AFFIDAVIT OF LOSS

I, JOHN WILLIAM G. HUGO, of legal age, single, Filipino, with residence

address at Coral ni Lopez, Calaca, Batangas, after having been duly sworn in
accordance with law do hereby depose and state:

1. That on May 2018, I completed college and graduated from STI College
Balayan;

2. That as proof of said completion, I was issued a Diploma by the said

school;

3. That I placed the original copy of my School Diploma inside a brown

envelope and kept it inside our house at the abovementioned address;

4. That on April 22, 2024, while I was tidying up my files and records, I
noticed that my High School Diploma is missing;

5. That despite diligent search and efforts to locate my School Diploma, I

could not find the same such that I now have reason to believe that it is
lost and could not be recovered;

6. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 3 rd day of June

2024 at Balayan, Batangas.
