Medical Letter - Mitigating Circumstances

To Whom It May Concern,

I have assessed Sherin Puzhithara Nizam's medical history with regards to her recent health condition and
determined that she is unwell.

Based on the information available to me, Sherin has been struggling with difficult emotions and behaviors,
including self-harming actions such as cutting and burning herself. She has also been experiencing frequent
and intense panic attacks, which have caused her to fear leaving her house. In addition, she has been feeling
consistently down and hopeless, with changes in sleep, appetite, and ability to focus. She has sought medical
care from her GP and is currently receiving continuous therapy and medication. Consequently, she has been
unable to meet her educational duties as part of her studies and should be allowed absence from 29 to 12
June 2024 (inclusive).

Sherin has been advised to update her regular GP for further medical support. Given these mitigating
circumstances, I respectfully request that you take her health and medical symptoms into account.
