Instructions:

1. Secure a student's clearance form at the Student Affairs and Services Office at the 3rd Floor Administration
Building, open every Monday through Friday from 8:00 a.m. to 5:00 p.m. only.

2. Laboratory / MIS clears the student of accountabilities in terms of utilization of equipment/tools and
computer laboratory.

3. The Health Services Office it promotes, develops and maintains the health and general well-being of the students
and also the school community.

5. Librarian clears the student of accountabilities in terms of book/s or any other library's information materials/s.

6. Program Head checks whether the student has subjected himself to evaluation in the previous semester
(enrolment purposes) to ensure that all units required for the program have been met; for graduating, whether
the academic requirements have been completed.

7. Office of the Dean clears the student upon confirmation with the Program Head as to the fulfillment of
the academic requirements

8. Student Affairs and Services Office clears the student on matters that concern the different units namely
Discipline, Guidance, Cultural and Performing Arts, Sports development, as well as the different student
organizations.

9. The Registrar's Office maintains student’s records and course data, which can include helping students schedule
or register for classes. They also make sure that their academic performance meets graduation requirements in
order to reach an institution's goals by verifying grades with professors on time.

10. This clearance shall be presented to the enrolling officer during enrolment or when claiming for
transfer credential, Transcript of Records (TOR), diploma and other certification

Note: The student needs first to write in full the names with honorific titles of their respective program chair, and
college dean before requesting for their signatures.
