your home postcode is not in Brent. In order to be eligible for Community Mental Health Services
(secondary care for people with longstanding and enduring mental health difficulties) your home
address will need to be in the borough of Brent.

Based on the information you have provided, I can see that your local secondary care team are in
Tower Hamlets. Please contact your GP to book an appointment and please discuss your
difficulties with them and request for a referral to the Community Mental Health Team in Tower
Hamlets (CMHT)
