one of the most effective means of eliminating accidents/incidents and enhancing
the HSE awareness of all personnel. The STARRT is a short meeting by individual work groups, and is
held in the close vicinity of the work location, prior to starting the work of the day.

The group leader (charge hand foremen or supervisor) conducts the STARRT and the whole crew shall
actively participate in the STARRT. EPCC CONTRACTOR managers and supervisors shall also attend
STARRT.
Mainly, the working plan and procedures for the day are explained and confirmed including the following:
• Briefing on topics raised in the weekly HSE meetings,
• Anticipated potential hazards and suitable countermeasures (Incl. fire prevention),
• Safety instructions for equipment and tools to be used,
• Inspection of equipment, tools and facilities,
• Confirmation that any Work Permits required for the work have been obtained,
• Use of proper personal protective equipment and safety devices,
• Information on other work to be conducted in the vicinity of the working area,
• Visual check of the health condition and proper allocation of workers according to the requirements.

8.7 TOOL BOX MEETING (TBM)

A TBM is one of the most effective means of enhancing the HSE awareness of all personnel. The TBM is a
short meeting held at least on a weekly basis and covering specific safety issues decided by the site HSE
team based on previous incidents or observations.
This meeting held by each SUBCONTRACTOR and EPCC CONTRACTOR attended by all employees
(Management, Supervision, workforce, office employees, …) and chaired by Site Manager or its delegate
assisted by HSE Manager shall address all issues of the ongoing activities, communication and sharing of
information from OWNER, EPCC CONTRACTOR and SUBCONTRACTOR.

9 HSE TRAINING AND CERTIFICATION

HSE Training at the Site consists of 10 programs that will be implemented according to the training matrix.

EPCC CONTRACTOR HSE Manager and his team set up the HSE training programs and materials and
conduct the various courses with the support of Sub-contractors HSE Managers and Officers.

The language used by instructors shall be in the French language, in case of different language spoken by
attendance, a translator shall be used. Sub-contractor to foresee assistance for translation (training and
documentation) if sub-contractor employees require another language for training to have a good
understanding. Training records shall be retained at EPCC CONTRACTOR HSE Office throughout the entire
construction period.
In addition EPCC Contractor will implement his Pulse training program consisting of
- Phase 1. The Pulse leadership workshops (EPCC CONTRACTOR and Owner)
- Phase 2. The Managers and Supervisors training (EPCC CONTRACTOR, Subcontractor and Owner)
- Phase 3. The Site based training (Subcontractor workforce)

9.1 HSE INDUCTION TRAINING FOR NEW ENTRANTS

It is mandatory for all managers, employees and visitors of OWNER, EPCC CONTRACTOR and its Sub-
contractors to attend the HSE Induction Training course prior to start any fieldwork. Without attending this
training course, they will not receive their ID card necessary to enter the construction Site.
Prior to the HSE Induction Training, all personnel shall submit their “Personal Information” using the
designated form (name, age, gender, profession, physical status, experience, license or certification, etc.).
The Induction Training includes, but is not limited to the following

(1) General Orientation


• Stresses the importance of HSE and each individual’s responsibility,
• Orientation on the Site layout and the work place, using maps/drawings,
• Working rules (duty hours; overtime work; holidays; transportation; prohibition of smoking, drugs and
alcohol; possession of firearms, etc.),
• Participation in the TBT and the STARRT program,
• Security rules, including Gate Control, Traffic rules and regulations,
• Actions to be taken in case of emergencies, (Outline of the Emergency Response Procedure),
• Work Permit System,

(2) Instruction for Individual Work Categories:


Individual instruction shall be given to SUBCONTRACTORS and EPCC CONTRACTOR personnel
according to their work categories, such as but not limited to:
• Work at heights (prevention of falls):
Safe working platforms, handrails, protection around openings, scaffolds, use of safety harnesses,
etc.
• Hot work (prevention of fires and accidental burns):
Gas cutting and welding work, electric-arc welding, handling acetylene and oxygen cylinders, use
of fire extinguishers, etc.
• Crane operation and lifting work, rigging
Crane certificates, operator’s license, safe operating manner, use of outriggers, inspection and
maintenance of equipment, signals, rigging work, sling work, safety devices,
• Operating vehicles and construction equipment:
Safe operating practice, operator’s license, maintenance and inspection, speed limit, etc.
• Electrical work:
Safe working practice, electrical isolation, LOTO, grounding, circuit breakers, electric-arc welding,
safety tools and use of insulated personal protective equipment, etc.
• Work in confined spaces:
Work Permit System, pre-job inspection/measurement, entry control, ventilation, respiratory
apparatus and its use, evacuation system, etc.
• Handling of hazardous substances:
Material Safety Data Sheets, (MSDS), specific PPE, waste management process, etc.
• Manual handling
• Excavation works
• Heat stress
• Housekeeping, noise and dust suppression.

9.2 HSE TRAINING FOR MANAGERS AND SUPERVISORS

In addition to the Induction Training for new entrants, specific HSE training will be given to EPCC
CONTRACTOR, OWNER and Sub-contractors’ Managers, Supervisors and Foremen by EPCC
CONTRACTOR HSE Manager and his staff.

9.3 HSE TRAINING FOR SPECIFIC HAZARDOUS WORK

Competent and qualified persons shall give specific training to any personnel involved in hazardous or
harmful work such as but not limited to

• Handling and storage of hazardous substances


• Deep excavation and trenching
• Work in confined areas or narrow spaces
• Rigging, lifting and crane operation
• Erection and dismantling of scaffolds
• Electrical safety (Licensed electrical workers, Explosive powered tools operator, etc.LOTO)
• Work in or near an existing plant (regardless of whether operating or not)

9.4 STARRT TRAINING

The STARRT (Safety Task Analysis Risk Reduction Talk) program is designed to train supervisors and
foremen in analyzing the tasks to be performed, identifying and eliminating hazards and communicating
with their crews on how to do the job safely. They will be trained in STARRT within three months of the
project kick-off.

9.5 BBS TRAINING

The BBS (Behavior Based Safety) is an approach to accident prevention that will help achieving
continuous safety performance. Primary in BBS, worker’s behavior is observed (pre determined checks
lists shall be used) and information via feedback on their problems and difficulties in working safely are
gathered.
Selected personnel will be trained as BBS observers and will then carry out daily observations in order to
report on both safe and unsafe situations and acts.

9.6 REFRESHER HSE TRAINING

All employees who seriously or repeatedly violate the HSE Requirements and Regulation, and are so
designated by the HSE Manager, shall receive a Refresher HSE Training Course so as to maintain their
HSE awareness at the highest level possible.

9.7 FIRE EVACUATION AND FIRST AID TRAINING

Basic Fire fighting and First Aid Training will be given to all supervisors and safety personnel as well as to
any volunteers that will then form the Project Emergency Response Team.
Emergency drills on any potential emergencies will be conducted periodically in order to assess the
reactivity of all employees as well as those of the Emergency Response Team.

9.8 PRE-COMMISSIONING / COMMISSIONING TRAINING

Specific Pre-commissioning / Commissioning Training will be given to all personnel remaining on site at
the start of Pre-commissioning / Commissioning activities. This training will cover all rules that will be
changed or modified due to the introduction of new risks on site. (Work permits, restricted access, etc.)

10 HSE INSPECTIONS AND AUDITS

10.1 HSE INSPECTIONS

HSE inspections consist of general inspections and scheduled (formal) inspections as detailed in the
inspection and audit matrix (to be provided later)
General inspection means the habitual observation of on-going work practices and working conditions and
activities by managerial personnel, supervisors and foremen of EPCC CONTRACTOR and Sub-
contractors.
Scheduled inspection means a periodic or specific inspection, which is set up by EPCC CONTRACTOR
HSE Manager. The HSE Manager shall decide a scheduled inspection program including participants,
areas to be inspected, frequency of inspections, etc.
During inspections, particular attention is paid to discovering any hazards or unsafe activities, critical and
hazardous working conditions, health and hygiene issues, security or environmental problems.
Any situations or practices, which are deemed unsafe or substandard, are directly notified to the parties or
personnel concerned and worked stopped if necessary The details of the nonconformance investigation
and the instructions for corrective action shall be reported to EPCC CONTRACTOR and the Sub-
contractors’ managerial and responsible personnel.
If an immediate danger is presented to personnel or property, all related work shall be immediately
stopped, or the use of the facilities or equipment shall be prohibited until the defect is remedied or
controlled.
These inspections are supported by checklists.

10.2 EQUIPMENT INSPECTIONS

All lifting equipments (cranes, forklifts, …), rigging appliances (shackles, slings, beams, …), electrical
tools, pneumatic tools, pressure equipments shall be inspected and tested by competent mechanics or
electricians on arrival at the construction SITE, periodically according to the applicable procedure, and in
compliance with the national and/or local laws and regulations, to confirm they are in a safe operating
condition. Inspections to be done by Sub-contractor and EPCC CONTRACTOR
Certification tags shall be displayed on accepted equipment, and details of certification will be forwarded to
EPCC CONTRACTOR HSE Office to be registered in a log and kept updated in the EPCC CONTRACTOR
HSE Office.
The user or operator shall carry out daily inspections supported by checklists before the construction
equipment or tools are used.

Instructions on the use of the daily inspection checklists shall be given to the users or operators before
they start their work.
EPCC CONTRACTOR Site HSE Manager shall prepare a standard form for each type of equipment to
record the inspection results, any substandard conditions, and maintenance or repair completed.
Where the national and/or the local laws and regulations specifies the inspection of major equipment, the
supplier of the equipment shall arrange legal inspection by an authorized third-party inspection company,
according to the type and/or capacity. It concerns mainly lifting equipments, rigging appliances, but
A copy of the certificate issued by the inspector shall be submitted to EPCC CONTRACTOR HSE Office
for review and recording.

10.3 HSE AUDITS AND MEASUREMENT

EPCC CONTRACTOR Corporate HSE Department and the Project HSE Manager will jointly audit and
conduct HSE inspection patrols on the SITE, at least every six months and as necessary to check the
implementation of recommended preventive or corrective actions.
The purpose of the audit and patrol of the SITE is to confirm whether the Project HSE management and
activities are complied with OWNER and EPCC CONTRACTOR HSE requirements and expectations, and
to enhance the HSE awareness of the SITE personnel.

Site HSE Manager will carry out the internal HSE audit of the Sub-contractors at least quarterly. Program
must be defined every start of quarter and notification must be sent to concerned Subcontractors at least 2
weeks prior the audit realization.

10.4 NOTICES OF UNSAFE ACTS AND CONDITIONS

When an unsafe act, unsafe condition or substandard equipment/facilities is detected, the finder must not
overlook nor ignore the situation. The finder should immediately take proper action to make the directly
responsible supervisors /foremen or workers rectify the situation. He should then record the action using a
BBS check sheet form.
This will be supported by a specific incentive scheme as described in the “Incentive Schemes Procedure”.
Where an immediate threat to personnel or property is recognized, all related work shall be stopped and
use of the facilities or equipment shall be prohibited until the defect is remedied or controlled.
The stoppage of the work or prohibition of the use of the equipment or facility shall be reported using the
BBS check sheet form, which shall be issued to the Site HSE Manager and Sub-contractors SITE
Manager or HSE Representative.
The Sub-contractors SITE Manager or HSE Representative shall instruct his subordinates to immediately
rectify the defect, and shall promptly report to EPCC CONTRACTOR Site HSE Manager on the remedial
action status and the results.
The safety/security directives and reports of remedial actions shall be recorded and retained in EPCC
CONTRACTOR HSE Office, during the entire construction period.
Unsafe or substandard situations shall be disclosed and deliberated at the HSE meetings by all personnel
concerned to prevent a recurrence of the defect.

The major observations found during an HSE inspection shall be used to develop, modify or improve the
work execution plans and procedures and/or the HSE procedures or requirements.

10.5 SAFETY TAG SYSTEM

The Site HSE Manager shall establish a safety tag system for specific equipment and facilities, such as
scaffolds, permanent work platform completeness, lifting equipment and lifting gears, electrical equipment
and tools, so that permission is obtained before use..
After the nominated inspectors’ inspection, the inspector shall issue a “green safety tag” for the equipment
or facility, if acceptable.
The validity date for use, the date of inspection, the signature of inspector, and any other important
information shall be entered into the Equipment Logbook, kept by the HSE Office and as much as possible
mentioned on the tag itself.
When any defect occurs in equipment or facilities, or a device malfunctions during the inspection, a red tag
shall be attached to the defective equipment or facilities indicating use is prohibited and the reason why.
Nobody may use the defective equipment or facilities unless it has a “green safety tag”.
The Sub-contractors shall take remedial action to replace the equipment, facility or the relevant
components. Afterwards, the Sub-contractors shall contact inspector to re-inspect the defective item.

11 RISK ASSESSMENT AND WORK EXECUTION PROCEDURES

11.1 IDENTIFICATION OF HAZARDS AND RISK ASSESSMENT

The identification and analysis of loss potential (Risk Assessment) and the establishment of measures to
prevent accidents/incidents and/or property damage are essential for executing the work properly and
safely. All work activities shall be systematically analyzed by competent personnel to identify any serious
loss potential anticipated during the execution of work.
The work analysis and identification of hazards will be used to support the development of effective and
safe work execution procedures, and include to:
• Examine how the work is to be performed and write down the work steps and procedures,
• Identify potential risks for each work step,
• Analyze and evaluate risks according to severity and probability, and evaluate whether risk control or
risk education measures are required,
• Examine and establish risk control or risk reduction measures (Elimination of hazards or reduction of
risk)

The risk control or reduction measures shall be practicable to execute and require reasonably minimal cost
and effort.

11.2 WORK EXECUTION PROCEDURE / METHOD STATEMENT

The results of the work analysis and risk assessment will be recorded and used to establish Work
Execution Plans, incorporating risk control or risk reduction measures.
For specific hazardous work, the Construction Manager and the Discipline Managers/Supervisors are
responsible for preparing a comprehensive list of hazardous works and ordering Sub-contractors to
prepare and submit work execution procedures for such hazardous work.
Competent and experienced personnel designated by Construction Manager will review the Work
Execution Plans submitted by Sub-contractors.
The following construction/erection activities, without limitation, are generally considered as hazardous
work and are covered by dedicated procedure:
• Installation and dismantling of scaffolds,
• Work on Energized Electrical Systems, (hot-line work),
• Work inside confined spaces, including rescue operations,
• Installation of heavy, tall, or unbalanced equipment and structures, using crane, gin pole, etc.
• Structural steel erection and general work at heights.
• Heavy lifts, (in excess of 20 tons)
• Use of crane hoisted man baskets
• Installation and use of cable climbers and gondolas
• Ionizing radiation work,
• Abrasive grit blasting and spray painting,
• Work on oil-filled/gas-in utilities, electricity energized equipment,
• Handling and storing of organic solvents and toxic or harmful chemicals,
• Deep excavation for foundation and trenching,
• Blasting activities requiring the use of explosives,
• Any work involving unique or specialized machinery or equipment operations
• Work requiring the Lock out and Tag out of equipment.
• Any work requiring the use of respiratory protection, other than the use of standard disposable
nuisance dust masks.
• Hydrotesting of tanks, lines, columns, vessels, equipment, instrument system.
• Pneumatic Testing, Air Blowing, high pressure and chemical cleaning,
• All works within OWNER area, and
• Other work designated by the EPCC CONTRACTOR Site Manager or when requested by
OWNER.

The work execution procedures including a dedicated Job Safety Analysis (JSA) submitted by Sub-
contractors are examined by competent and experience personnel designated by EPCC CONTRACTOR
Construction Manager.
Those responsible personnel shall explain the contents of the work execution procedures to the personnel
concerned with the work, prior to commencement of the work and at the daily STARRT meeting.
Accidents/incidents also occurred during minor work or routine work. Therefore, responsible personnel
concerned with the work should also examine such minor or routine work other than the work listed above,
regarding potential hazards and measures to prevent potential losses.

11.3 JOB SAFETY ANALYSIS

Work shall not be conducted without a job safety analysis (JSA) or pre-job risk assessment discussion
(STARRT) appropriate for the level of risk, in accordance with the “Risk Assessment and Work Execution”
9952T-0000-PP-0849
For hazardous activities as defined in paragraph 8.2, Sub-contractors shall perform a detailed JSA that will
be attached with their work execution procedure.
For any other jobs, Supervisors/Foremen will perform a JSA with their crew to identify the potential
hazards that the group may encounter, and eliminate the hazards or implement the necessary controls.
For that purpose, they will use the STARRT cards (Task assessment and implementation card) that will be
signed by all employees at least on a daily basis.
Another best practice is to hold a follow-up meeting after completion of the task or workday to identify safer
and more efficient work methods and ensure there were no incidents or injuries. After the analysis is
documented, it is kept as a reference for future similar operations.
At shift changes, supervisors/foremen shall review active JSA or STARRT card and inform their
employees of any changes. Supervisors are responsible for ensuring all workers are familiar with
applicable JSA or STARRT card prior to commencing work.

11.4 SAFETY TASK ANALYSIS RISK REDUCTION TALK (STARRT)

The STARRT (Safety Task Analysis Risk Reduction Talk) process is held before beginning work each
workday; when starting any new tasks during the day, or when a safety problem has been identified in
crew involving more than one individual.
The basic steps of the STARRT process are to:

• Assemble the work team (ensure all people participating in the task are involved in this process),
• Ensure all persons are trained and competent in the work they conduct,
• Define the task or scope of work,
• Clearly define individual sequential step to be done,
• List the workers and equipment to be used in the job,
• Identify and list potential job hazards and employees’ exposure for each step of work.
• Define the prevention measures to be implemented

11.5 PRE-COMMISSIONING/ COMMISSIONING

11.5.1 SIMULTANEOUS OPERATIONS (SIMOPS)

For ensuring that all hazards are controlled, EPCC CONTRACTOR will apply following safety
management system for simultaneous operations of pre-commissioning, commissioning and construction
activities (SIMOPS).

(1) Pre-start up HSE review


Prior to commencement commissioning and start-up activities, pre-start-up HSE review will be carried
out to check the readiness. The review includes the following.

• All recommendations raised during previous HSE reviews are implemented in the installations.
• Completion of operation test of safety systems, fire protection system and environmental
protection system and they are ready for operation. Also all required personnel protection
equipment
• All documents related to safety and operations (such as safety manuals, operation manuals,
procedures, emergency response plan, etc.) are available.
• Training is conducted for all personnel, who will engage in pre-commissioning, commissioning
focused on potential risk, precaution for eliminating the risks, use of personnel protection
equipment, measures in case of accident/emergency and rules to be observed.

(2) Custody transfer of SYSTEMS


When all required inspection/test of component included in the SYSTEM are completed and
documented, the custody of SYSTEMS is transferred to a commissioning team from construction
team. The extent of the SYSTEM is to be clearly defined on P&ID with component list included in the
SYSTEM.
Upon transfer of the custody, the physical area of the SYSTEM shall be identified with signs and
barrier tapes by commissioning team. The access and/or entry to the area is restricted to essential
personnel who have been trained for commissioning and holding a project specific
“COMMISSIONING” badge.

(3) Control of risks and permit to work


Any construction activity inside the area of commissioning team’s custody shall be performed under
strict “Permit to Work” system.
Construction supervisor will issue an application of “Permit to Work” to commissioning team describing
the details of work (method statement) including precautions/measures to be taken.
The joint team of construction supervisor, commissioning supervisor and HSE supervisor shall review
the application and;
• Identify hazard/risk involved in the work.
• Assess if the precautions/measures are adequate against the identified hazards/risks, and prepare
additional precautions/measures as required.

Before start of the work, construction supervisor and commissioning supervisor jointly check and
confirm that the precautions/measures are provided and observed per the “Permit to Work”. During the
check, special attention shall be paid that valve position (open/close), and isolation blind are set with
caution tags for isolating live systems/lines as per the “Permit of Work”

During the execution of the work, the construction supervisor shall watch if the work is performed in
compliance with conditions of “Permit to Work” as well as other safety rules. Upon completion of the
work and clean up the area, the construction supervisor and commissioning supervisor jointly confirm
the completion of the work.

(4) Communication and Co-ordination


For facilitating communication and co-ordination between construction team and commissioning team,
a co-ordination meeting shall be held on a weekly basis. In the meeting following is to be discussed.
• Review of activities performed previous week, and prepare resolution/ improvement plan if any
problems are detected.
• Planned activities for next week by both construction and commissioning team.
• Commissioning team to inform schedule and location of commissioning activities such as
blowing/flushing, machinery run-in test and introduction of feed stocks/utilities into systems and/or
piping.
• Construction team to inform schedule and location of construction activities within/near-by the
SYTEMS under commissioning.
• Coordination of the progress and interfaces of each activities to use the working areas most safety
and effectively and to eliminate conflicts of interest among teams.

In addition to above weekly co-ordination meeting, a toolbox meeting will be held every morning to
confirm activities and the latest situations of the day.
The weekly and daily activities of pre-commissioning and commissioning as well as the information of
live systems/lines will be distributed to all parties of construction office including Sub-contractors, and
the same will be displayed on HSE notice board

11.5.2 RISK ASSESSMENT FOR HAZARDOUS WORKS

(1) Testing Activities


The following testing activities, such as but not limited to, are generally considered as specific
hazardous work:

Steam blowing of line. Steam will heat up the lines and equipment, which can cause
burns. Steam blowing is also associated with high levels of
noise, which may harm the ears.

(2) Live Lines


After the lines have been cleaned, pressure tested and dried to the proper dew point, they are
commissioned. This is when they are charged/loaded/filled with the products they were designed to
carry during the facility's operation i.e.:- Hydrocarbons, nitrogen, gases, potable water, etc.
They will then be marked with stickers on which "LIVE LINE" is written.

11.5.3 SAFETY CONTROLS

Safe work procedures and safety systems shall be implemented in an effort to prevent injuries, loss of life,
and damage to equipment or environmental contamination during "start-up" and commissioning activities,
that are high potential criticity activities. It is imperative that EPCC CONTRACTOR strictly adhere to their
procedures and systems at all times for protection of its employees and those around.
(1) Safety Action Plan
EPCC CONTRACTOR shall develop a Safety Action Plan and submit it to OWNER for review and
approval. EPCC CONTRACTOR shall also submit associated procedures to OWNER for review and
approval 15 working days prior to the start of any pre-commissioning and or start-up work (for Class 1
procedure).

(2) Permit to work


ALL WORK IN THE "START-UP" and RESTRICTED AREA'S WILL ONLY BE CARRIED OUT ON AN
AUTHORISED PERMIT.
EPCC CONTRACTOR shall submit OWNER Permit to Work System for an operating facility and
attach it to the Safe Work Plan for OWNER approval/authorization at least 24 hours before the start of
the work.
The system and procedures constitutes the most methodical means of ensuring that safe work
practices are followed and that the work environment is safe. The objective is for an experienced and
trained authorized person, (who will ultimately sign the permit-to-work), to pre-assess (with all the
necessary technical assistance) the hazardous circumstances involved and prescribe:

• All precautions required.
• Who may perform the work
• The limits of the permit area and equipment
• LOTO (Log Out Tag Out) requirements

In particular, the permit will assist the authorized person in prescribing all the necessary precautions.
Each permit should detail the specific precautions to cope with, for example, confined spaces
excavations, pressure tests, hydro static tests, radiographic tests, spading, mechanical and electrical
lock-out etc. The permit is designed to ensure that all parties involved are aware of:

• The nature of the work to be performed


• The place the work has to be carried out
• The equipment or plant involved
• The period of time in which the work may be carried out
• The hazards which are, or might be, present
• The tests and checks which have to be made and the precautions to be taken before starting the
work
• The equipment to be used or to be made available on a standby basis
• The personal protective equipment to be used by those involved in the work
• The requirement, if any, for further periodic tests and checks
• Personnel permitted to do the work
• The emergency/rescue procedure and other arrangements for the evacuation of personnel

The supervisor in charge of the work to be performed must make sure that he and his crew fully
understands all the permit conditions and will be able to comply.
Therefore the supervisor in charge must personally check all the tools and equipment to be used, the
work environment, PPE and explain the work and the permit conditions to his crew before the work
commences, actions done during STARRT meeting.
A work permit therefore authorizes entry into "start-up" area's/process units/restricted areas.
EPCC CONTRACTOR supervisor shall always ensure that the following requirements are carried out:

• Sparks must be contained with fire resistant material (i.e. fire blanket, etc.)
• All sewers/drains within 10 meters must be covered.
• A fire extinguisher(s) must be available at the work point.
• A metal container to deposit welding rod stubs, - never deposit welding stubs anywhere else than
in such metal container.

(3) Barricades/Fences
Barricades and fences are there to prevent people from going into hazardous/dangerous areas.
This should not be taken lightly and everyone should comply. In general barricades are required
around excavations, holes, openings in floor or roof areas, edges of roofs, elevated platforms, around

certain types of overhead work, around areas where radioactive tests are performed, where steam,
chemicals or air blow-outs are possible because of pressure tests, where equipment could be hot, etc.
All areas where hazardous pre-commissioning and or start-up work is being performed shall be
properly barricaded and access into the area controlled. EPCC CONTRACTOR shall keep a sign
in/out log of all authorized people entering/exiting the restricted area.

(4) Banners and Signs


Banners and signs are displayed on SITE to warn or inform people of hazards and hazardous zones,
or to prescribe the use of certain personal protective equipment or to prohibit certain activities. EPCC
CONTRACTOR shall display banners and signs to warn people against the possible hazards and
dangers. The following are possible but not limited:

• Prohibited smoking zones.


• In noise zones to wear ear protection.
• Touching hot pipes, valves, equipment.
• Chemical cleaning of pipes, valves and equipment.
• Chemical, Oil spillage.
• Possible blasting particle during air blowing activities.
• Electrical hazards.
• Hazardous work overhead.
• Permit areas.
• Various pressure tests and associated hazards/dangers.

11.5.4 HAZARDOUS MATERIALS/SUBSTANCES

Hazardous materials are substances capable of posing a risk to personal health and safety. Many of the
pipe lines and vessels will contain such hazardous substances.

The best way to protect your health and safety is to know about each of the chemicals being handled or
worked with and comply with the conditions of use described in the relevant Material Safety Data Sheet
(MSDS).
Hazardous materials/substances are generally be classified as follows:

Toxic Materials A toxic material is one which adversely affects the body and organs.
Carcinogenic Materials, which have the potential to cause cancer, are known as
Materials carcinogens.(Substitutes whenever possible are to be used)
Corrosive Corrosive are chemicals that cause visible destruction or changes in living
tissues by chemical action upon contact.
Irritants An irritant is a non-corrosive chemical that causes a reversible
inflammatory effect on living tissue by chemical action on contact.
Asphyxiates Chemicals/gases which can cause suffocation by restricting the uptake of

oxygen or by respiratory paralysis or by diluting/displace oxygen below


levels needed by the body.
Flammable & Liquids that give off enough vapor to form an ignitable mixture with air and
Combustible Liquids produce a flame when a source of ignition is present.

Sub-contractors must receive approval from Owner & EPCC CONTRACTOR for all new hazardous
materials / substances on site. To receive this approval, Sub-contractor must issue the MSDS of the
hazardous material / substance to EPCC CONTRACTOR HSE Manager.
MSDS of zardous materials / substances must be available & displayed at place of storage and use.
Copies of all MSDS must be available at EPCC Contractor HSE Office & in the Site Clinic.

11.5.5 TRAINING

EPCC CONTRACTOR shall ensure all employees involved in pre-commissioning and start-up work are
trained in their task and associated hazards and dangers of their work.
All employees shall attend the Pre-commissioning and Start-up safety orientation training during this phase
and a color code employee ID badges to indicate training received will be applied.
Only people who have received this orientation training and are authorized on Permit to Work will be
allowed to perform work inside restricted areas.

12 PRACTICAL SAFETY PROCEDURES

Practical HSE standards will contain the minimum requirements for safe working practices and equipment
standards, and consists (but are not limited) of the following major subjects:
• Working at heights
• Scaffolds Metal-tube/system/slung/bracket type/movable scaffolds, safety nets, staircases and
ramps, ladders and trestles, erection and dismantling
• Prevention of accidents due to falling objects
• Prevention of collapses Excavation, concrete form shoring, erection of steel structures,
demolition work, gas detectors in excavations, etc.
• Electrical work on construction Sites Electric appliances for construction, movable wiring,
transformer banks, electrical distribution panel boards, circuit breaker, electrical work, overhead
power lines

13 WORK PERMIT PLAN

13.1 GENERAL

The Work Permit System on the brown field shall be well coordinated with the OWNER especially for work
inside or in the vicinity of existing plants, regardless of whether they are being operated or not. Within
green field area EPCC CONTRACTOR PTW system will be implemented
Every item of work to be done must be evaluated to determine what specific work permit is required or not.
The following work is generally subject to the Work Permit System:

• Work inside or in the vicinity of an existing plant


• Hot work in designated locations (inside confined spaces, near combustible material, cf. “Permit To
Work” 9952T-0000-PP-0848)
• Cold work in designated locations
• Entry in a confined space
• Deep excavations and trenching
• Pressure testing (hydrostatic, pneumatic)
• Entering or operation of mobile cranes, vehicles or other construction equipment at designated
locations
• Radiation work
• Any work on electrically energized circuits and equipment
• Hand rail & floor removal
• Critical Lifting
• Other work or locations designated by OWNER or EPCC CONTRACTOR

In addition, extended hours and Friday works are subject to formal agreement by EPCC CONTRACTOR.

13.2 WORK PERMIT PROCEDURE

The work permit procedure and various works subject to the Work Permit must be established and fully
informed to all Sub-contractors and personnel responsible for the work.
The Construction Manager and HSE Manager are responsible for finalizing a written Work Permit
Procedure describing the following:

• Conditions and type of work requiring Work Permits


• Department/ Authority who issues Permits
• Deadlines for submissions of applications
• Application process/steps
• Survey and/or measurements required

For the existing plant areas and as soon as hydrocarbons are introduced in the new units, OWNER work
permit procedure shall be applied and the requirements reflected in EPCC CONTRACTOR Work Permit
Procedure.

14 ENVIRONMENTAL CONTROL PLAN

EPCC CONTRACTOR fully recognizes that management of environmental matters during construction and
commissioning work is very important. These responsibilities include the proper handling of waste materials
and environmental protection.
The EPCC CONTRACTOR SITE Manager, Construction Manager and HSE Manager shall establish an
environmental management plan to handle waste materials properly, and to prevent or mitigate adverse
environmental impact and environmental pollution due to accidental discharges, spills and emissions of
gases, liquids, or noises, etc., anticipated during construction and commissioning work at the construction
SITE or other temporary facilities associated with the work.
The EPCC CONTRACTOR SITE Manager shall assign experienced, competent personnel for monitoring
and control of such environmental pollution. These personnel shall be supplied with adequate measuring
instruments and apparatus.
All measurement shall be conducted in accordance with local regulatory or OWNER requirements.
The following environmental problems are often observed at construction Sites, and the countermeasures
described below are usually implemented to prevent environmental hazards:

14.1 WASTE MANAGEMENT

The objectives of waste management activities will be:

• To reduce waste generation and to recycle or reuse them as much as possible,


• To ensure a collection of all construction wastes in a timely manner to avoid any temporary waste
disposal around the site or on laydown areas,
• To ensure the responsible and safe disposal of wastes, as defined by the environmental requirements,
• To ensure safe, ultimate disposal of hazardous waste (if any) while retaining control for future
treatment.

All companies, including EPCC CONTRACTOR, shall be responsible for waste, debris, and surplus
materials from their work or locations.
Flammable waste, non-flammable waste, hazardous waste and perishable waste shall be stored
separately in proper containers or bins.
All companies shall be responsible for removing their own waste from the SITE to the designated disposal
locations.

EPCC CONTRACTOR HSE Manager shall establish the On-SITE waste disposal policy and procedure of
the manifest control of waste transportation, disposal or treatment in line with the statutory requirements.
Before the disposal of waste, the HSE Manager and Subcontractor’s SITE Manager shall contact the
waste transporters and the waste disposal or treatment EPCC CONTRACTOR licensed by the local
authority.

(1) Non-Hazardous Waste

EPCC CONTRACTOR and all its Sub-contractors are responsible for the clean up and disposal of waste,
debris, and surplus materials that they generate during the work.
Flammable waste, non flammable waste, hazardous waste and perishable biodegradable waste shall be
stored separately in properly designated containers or bins.
Each subcontractor shall be responsible for disposal of the waste generated by their activities to an
approved waste disposal facilities approved by EPCC Contactor

(2) Hazardous Waste

Before Subcontractors dispose of substances which are categorized as hazardous in accordance with
statutory regulations, they shall notify EPCC CONTRACTOR SITE Manager with documentation
describing the chemical property, category, and quantity of waste involved.
SUBCONTRACTOR SITE Manager shall obtain approval prior to the of EPCC CONTRACTOR & Owner.
Each subcontractor shall be responsible for disposal of the waste generated by their activities to an
approved waste disposal facilities identified by EPCC Contactor and approved by Owner.

(3) Pest Control

No person shall place, leave, dump or allow to accumulate any garbage, refuse or litter in any building or
any premises or open SITE in a manner that will provide food and nutrients for insects, rodents, or wild
animals.

(4) Sewage Disposal – Sanitary Waste

EPCC CONTRACTOR and its Subcontractors shall provide sewerage systems in offices and camp with
collection to holding tanks. Vacuum truck will empty the tank regularly and the sewage will be transported
to an approved sewage treatment plant.

(5) Solid Waste

EPCC CONTRACTOR will undertake an assessment of the options for disposal of solid waste in order to
identify acceptable disposal options. EPCC CONTRACTOR will produce a tabular Waste Inventory
quantifying all wastes produced during the construction and commissioning phases of the project.
The assessment will identify all solid waste that may be produced during construction and commissioning
of the plant and will include an analysis and recommendation of the disposal route based on the hierarchy
of prevention, recycle/reuse, treat or dispose.

(6) Medical waste

To be considered as Hazardous Waste (according to Algerian law of the 12th december 2001).

14.2 GENERAL APPROACH TO RE-USE, RECOVERY AND RECYCLING

EPCC CONTRACTOR shall ensure waste minimization and management and develop a Waste
Management Plan. The Waste Management Plan shall be in line with local regulations and used to control
every waste generated on the work SITE and/or construction camp.
The Waste Management Plan should include but not be limited to: sanitary waste, blasted rock and debris,
waste water, dust (Blasting/grit blasting/Grouting), dust (road), , soil contamination, office and domestic
waste etc.
The objectives of the Waste Management Plan shall be defined by observing the following hierarchy:
• Avoidance and minimization (avoiding generating waste though changing or improving practices and
design)
• Reuse of materials, thus avoiding disposal (generally with only limited processing)
• Recovery and recycling, thus avoiding disposal (although reprocessing may be required)
• Treatment and disposal according to relevant regulations, guidelines and good practice.

14.3 ENVIRONMENTAL PROTECTION PLAN

EPCC CONTRACTOR and its Sub-contractors will comply with the following:

14.3.1 GENERAL

• Oils, grease, fuels, lubricants, paints, solvents, acids and alkalis, and contaminated waste water will
not be allowed to migrate into the plant ditch system, or discharge to the ground.
• Spills of petroleum products, chemicals and other materials must be immediately reported to EPCC
CONTRACTOR HSE Office, and containment and clean-up started immediately.
• Washing and maintenance of vehicles may only be done in specifically designated contained areas.
• All storage into tanks, storage, and loading/unloading of chemicals, fuels and similar bulk materials
must be in curbed or diked areas.
• Cleaning, washing and hydro-blasting of process equipment which has been exposed to chemicals,

oils, acids or other contaminants must be performed in run-off containment areas approved for this
type work.
• There will be no open burning of materials, brush, tires, construction materials, oils, etc.
• The use of any toxic or hazardous materials or chemicals must be approved by EPCC
CONTRACTOR HSE Office.

14.3.2 CONSTRUCTION EMISSIONS

A major source of environmental pollution during construction work is construction emission.


Emissions during construction will mainly come from vehicles used for transportation, heavy
construction equipment and from large diesel generators. The other construction equipment, generators
and vehicles will usually be modern and well maintained so noise and exhaust emission levels will be
relatively low. Countermeasures for construction emissions are as follows:

• All vehicles shall be properly maintained according to the manufacturer‘s handbook to minimize
excessive exhaust emissions. Proper records shall be maintained for auditing purposes.
• All internal combustion engines will be suitably serviced and maintained to reduce polluting
emissions to an acceptable level. Where possible, engines will be located away from work areas.
• Dust control shall be achieved by properly wetting the work area with water trucks, etc... prior to
commencing the work and/or other approved measures.

Further details of environmental pollution caused by construction activities and the countermeasures
are summarized below:

Soil Contamination
Adequate washing areas with impermeable floors will be provided for washing construction equipment
or vehicles. The wash water will be collected in basins with oil separator. Contaminated water shall be
discharged after sedimentation or disposed at the designated location by certified contractors.
Any oil spills is to be reported as per EPCC CONTRACTOR and OWNER requirements.

(1) Measures to reduce Noise, Dust, etc.


Noise
The following noise and vibration control measures mainly concern employees on the work SITE:
• Provide proper silencers/mufflers for mobile and engine driven equipment.
• Replace or repair defectuous equipment/parts with abnormal noise and/or vibration.
• Minimize high noise operations like pile driving at night.
• Use proper ear protectors.
• Confine temporary generator

Dust (Blasting/shot blasting, /Grouting)

Grit blasting will be performed on site in dedicated location. The grit blasting location shall be protected
on all sides with canvas or similar, and provided with proper ventilation if not in open air.

Dust (Road)
Limit traffic speed and spray water in dry season. Vehicles will be regularly washed and cleaned in
areas specially constructed with an impermeable concrete floors and oily water collection basins, if
required.

(4) Measures to Minimize Impact of Chemical Handling


Material Safety Data Sheet (MSDS) shall be supplied for chemicals and other harmful materials used or
stocked at the SITE. A copy of MSDS shall be submitted to EPCC CONTRACTOR HSE Office.
Handling, type of PPE to be used, storage and transportation, emergency spill measures must be done
according to MSDS.
Hazardous materials such as radioisotopes, chemicals, etc., shall be kept in suitable and approved
containers which are properly labelled identifying the substance, nature of hazards, precautions in
handling, etc.
Areas where hazardous materials are used or stocked and hazardous conditions exist shall be identified
and properly isolated with enclosures or barricades.
Written handling procedures of substances shall be established and displayed near the handling and
storage location, which include: Substance Name, Nature of the Hazards, Precautions for Handling,
Emergency Treatment of Exposure, Spill Clean-up Disposal
The inventory of hazardous substances should be recorded and updated weekly including the names of
substances, the volume brought in and the volume consumed and any incident reports, etc.
The inventory must be communicated to EPCC CONTRACTOR HSE Manager every week.
Special HSE training shall be given to employees risking exposure to hazardous substances.
Personnel exposed to or handling specific toxic chemicals or substances or radioactive substances shall
undergo periodic medical examinations in accordance with Industrial Safety and local Health Laws and
Regulations.
Prior to the commencement of the day’s work, the hazardous work location is measured by qualified
inspectors with adequate measurement apparatus, such as oxygen meter; inflammable/explosive gas
detectors.

Handling of chemicals
• Caution sign boards are displayed at conspicuous locations near storage and handling place
with the name, property, handling method, and actions required at an emergency.

• Proper Personal Protective Equipment and safety devices, such as rubber apron, gloves, gas
masks, ventilation systems, eye showers, washing facilities, etc... shall be provided and used
according MSDS (Material Safety Data Sheet).

A Chemical Handling Procedure shall be established for using, storing and disposing of chemicals,
including identification of potential hazards, occupational exposure limits, necessary personal protective
equipment (PPE), fire prevention and first-aid requirements.
All personnel involved in the handling and use of these chemical and those supervising these jobs are
to receive a pre-job training which shall also emphasize the hazard of static electricity when transferring
flammable liquids from one container to another.
MSDS and Chemical Handling Procedure shall be submitted to EPCC CONTRACTOR HSE Manager
and relevant personnel. Chemicals shall be stored in their original or approved containers and labelled
accordingly.

Personal Protective Equipment (PPE)


When using, handling or cleaning up chemical, chemical-resistant PPE such as face shields, face
masks, aprons, long sleeves and pants clothing, gloves, and boots, etc., shall be worn to protect against
contact and exposure.

• Respirator shall be worn to prevent inhalation of chemical vapor.


• Personnel shall undergo a respirator fitting training.
• Clean water supply shall be readily available for washing off any chemical contact during
spillage or splash.
• Where required, emergency shower and eye washer shall be provided.

Storage
• All containers shall be labelled with the chemical's name, ingredients, and cautions for storage
and handling.
• Flammable liquid shall be stored in the safety cabinet.
• Flammable chemicals shall be used in a safe can.
• Chemicals shall be stored in a non-combustible warehouse with adequate ventilation, an
impermeable floor, washing and drain facilities, shower and/or eye shower, and air conditioning,
as required.
• The warehouse shall be equipped with warning signs and locked to avoid unauthorized entry.
• The name of the responsible person shall be posted at the warehouse.

Transportation
• Persons handling hazardous chemical wastes shall wear appropriate protective gear and take
all precautions to ensure that the potential for spills and accidents is minimized.
• Employees and Subcontractors shall be trained in emergency response procedures in the event
of a spill or an accident.
• Information on the safety precautions necessary to ensure the protection of health and the
environment shall be maintained. This information shall be made available to all employees and
Subcontractors.
• Training in the handling of hazardous chemical wastes shall be provided.

(5) Emergency Spill Measures

EPCC CONTRACTOR will endeavor to minimize chance of spill and to provide adequate facility to
prevent spill from dispersing or soak into the ground by furnishing dike and oil-separator at refueling
facility or providing oil pans for equipment, if required.
Specific pollution contingency and emergency response procedures shall be issued to cover any
potential spill.

15 EMERGENCY RESPONSE PLAN

The Plan shall be communicated to all responsible managers and supervisors, and thoroughly
explained to all employees at the HSE induction training upon entry to the SITE and during Fire and
Evacuation Training.
The Emergency Response Plan should include:
(1) Identification of all possible emergency situations including situations involving other facilities
within the immediate area
(2) Emergency Communications Network
(3) Emergency Alarm System
(4) Individuals’ roles and responsibilities in an emergency
(5) Evacuation routes and assembly points
(6) Systematic shutdown of operations to minimize loss and reduce additional hazards
(7) Designation of an emergency headquarters to collect information and direct activities
(8) A search and rescue plan to save people that are trapped or have lost consciousness
(9) A procedure for a deciding when to terminate the emergency situation
(10) Emergency and evacuation training and drills plan
(11) Notification and reporting
(12) Emergency control room

The emergency communication network showing a list of personnel and parties to be contacted,
available emergency medical services and fire stations with telephone numbers and addresses, or other
addresses for obtaining assistance for quick communication, shall be established at the construction
SITE.
The evacuation routes and assembly location, and emergency communication network shall be
prominently displayed at key points around construction sites and in EPCC CONTRACTOR and
Subcontractors’ main offices, field offices and at the gates of the construction SITE.

15.1 ORGANIZATION AND RESPONSIBILITIES

(1) Organization
The SITE Manager shall establish an Emergency Response Plan in line with Owner facilities before
the commencement of the work and assign staff to. If EPCC CONTRACTOR staff is insufficient to staff
the organization, a request to the HSE Committee members to cooperate in staffing the organization
will be made. The Emergency Organization shall consist of:

Emergency Control Center (ECC):


Controls overall activities under the direction of the EPCC CONTRACTOR SITE Manager.

Information Team:
Collects information from all sources and inform the personnel concerned such as the management of
EPCC CONTRACTOR and the Subcontractors, the Authorities, the OWNER Emergency Command
Center, and the Project Director and EPCC CONTRACTOR HSE Management Office (Head Office).

Rescue Team:
Rescues, administers first aid and sends victims to the designated hospital.

Fire Fighting Team:


Fights fires to the extent of its capability, cooperating with the OWNER and/or official Fire Brigade.

Preventive Team
Takes preventive measures against the recurrence or spread of accidents.

(2) EPCC CONTRACTOR SITE Manager


The SITE Manager shall have overall responsibilities during Emergencies to direct his staff and for
taking all necessary precaution to prevent inducing an emergency.

His major duties and responsibilities are to:


• Establish and make public an Emergency Response Plan. When establishing these plans, he shall
obtain information on the OWNER requirements and system for emergencies and evacuation.
• Direct the staff of the Emergency Control Center.
• Issue the Emergency Alert according to the alert system.
• Issue the evacuation order to all personnel on the SITE.
• Issue the call off of the emergency after confirming it.
• Liaise closely with the OWNER during an emergency.
• Ensure fire fighting and evacuation training.
• Report emergencies in a timely manner to the Project Director and to EPCC CONTRACTOR HSE
Management Office (Head Office).

(3) EPCC CONTRACTOR Construction Manager and Discipline Managers


The Construction Manager and the Discipline Managers shall assist the SITE Manager.
Their major duties and responsibilities are to:
• Inform the Emergency Control Center by Radio (Walkie-Talkie) of the status of the emergency and
its location and wait to receive further instructions for evacuation, etc.
• Direct the construction supervisors.
• Lead the Prevention Team in establishing measures against the recurrence and spread of the
emergency.

(4) EPCC CONTRACTOR Construction Supervisors


The Construction Supervisors shall act under the direction of the Construction Manager and the
Discipline Managers.
Their duties and responsibilities are to:
• Instruct workers on other floors to proceed to the ground floor and to await further instructions in a
safe location.
• Instruct Subcontractors' personnel to evacuate when receiving evacuation instruction.
• Assist the Construction Manager and the Discipline Managers in directing the Prevention Team to
establish measures to prevent and minimize emergencies.
• Assist the Fire Fighting and Evacuation Teams.
• Instruct and ensure that all workers stop work and switch off all electrical and construction
equipment.

(5)EPCC CONTRACTOR SITE HSE Manager


The HSE Manager shall assist and advise the SITE Manager in the Emergency Control Center.
His major duties and responsibilities are to:
• Assist the SITE Manager in directing all Teams as stipulated in paragraph 12.1 (1).
• Prepare the Emergency Response Plan for the SITE Manager.
• Plan and conduct Training and Drills for fire fighting, rescue and evacuation at least twice a year.
• Prepare Equipment for emergency use.
• Instruct new staff and workers during the new employee orientation regarding the Emergency
Communication System and how to act during emergencies and how to evacuate.

(6) EPCC CONTRACTOR Security Manager


The Security Manager shall assist and advise the SITE Manager in the Emergency Control Center.
His major duties and responsibilities are to:
• Assist the SITE Manager in directing all Teams as stipulated in paragraph 12.1 (1).
• Coordination of all security related activities on the project and all parties involved.
• Supervision of the Security Company on Site (local Security Provider) during Emergency.
• Ensuring that threats are systematically assessed during risk reviews held at strategic phases of
the project and advise the level of threats likely to be encountered.
• Ensuring that all necessary personnel are attending security induction and training in order to
enhance security awareness and vigilance.
• Requesting relevant means to ensure a good level of security.

(7) EPCC CONTRACTOR SITE HSE Coordinator


He shall assist the HSE Manager.
His major duties and responsibilities are to:
• Direct the Rescue Team, First Aid Personnel and Fire Fighting Teams under the direction of the
HSE Manager.
• Direct the HSE Supervisors.
• Report to the HSE Manager by radio on the status of the evacuation, etc. and wait to receive
further instructions.

(8) EPCC CONTRACTOR SITE HSE Supervisors


They shall act under the direction of the HSE Manager.
Their major duties and responsibilities are to:
• Ensure the green flags are lowered and the red flags raised.
• Instruct Subcontractors' employees to evacuate the SITE when instructed to do so by the
Emergency Control Center.
• Ensure that Subcontractors confirm the number of workers at the assembly location.
• Assist the Rescue Team and the Fire Fighting Team.
• Inform the HSE Engineer/Coordinator of the evacuation status.

(9) EPCC CONTRACTOR Administration Manager


The Administration Manager shall assist the SITE Manager in the Emergency Control Center.
His major duties and responsibilities are to:
• Manage all administrative affairs during emergencies.
• Direct the Information Team.
• Provide all measures necessary for transportation and communication.
• Record emergencies and the actions taken in a sequential manner.

(10) Sub-contractors
Sub-contractors of the Safety Committee members shall cooperate with EPCC CONTRACTOR during
emergencies.
Their duties and responsibilities are to:
• Establish their own emergency organization and to define the role of individuals during an
emergency before the commencement of work.
• Join Training and Drills provided by EPCC CONTRACTOR for fire fighting, rescue and evacuation.
• Instruct their personnel on how to act during an emergency under the direction of EPCC
CONTRACTOR Management.

Establish an Emergency Alarm System


• Alert System
• Streamer or Flag System
Flag poles shall be installed in several locations at the SITE, with green flags raised during normal,
conditions and red flags during an emergency.
The alarm systems shall be different based on the type of emergency and its urgency.
Designate Evacuation Route and Assembly Area
Designate safe evacuation routes and an assembly area where all workers and staff shall gather.
The evacuation routes and the assembly area shall be shown on the layout plans of both the construction
SITE and the camp.
The evacuation route can also depend on wind direction, for this purpose, wind socks will be installed on
different point of the site.

Establish an Emergency Communication System


An emergency communication system shall be established between the person(s) that discovers the
emergency, EPCC CONTRACTOR office, OWNER Emergency Control Center, the Subcontractors' office,
and the fire station, hospital, etc…
Notification of this emergency communication system shall be posted in prominent locations at EPCC
CONTRACTOR and Subcontractors' offices.

Emergency Response Training and Drills


Emergency Response Training and Drills for Fire Fighting, Rescue and Evacuation shall be conducted at
least twice a year for all personnel in the construction SITE and the camp.

Equipment for Emergency Use


The following equipment shall be made readily available at or near the HSE Office:
• Talkie-walkies (radios)
• Portable Loudspeakers
• Portable Fire Extinguishers
• First Aid Kit
• Stretchers
• Ambulance for Medical Team
• Car for HSE Team

Register of Personnel
All personnel at the construction SITE and in the camp shall be registered. The register shall be kept up to
date permanently
Poster
The emergency alarm system, and the evacuation routes and assembly area shall be posted in prominent
locations in both the construction SITE and the camp SITE.

15.3 EMERGENCY COMMUNICATION

General
The Emergency Communication System (ECS) for normal working days, during week-ends and after
normal working hours shall be established before the commencement of the work.
The ECS shall be established in relation to the OWNER Emergency Communication System, and with the
advice and consultation of the OWNER.
Activating Alarm and Confirmation

(1) First Information


Whoever discovers an emergency shall immediately inform EPCC CONTRACTOR Field Office in
accordance with the Emergency Communication Channel.

(2) Activating Alarm

(Case-1): At the construction SITE and camp.


The person first receiving the information from the person who discovered it shall inform EPCC
CONTRACTOR SITE Manager and HSE Manager, and the SITE Manager shall inform the OWNER
Emergency Command Center. The SITE Manager or the HSE Manager shall order the activating of
the alarm in accordance with the Emergency Alarm System.

(Case-2): At an Existing Plant


When a person detects an emergency in or near an existing plant, that person shall inform the
OWNER Emergency Command Center of the facts and the responsible person shall activate the
emergency alarm.

(3) Confirmation

(Case-1): At the construction SITE and the camp


Immediately after receiving the information, a member (or members) of the Information Team shall
make his way to the location of the emergency with maximum haste and confirm the situation. Using a
(Talkie-walkie) Radio, he shall inform the Emergency Control center of the status of the situation and
shall request the dispatch of either the Rescue Team or the Fire Fighting Team according to the
situation.

(Case-2): At an Existing Plant


The Information Team shall contact the OWNER Emergency Command Center to confirm the alarm,
and to receive any other information and instruction.

(4) Instruction
Upon instruction from either the OWNER Emergency Command Center or EPCC CONTRACTOR
Emergency Control Center, the Information Team shall inform all persons at the SITE of the status of
the emergency and the actions to follow such as waiting for further instructions or proceeding to the
assembly area.
This information shall be conveyed by use portable loudspeakers, the green / red flag system, or the
designated siren system.

(5) Evacuation
After receiving an evacuation order from the Emergency Control Center, HSE Supervisors and
Construction Supervisors shall proceed to the SITE and ensure that all workers have stopped work,
that all equipment is switched off, and to instruct all workers to gather at the assembly area.

15.4 SPECIFIC INSTRUCTION FOR INDIVIDUAL EMERGENCIES

Fire

(1) The person who discovers a fire shall call his colleagues, and one of them shall immediately inform the
first contact point designated in the Emergency Communication System.
(2) The others shall attempt to put out the fire with the available fire extinguishers to the maximum extent
possible in this situation.
(3) On hearing a fire alarm, all workers shall cease work, stop all engines and switch off all electrical
equipment and tools.
(4) Persons working at height shall proceed to the ground, proceed to a safe location and await further
instructions.

Explosions

All personnel shall immediately evacuate to the designated assembly area when notified of an explosion
or on hearing the emergency alarm.

Leakage of Toxic or Flammable Gas or Liquid

(1) The person who discovers a leakage in an operating plant shall immediately inform the nearest
OWNER operators.
(2) One of the operators shall inform the first connection point designated in the OWNER Emergency
Communication System of the fact and await instructions from a responsible person.

(3) The other operators shall attempt to stop the leakage of gas or liquid, however if failing to do so or
danger of explosion or infection from the gas or liquid is suspected, they shall evacuate to a safe
location.
(4) On hearing the gas alarm, all workers shall cease work, stop all engines, switch off all electrical
equipment and tools, and evacuate to the assembly area.

Natural Calamity

(1) Apart from earthquakes, there are weather forecast warnings of floods due to heavy rain, or extreme
weather (wind storms, sand storms, tornadoes) available in most districts. Therefore the manager shall
continuously obtain such information during the district’s rainy or an extreme weather seasons.
(2) When the weather forecast office issues a preliminary alert, an Emergency Control Center shall be
established at the SITE to instruct personnel to evacuate or to take measures against collapse of
facilities.

15.5 DE-ACTIVATION OF AN EMERGENCY

All personnel in the assembly area may return to the work SITE after EPCC CONTRACTOR SITE Manager
calls off the emergency alert. A HSE Supervisor shall ensure that all red flags are lowered and that green
flags are raised. Workers shall return to the work SITE in an orderly manner and resume work.

16 PERSONNEL PROTECTIVE EQUIPMENT

EPCC CONTRACTOR and Sub-contractors shall provide their employees with the necessary and well-
maintained or new PPE’s free of charge. All PPE shall be regularly maintained and kept in good condition.
Worn-out or damaged PPE shall be returned for repair, or for disposal and replacement.

16.1 BASIC PERSONNEL PROTECTIVE EQUIPMENT

Basic PPE’s are:


- Safety Helmet
- Gloves.
- Safety Footwear (steel toe-cap and puncture resistant soles).
- Safety Glasses with side shield (dark type glasses for sunny daytime use + clear type for inside
building works and cloudy daytime use). For people wearing spectacles, they have to use safety
glasses specific for spectacles.

- Clothing suitable for the weather and working conditions


- Full covering clothing, during construction works these may be of cotton fabric. Following
introduction of hydrocarbons and during system start up activities only fire retardant coveralls will
be allowed.
- Fire retardant coveralls (if required by EPCC CONTRACTOR or OWNER in designated areas).

Wearing of basic protection is required for all people (incl. Visitors) on all site locations with the
exception of the site-offices, canteens and sanitary facilities.

16.2 SPECIFIC PPE REQUIRED BY CATEGORY OF WORK

Specific PPE may be required accordind MSDS or type of work.

For each case, refer to dedicated procedures (§ 4):

17 FIRE PREVENTION AND PROTECTION

17.1 GENERAL

A specialist with adequate training and experience in fire prevention and fire fighting to coordinate EPCC
CONTRACTOR overall fire prevention and fire fighting program, fire prevention / fighting training program,
and the training of Fire-watchers at the work SITE.
Emphasis is placed on pre-planning, hot work permit controls, flammable gas, liquid, and material control;
the control of smoking; training and use of warning signs, proper electrical wiring, and proper waste
storage and removal.

17.2 HOT WORK

Fire hazardous areas requiring to be defined as hot work permit area are designated by the EPCC
CONTRACTOR HSE Manager and the work is carried out in accordance with the hot work permit
procedure (ref. Site procedure 9952T-0000-PP-0848 “Permit to work)” for construction areas under EPCC
CONTRACTOR responsibility and “Referentiel Système Permis De Travail “ du groupe Sonatrach ” for
areas under OWNER responsibility.
All flammable materials and debris near and/or under any hot work, such as welding, gas cutting, etc.,
shall be removed or covered with non combustible materials. Fire extinguishers shall be supplied near
and/or under the hot work locations.
An experienced person shall be assigned responsibility for the hot work and his/her name shall be
displayed in a prominent location at the workplace. The person responsible shall always inspect and

validate the workplace and conditions. Such inspection includes checks on whether there are any fire
hazards during the work and that no sources of fire remain after the work as well as the proper
containment of sparks during cutting, grinding and welding activities.
Watchman must be present during the complete hot work duration.

17.3 FIRE WALLS

Firewalls are constructed between an existing plant and construction area, and where required, to arrest
flammable and explosive gas or fireballs from flammable work.
Firewalls shall be constructed with specified materials and be adequately dimensioned to prevent the
aforementioned hazards, and structurally sound against the strongest anticipated wind

17.4 AUTOMATIC GAS DETECTORS

Automatic flammable gas detectors with buzzers, that emit a loud warning when they detect gas, shall be
installed in gas leakage prone areas. This requirement applies even if firewalls have been erected. EPCC
CONTRACTOR HSE Engineer shall determine the positioning of detectors and the height of installation.
When a buzzer sounds a warning, all work at the SITE shall cease, construction equipment shall be turned
off, and all workers evacuated to the designated assembly point, in accordance with the emergency and
evacuation plan.
Where automatic gas detectors are unavailable or cannot be installed at a fixed position, portable gas
detectors may be used with the consent of the HSE Manager / HSE Engineer and the OWNER personnel
responsible for the location.
Qualified personnel in accordance with the manufacturer’s recommendations shall periodically calibrate
the gas detectors.

17.5 FIRE EXTINGUISHERS

Fire extinguishers shall be provided in offices, smoking sheds and hot work areas, and near electrical
equipment or diesel / gasoline engine equipment. The fire extinguishers shall be appropriate for the type of
fire hazard present.
The type, capacity, and number of fire extinguishers shall be subject to the EPCC Contractor HSE
Manager’s approval. HSE Supervisors shall inspect the locations and condition of the fire extinguishers
periodically according to a predetermined schedule and checklist.
As part of the general site induction, all personnel will be trained for the use of a fire extinguisher.

The Health Management Plan covers the general requirements to maintain employees and workers health
condition on the construction premises, and how to protect them from potential health hazards associated
with work and work environment.
Project Health Policy
EPCC CONTRACTOR Management commits to preserve employees and workers from health hazards and
to promote their consciousness of health .All EPCC CONTRACTOR personnel working for the project shall
comply with the following codes of conduct as project health policy.

• Adhere to the local regulations and OWNER HSE policy and requirements and EPCC CONTRACTOR
HSE Plan and requirements
• Pursue the goal of no harm to people and promote their health,
• Apply risk assessment process for anticipated health hazards,
• Ensure a hazard free and clean work environment,
• Monitor the compliance and effectiveness of the risk control measures adopted by the risk assessment,
and
• Assign experienced and competent personnel to implement the occupational health programs

Site Health Objectives


In order to ensure the project health policy, Project Director and appropriate levels of management shall
establish and maintain documented Project Health Objectives, which should be quantified and measurable
wherever possible by:

• Promoting and proactively pursuing the goal of zero occupational health incidents,
• Undertaking timely and systematic identification of potential health hazards across all disciplines and
areas of exposure,
• Conducting risk assessment for every identified health hazards and establishing adequate risk control
measures,
• Recording, tracking and documenting all actions to mitigate identified health hazards across all phases
of the project, and
• Conducting regular EPCC CONTRACTOR management reviews and audits of the effectiveness of the
health management system in fulfilling policy, objectives, and performance criteria.

18.1 RESPONSIBILITIES

18.1.1 EPCC CONTRACTOR Site Manager

The overall responsibilities of health control on the construction SITE shall be borne by the SITE Manager.
However, the requirements of this Health Management Plan and the OWNER requirements shall be
observed by every personnel involved in the Project, including the Subcontractors. The HSE Manager
shall assist them to fulfill the requirements of health control.

18.1.2 EPCC CONTRACTOR Site HSE Manager

The EPCC CONTRACTOR HSE Manager shall establish a Health Management Plan and the On-SITE
Health Rules for approval by EPCC CONTRACTOR Site Manager to ensure the creation of a safe and
healthy work environment for all employees concerned with construction work, referring to this document
and including the Legal regulations and the OWNER requirements.
The responsible persons designated by the HSE Manager shall patrol on the construction SITE at least
once a week to confirm health status and the conformity with the established requirements and rules.
If they find any unsatisfactory or substandard conditions that may affect personnel’s health condition, they
shall report it to the HSE Manager. The HSE Manager shall take necessary measures to prevent its
detrimental influence on peoples’ health, after consulting the SITE Manager and the persons concerned.

18.1.3 Subcontractor

The Subcontractor shall establish a Health Management Bridging Plan for approval by EPCC
CONTRACTOR Site Manager to ensure the interactions with EPCC CONTRACTOR Health Management
Plan, including the Legal regulations and the EPCC CONTRACTOR requirements.

18.2 HEALTH RISK CONTROL

For effective control of health risk associated with the work, the following proactive process is essential.
This process shall be carried out at an examination meeting involving engineers or specialists but also
supervisors or foremen with experience and knowledge of the work.

(1) Health Hazard Identification


Identify and make an inventory of the potential health hazards associated with the work and the work
environment.
All activities, material and environmental conditions associated with the work shall be carefully and
systematically examined to identify potential health hazards appropriately: observation, sampling,
inspection, measurement, interviews with people involved in the work, etc.
A survey shall be conducted to identify and evaluate the following potential occupational health
hazards:
• Toxic Substances
• Physical Hazards
• Psychological Hazards
• Ergonomic Hazards
• Manual Lifting of Loads
• High Noise and Vibration Hazards
• Other Health Hazards, if any

(2) Health Risk Assessment


Evaluate the risk anticipated from identified health hazards, and establish appropriate risk control
measures. The assessment requires suitable systematic evaluation techniques in order to quantify the
severity of the risk and the frequency and the time exposed to the hazards.

(3) Health Hazard Control Measures


Health hazard control measures should be established during the planning phase of the project
execution and engineering phase of plant/facilities, and improved when necessary during construction.
The principles of health hazard control are:
• Elimination of hazards
• Substitution with safer substances and/or devices
• Engineering control, such as improvement of operation, providing safeguards, ventilation, etc.
• Administrative control, such as providing proper work procedures, educational training,
observation of compliance with the established hazard control measures, HSE requirements and
rules, etc.
• Use of PPE

(4) Health Surveillance


Health surveillance aims at:
• Assessing the health of the individuals (EPCC CONTRACTOR and Sub-contractors’ employees
and workers);
• Assisting in checking the effectiveness of control measures;
• The collection of data for the detection and evaluation of health hazards.

Health surveillance may include physical monitoring and medical examination. The contents and
frequency of health surveillance should be determined according to the risk evaluation results
including pre-employment / pre-placement medical examination reports.

(5) Record Keeping


Health surveillance records and the records of exposure to identified health hazards shall be kept in
HSE Office to monitor individual’s health trends and occupational health problems arising from the
work and work environment, to fulfill legal requirements and to safeguard the OWNER against
unjustified compensation claims.
Personal medical records are confidential and must be treated accordingly.

18.3 HEALTH HAZARDS

(1) Any toxic substances for which possession or use is prohibited by law and regulation, such as
asbestos or asbestos containing product, PCBs, etc. shall not be used on this project.

(2) For other toxic substances, such as chemicals, including solvents, catalyst, etc., the Subcontractors
or suppliers shall submit Material Safety Data Sheets (MSDS) to HSE Manager for information.

(3) Material Safety Data Sheets shall be used for risk assessment and establishing risk control measure:
which shall cover how the substances are stored, used and disposed of while they are on the SITE.

(4) The Assessments shall state the precautions to be taken by personnel who use or come in contact
with the substances on the work SITE

(5) Hazard assessment shall be carried out by clarification of:

• The expected duration and frequency of exposure,


• The type of exposure i.e. vapour, dust, or liquid, inhalation or absorption by skin contact,
• Means of reducing potential risk to tolerable level i.e. keeping in tightly sealed containers,
isolating the operator from the material (by personal protective equipment, or barriers, etc.) or
modifying or improving the operation methods,
• Whether hazardous materials, such as radioisotopes, chemicals, etc… are kept in suitable
containers, as accepted by HSE Manager. The containers shall be properly labeled to identify
the contents, nature of hazard, handling, precautions, etc…

(6) Areas where hazardous materials are used or stored, and areas where hazardous conditions exist
shall be identified and properly isolated with enclosures or barricades,

(7) Exposure to X-rays, Y-rays and lasers shall be strictly controlled. All radiographers shall be trained
personnel, and all radioactive sources shall be controlled under the Permit to work system. Areas in
which radiography is taking place shall be roped off to restrict the access of unauthorized personnel.

(8) Written instructions for handling of hazardous substances shall be established and displayed near
the handling and storage location. The instructions shall indicate:

• Substance Name
• Nature of the Hazard(s)
• Handling Precautions
• Emergency Treatment after Exposure to the Substance
• Spill Clean-up
• Disposal

(9) The inventory of hazardous substances should be recorded and updated, periodically including the
names of substances, the volume brought in and the volume consumed, any incident reports, etc…

(10) Special HSE training shall be given to employees with at risk of being exposed to hazardous

(11) Personnel exposed to or handling specific toxic chemicals or substances or radioactive substances
shall receive periodic medical examinations in accordance with Industrial Safety and Health Laws
and Regulations.

Physical Hazards
People responsible for supervising workers shall identify all physical hazards and establish adequate
control measures for the physical hazards:
• Noise
• Vibrations, against both whole body and parts (hands and arms, etc.)
• High working temperatures
• Welding fumes
• Operations likely to cause upper limb disorder
• Eye injuries from welding work, etc.
• Cuts and contusions
• Electrical hazards.
• Burns from exposure to the sun
• Working at height

Psychological Hazards
Psychological hazards shall be identified and the risk shall be evaluated. The sources of stress identified
shall be reduced as much as reasonably practicable. Experienced medical staff or psychologists shall treat
psychological issues.

Ergonomic Hazards
Ergonomic hazards in the facilities shall be identified during engineering phase by systematically
integrating ease and safety with constructability, operability and maintainability. All unsafe or unhealthy
situations shall be resolved before purchasing or construction starts.

Manual Lifting of Loads


Manual handling of heavy or large loads shall be avoided when possible, however, if necessary the weight
and size of the loads shall be restricted and the working posture shall be adequate to avoid backache or
other disorder. Particular emphasis shall be placed on avoiding repetitive or forceful movements. Tasks
shall be designed to be within the recognized ergonomic limits. Specific training shall be provided for
potentially exposed personnel.

High Noise and Vibration Hazards


All equipment purchased for this project will be restricted to comply with the project specification on noise
and vibration levels. Equipment brought onto the work SITE shall be monitored by the Safety Officers to
ensure that the noise and vibration levels generated are within acceptable limits.

18.4 GENERAL HEALTH PROMOTION

(1) General
General health promotion aims at preventing health risks associated with the living environment and
life style such as infectious diseases (by immunization and prophylaxis), inadequate nutrition (by
information), use of tobacco, alcohol and other drugs (by employee assistance programs), and lack of
exercise and physical fitness (by information and fitness programs).
An effective health promotion program will lead to the improvement and upkeep of the overall health
status of employees at a high level, which may be assessed in the work environment by indices of
absenteeism and employee turnover.
The following activities are essential for an effective health promotion program:
• Assessment and prevention of health risk associated with the living environment;
• Assessment of life style associated with health risk in so far as they relate to work performance;
• Employee information and assistance programs to prevent these health risks;
• Record keeping to monitor progress in controlling these health risks.

(2) Sanitation Facilities


EPCC CONTRACTOR and all Subcontractors shall ensure the provision of adequate sanitation
including sufficient toilets, urinals (as OSHA ref. if < 20 worker, 1 toilet seat is required, if worforce >
20, 1 toilet seat + 1 urinal per 40 workers are required and if worforce is > 200, 1 toilet seat + 1 urinal
per 50 workers)and washing facilities for all personnel. All facilities will be kept clean and hygienic.
Urination or defecation in non-designated areas shall be cause for immediate dismissal.
Where men and women are working together in offices, canteens, workshops, etc., separate wash
rooms and toilets shall be supplied. Washrooms and toilets shall be kept tidy, clean and hygienic.
Sanitary water shall be treated or disposed in accordance with local regulations and the OWNER
requirements.

(3) Potable Water


Potable water shall be periodically tested and certified by an authorized laboratory. The quality of
potable water shall satisfy the local regulations and EPCC CONTRACTOR and the OWNER
requirements.
(4) Infectious Disease
If a patient is suspected of having an infectious disease, as designated by the local regulations, the
HSE Manager shall report it immediately to the SITE Manager and the local health officer, with the
name of the disease and the name, age, position and address of the patient. He/she shall be sent to
an appropriate hospital.

18.5 AUDIT OF HEALTH MANAGEMENT

Using an assigned assessment team, EPCC CONTRACTOR management will periodically assess the
health management performance on the SITE. These assessments will include Health Management Plan
and the overall performances. SITE Manager shall conduct self-assessment on a regular basis.
The management leadership is essential for health management on the SITE.
The audit team assigned by EPCC CONTRACTOR Management will periodically audit the project health
management performance and issue a formal report to the Management.

19 ACCIDENT/INCIDENT INVESTIGATION AND REPORTING

The EPCC CONTRACTOR SITE Manager shall take the initiative conducting accident/incident
investigations.

19.1 ACCIDENT/INCIDENT INVESTIGATION

If an accident occurs resulting in personnel injury, death, or property damage, EPCC CONTRACTOR SITE
Manager will immediately report the essential details of the accident to all concerned according to the
emergency communication network, and organize an accident investigation team on the SITE. Preliminary
report will be issued within 24 Hrs
The accident investigation team, directed by the SITE Manager, will consist of the Construction Manager,
Line Manager / Supervisors, HSE Manager, Engineer and Supervisors directly responsible for the work.
The Subcontractor‘s managerial personnel, and supervisors and foremen directly concerned by the
accident, and if possible, injured person will be involved in the investigation team.

19.2 EXTENT OF ACCIDENT INVESTIGATION

The following items will be included in the scope of the investigation:


• Any fatalities
• All non-disabling injuries and occupational illnesses
• All disabling injuries and occupational illnesses
• Traffic Accidents
• Fires and Explosions
• Equipment Failures
• Failure of Facilities
• Discharges of any hazardous substances and related accidents and incidents (near-misses)
• Near misses with the potential for a major accident.
• Or any event as required by the EPCC CONTRACTOR site manager or Owner

19.3 INVESTIGATION PROCEDURE

The following procedure will be applied when investigating an accident:


• Sealing-off of the vicinity of the accident, if possible
• Securing the SITE of the accident, preserving any physical evidence and taking photographs to record
the situation.
• Collecting information from personnel present at the time of the accident.
• Analyzing the cause(s) of the accident using a root cause analysis process as Tap Root
• Establishing countermeasures to prevent a recurrence of similar accidents.

19.4 ACCIDENT REPORT

All accidents/incidents shall be immediately reported to the Project Director and HSE Management Office
at EPCC CONTRACTOR Home Office, to the OWNER and other concerned parties or authorities,
according to the emergency communication network.
The preliminary and the final investigation reports shall be initiated immediately, and in accordance with
the accident/incident investigation and reporting procedure.
The SITE Manager will be responsible for finalizing and signing all Accidents/Incident reports, and
subsequent reporting.

19.5 FOLLOW-UP ACTIONS

After receiving permission from the OWNER and/or other parties, such as the insurance OWNER, local
authorities, police, etc… the follow-up activities for remedial action and re-starting of construction
operations will commence according to the SITE Manager‘s and/or Construction Manager’s instructions.
The progress of the remedial and follow-up action shall be recorded in the appropriate form and the
completed form retained for record during management meetings and managerial reviews, progress
reports, evaluation of the results, and communications to employees.
The results of each accident investigation, especially the outline, basic and direct causes of the accident
and measures taken to prevent reoccurrence of similar accidents, shall be presented by the investigation
team leader (Area in charge construction mgr ) at the weekly incident review committee and.announced by
electronic messaging to all EPCC CONTRACTOR management and employees, and utilized for HSE
training and HSE flyers

19.6 PROCEDURES FOR DISSEMINATING LEARNING POINTS

(1) Accident Data Analysis


Full use shall be made of all accident information.
• Root cause analysis shall be used to identify the organizational latent failures leading to serious
incidents.
• A weekly summary of incidents will be prepared for review by the weekly incident review
committeeand widely distributed across the project.
• An analysis of lost workday cases and the lost workday case rate by craft or Sub-contractors will

be used to highlight problem areas. This must include as well all Hipo near miss and RWC and FA
• Cumulative statistics will be used to show trends over the life of the Project.

(2) Project Monthly HSE Report

Significant HSE Activities shall include a concise review of HSE problems that occurred during the
month and the corrective actions taken to resolve them. HSE inspections by OWNER or host country
personnel should also be recorded here.
Unsolved HSE Issues will be described in sufficient detail to enable EPCC CONTRACTOR HSE
Management Office (Home Office) to assist in resolving the problem.
Anticipated HSE Issues, which are expected to arise within the next few months, shall be listed here.
Awareness of potential problems will enable EPCC CONTRACTOR HSE Management Office (Home
Office) to give better assistance to the field.
The SITE HSE Manager will submit this report together with the Monthly HSE Performance Report to
the Project Director, EPCC CONTRACTOR HSE Management Office (Home Office) and OWNER.
Report will include the HSE action register listing all main HSE action strategic decisions , initiatives
and close out status

(3) Lessons Learned Program

Key information and lessons learned derived from the investigation and analysis of any incidents will
be appropriately disseminated in scheduled or special safety meetings and via the other
communication tools within the HSE Program.
An Lesson Learned register shall be maintain for future reference and preparation of the Project final
Lesson Learned report

20 MEDICAL SERVICES PLAN

Trained first-aid personnel and suitable first-aid supplies and facilities shall be supplied at the SITE and
the camp, and a doctor shall be assigned at those locations.
Work SITE shall maintain close communication with and have access to occupational Medical Service
(hospital or clinic).
A list with details of the nearest hospital, including its locations and phone numbers, shall be posted in
all the SITE Offices, including Subcontractors’ and the camp offices.

(1) Prior to the start of work, arrangements shall be made for medical facilities, ambulance service when
site population exceed 500 as required by owner 6997-00-19-43-0001 rev1 (a properly equipped
emergency vehicle), and medical personnel to be available at the construction SITE.
(2) Effective communication and transportation to care for injured workers shall be provided.
(3) Suitable medical service facilities are determined according to the maximum number of employees
working on the SITE and availability of professional hospitals near the construction SITE shall be
surveyed.
(4) Where such professional medical services are available, each work SITE shall set up terms and
conditions to use, and keep close communication with them.
(5) A list of the available hospitals including locations and their phone numbers shall be displayed at all
SITE and camp offices including Subcontractors’.
(6) First aid arrangements and how-to use them shall be thoroughly covered in inductions and other
appropriate training.

20.2 MEDICAL FACILITIES

A medical facility will be setup on Site after the medical survey defines the size, equipment and
medical staff requirements and the selected subcontracting plan. For Subcontractor, access will be
granted only in case of MEDEVAC.
For 1st aid & MTC, Subcontractor shall explain in their HSE Plan the medical organization they will
implement in order to treat them directly.

20.3 MEDICAL STAFFS

(1) Doctor or Substitute (Licensed Physician/Emergency Medical Technician)


His major roles and responsibilities are to:
• Provide leadership to ensure all medical matters are dealt with in a professional and efficient
manner.
• Monitor the health and well-being of all SITE employees and notify any adverse trends to the HSE
Manager.
• Make sure the medical facility is equipped with sufficient and adequate materials, consumables,
instruments and staff to function effectively.
• Ensure that the medical facility and ambulances are kept clean and hygienic and ready to use at
all times.
• Be aware of any dangerous substances in use at the SITE, and obtain any Material Safety Data
Sheets (MSDS) to take proper care when required
• Notify the HSE Manager of any trends with illness or injuries, especially for infectious disease.
• Periodically check hygiene conditions of the canteens/restaurants, including all food delivery, food
preparation, cooking and serving areas, and inspect and perform medical on canteen/restaurant
staff to confirm they are free of any condition that may affect hygiene requirements.
• Periodically check hygiene conditions of all camps.
• Perform drug/alcohol tests on personnel as directed by the HSE Manager, and arrange for
analysis and report to the HSE Manager.
• Keep accurate records of all medical matters.
• Prepare Monthly Report on major activities and topics on medical matters and monthly statistics
including trend of patients, etc..., for submitting to the HSE Manager.

(2) Nurse
His or her major roles and responsibilities are to:
• Provide all possible medical assistance and care to improve the condition of persons suffering
injury or illness.
• Administer minor treatment, under the direction of the doctor.
• Be available to attend accidents/incidents on the SITE to give medical treatment as required.
• Assist the doctor in carrying out training programs for construction personnel. (First aid training,
etc.)
• Dispense pharmaceutical items as prescribed by the doctor, and keep records of prescriptions.
• Keep stock records of pharmaceutical/clinic items, and order replacement items to maintain
minimum stock levels.
• Report to the doctor on major activities and topics.

21 TRAFFIC RULES

Prior to initiate construction, EPCC CONTRACTOR will establish SITE Traffic Rules. Traffic rules shall
include the requirement for all drivers to possess a valid local driver’s license or International Drivers
License (if accepted by local regulations), speed limits, access route, parking limitations, and inspection and
maintenance requirements for vehicles, penalties for violators, etc.
All drivers and pedestrians shall strictly observe the Traffic Rules. Off the SITE, local traffic regulations are
to be observed.
Site traffic plan will be maintained by HSE group on weekly basis. During peak construction activity, A
construction manager or area superintendent will chair a weekly traffic coordination meeting with
subcontractor to update as necessary the site circulation plan.

22 HOUSEKEEPING PLAN

All EPCC CONTRACTOR and Sub-contractor employees are responsible for keeping the SITE, Camps,
and other facilities clean and orderly at all times.
The major objectives of housekeeping are to:
• Eliminate the causes of accidents and fires by removing obstacles and flammable hazards, which might
interfere with activities and passage of personnel and vehicles, and become a source of fire.
• Effectively utilize area and equipment by providing comfortable and safe working conditions.
• Create and maintain hygienic and safe working conditions.

All employees shall be required to maintain their work areas to the highest standards of housekeeping.
After each shift, all work crews shall perform housekeeping of their workface .

EPCC CONTRACTOR may stop works at any time when he considers that housekeeping standard is not
acceptable and instruct subcontractor’s resources to execute housekeeping activity immediately until entire

satisfaction of EPCC CONTRACTOR SM. Works may be allowed to resume only after confirmation by SM
that housekeeping is acceptable.

In order to improve site tidiness EPCC CONTRACTOR will implement a just in time delivery of material and
equipment and tools to the work face to avoid congestion. Generally the limit will be set a 3days maximum
storage or lay down at workface.
Subcontractor will arrange sufficient and adequate bins to be distributed on site. These bins will be collected
and disposed by subcontractors on regular basis or when full.

EPCC CONTRACTOR reserve his right under the subcontract agreement to execute housekeeping in case
of default of subcontractor and back charge relevant cost as required.

23 COMPLIANCE WITH RULES AND REGULATIONS

All employees shall comply with the Safety and Security Rules and Regulations on the SITE.
EPCC CONTRACTOR SITE Manager has the right to remove any employee from the SITE when they
seriously or repeatedly violate the rules or regulations.

23.1 WORKING CLOTHES AND FACIAL HAIR

The working clothes shall suit the climate, be task specific and not offend the cultural sensitivities.
The wearing of short pants or skirts and of sleeveless shirts shall not be allowed even for office activities. As
well, loose or torn clothing and sandals or sports shoes are prohibited.
Facial hair must be worn short so as not to hinder the wearing of PPE.
The above restrictions shall be applied to all personnel unless otherwise specified,

23.2 ALCOHOL AND DRUGS

Entry to the SITE while under the influence of, as well as possession use or sale of drugs, is strictly
prohibited and is cause for dismissal.
Substance abuse (for example, the use of drugs or consumption of alcohol) impairs performance at work and
gives a serious threat to HSE and productivity.
EPCC CONTRACTOR has a right to conduct unannounced searches for prohibited substances at work
locations
EPCC CONTRACTOR shall ensure that all employees are aware of this threat and aim to eliminate the risks
involved.

23.3 PERSONAL BEHAVIOR

All employees shall observe the following prohibitions:

• Horseplay and fighting are strictly forbidden.


• Running, except in an emergency, is discouraged.
• Sleeping,eating, smoking, praying in non-designated locations is forbidden.

23.4 SMOKING AND OPEN FIRES

Smoking and use of matches or lighters shall be strictly forbidden, except in designated areas where
permitted by EPCC CONTRACTOR SITE Manager and OWNER.

• Smoking is allowed only at the area designated by SITE Manager and under controlled conditions.
• Smoking in vehicles is always prohibited at the SITE.
• Open Fires are strictly forbidden on the SITE, except where permitted.

24 HSE PROMOTION

To enhance the participation of consciousness of all personnel to the Site HSE effort and stimulate their
interest in, and enthusiasm for, HSE, the following HSE promotion programs are adopted on the Algier
refinery construction site

24.1 HSE BULLETIN BOARDS

• HSE bulletin boards are provided at designated entrances to the construction SITE and other
conspicuous locations:
The accident-free man-hours figures for each Sub-contractors, total accident-free man-hour figures,
monthly HSE objectives/targets and Safety slogans, etc… are displayed on the boards.

• Information Boards :
Information and notifications regarding HSE subjects, etc. are posted at prominent locations in the
offices, canteens and camps.

• HSE Posters flyers and Signboards :


HSE posters and signboards are displayed at prominent locations on the construction SITE to enhance
HSE consciousness and provide warnings against hazards and unsafe acts.

24.2 HSE HANDBOOKS AND BROCHURES

To increase HSE awareness and as part of personnel HSE training, HSE handbooks or brochures will be
issued to all personnel at the SITE.
In order to cascade down the HSE information to the line level, there is a need to establish risk
communication material such as HSE handbook. However, no one will be allowed at the SITE until the
induction-training course has been carried out. Induction training course content should include the HSE
handbook topics such as the information on the project HSE Policies, work hazards and the emergency
response plan.
All personnel at SITE are obliged to understand the HSE rules and regulations and other relevant information
in the handbook.
The Line Supervisor should ensure the workers are briefed on the handbook and they should sign the
acknowledgement slip to demonstrate their commitment to the HSE rules and conditions. Individual workers
will receive a copy of the HSE handbook written in English / French / Arabic. A register shall be kept.

24.3 HSE INCENTIVE PLAN

Incentive program is an effective tool to enhance Subcontractors’ and their worker’s HSE consciousness and
to enhance their interest in safety, health, environmental and security.
The purpose of the Incentives program is to improve HSE management and to finally eliminate accidents, by
enhancing Subcontractors’ and their employees’ HSE consciousness and enthusiasm;
Subcontractors will participate.

25 HSE PERFORMANCE MEASUREMENT

The project uses both the leading and trailing indicators to measure and monitor HSE performance, which
include the following:

• Weekly/Monthly report of SITE HSE Statistics which covers man-hours worked, LTI’s, near misses,
accidents, incidents, TRIR.

• Periodic HSE inspections at SITE using a Score Card System and performed either by the SITE HSE
Manager and other staffs or by a third party to provide relevant information for further improvement on
specific HSE issues.

• The loss prevention program

• The behaviour based safety program with trends

• Weekly progress meeting on HSE Management Plan implementation to review the status of
achievement towards set objectives and targets.

suitability of the HSE Management Plan and HSE procedures implementation at the SITE.

conformance reports and any incident reports.

• Close surveillance of waste disposal, discharge, emissions, noise and disease outbreak at construction
Sites. Any critical incidents will be immediately highlighted to all workers and to the management.

26 HSE MANAGEMENT REVIEW

The HSE Management review will assess the policies, procedures, practices and controls to determine
whether the necessary HSE system exists to maintain compliance and verify that these operating
procedures are being followed and documented. It will also assess historical performance to determine if
relevant legal requirements have been addressed. It will note ‘Gaps’ in the HSE Management System
(HSEMS).

The following are performed in the HSE Management review:

• The HSE Management review meeting will be held every three months to review the results, statistics
and performance related to the HSE Management System in order to ensure its continuing suitability,
adequacy and effectiveness. The review is recorded by the SITE HSE Manager and approved by the
Top Management representative (Project Director), i.e. minutes of meeting. The HSE Management
review is chaired by the EPCC CONTRACTOR HSE department

• This review will address the possibility of changes to HSE policy, objectives and other elements of the
HSE Management System for continuous improvement purposes.

• This review will be organized by the EPCC CONTRACTOR Corporate HSE Manager and will be
attended by the key personnel at the SITE.

• This review should be recorded and formally communicated to the HSE committee and to the
responsible people for their respective HSEMS element(s) so that they may take appropriate action.

27 WORK SITE DOCUMENTATION
