NATIONAL FINALS

CONSENT AND WAIVER

This is to confirm that I am allowing my son/nephew/ward (name of player) to

participate in the NBTC League National Finals at SM MOA Arena, Pasay City on March 17-25, 2024.

I acknowledge the risks involved in his participation, including the travel from our place to the tournament
and vice versa.

In case of an injury, I give permission to the organizers to seek the appropriate medical attention for my
son/nephew/ward.

I likewise understand that NBTC, its organizers, officers, employees and sponsors are not liable for the
payment of the medical costs in the event of injury sustained during travel and the competitions.

I therefore release the NBTC, its organizers, officers, employees and sponsors from any liability resulting
from any injury, loss or damage that my son/nephew/ward may suffer while traveling to and from the venue
and during the duration of the competition.
