Cleaning and Sterilization

User's Reference Manual


User responsibility
This will perform in conformity with the description thereof contained
in this User’s Reference manual and accompanying labels and/or
inserts, when assembled, operated, maintained, and repaired in
accordance with the instruction provided.
This product must be checked periodically. A defective product
should not be used. Parts that are broken, missing, plainly worn,
distorted, or contaminated should be replaced immediately. Should
repair or replacement become necessary, the manufacturer
recommends that a telephonic or written request for service advice
be made to the nearest manufacturer Customer Service Center. This
product or any of its parts should not be repaired other than in
accordance with written instructions provided by the manufacturer
and by the manufacturer trained personnel. The product must not be
altered without the prior written approval of the manufacturer. The
user of this product shall have the sole responsibility for any
malfunction which results from improper use, faulty maintenance,
improper repair, damage, or alteration by anyone other than the
manufacturer.

Cleaning and sterilization

This User’s Reference manual provides information and instructions


for the anesthesia breathing system assembly and disassembly,
automated and manual cleaning, and sterilization.
For reliable operation of the anesthesia breathing system, clean and
sterilize the system according to the information in this manual.
An overview of the cleaning process is described below. Detailed
instructions for each step are provided within this manual.

1. Remove the canister and breathing system.


2. Disassemble the breathing system.
3. Wash the identified parts and cleaning cassette in an automated
washer.
4. Remove the parts from the automated washer and inspect parts
for damage.
5. Attach the cleaning cassette to the breathing system. Wash the
breathing system using the cleaning cassette in the automated
washer.
6. Remove the parts from the automated washer and remove the
cleaning cassette from the breathing system. Inspect parts for
damage.
7. Manually clean parts that are not allowed in automated washer
and inspect parts for damage.
8. Remove the bellows base and wrap the parts prior to
sterilization.
9. Perform the sterilization.
10. Inspect parts for damage before reassembly.
11. Reassemble the bellows and breathing system parts, install the
breathing system, and the canister.
12. The cleaning and sterilization is complete.

Intended use
This manual contains instructions for cleaning and sterilization of the
Carestation 600 series breathing system. Follow these instructions
for cleaning and sterilization of the system parts.

Important Use this manual along with the anesthesia system User’s Reference
manual. Always perform a preoperative checkout before using the
anesthesia machine after reassembly. The anesthesia system User’s
Reference manual contains a preoperative checkout list and
preoperative test instructions.

ISO 17664 compliance
The process for sterilization of the breathing system has been
validated by a third party firm and found to be in compliance with ISO
17664:2004.
These instructions have been validated by the manufacturer of the
medical device as being capable of preparing a medical device for
reuse. It remains the responsibility of the processor to make sure
that the reprocessing is actually performed using equipment,
materials, and personnel in the reprocessing facility achieve the
desired result. This requires validation and routine monitoring of the
process. Any deviation by the processor from the instructions
provided should be properly evaluated for effectiveness and potential
adverse consequences.
The manufacturer recommends the use of filter(s) that protect both
the inspiratory and expiratory ports to reduce the risk of cross-
contamination.

Cleaning information

Clean the breathing system prior to first use after installation. Wash
and sterilize the breathing circuit separately. For the most effective
cleaning and sterilization, do not mix the breathing circuit with other
devices or components.
See "Compatible chemical agents" for a list of the approved cleaning
agents.
Immediately clean potentially infectious material, for example blood
or secretions, from the system by wiping the contaminated area
using a disposable cloth and an approved cleaning and disinfectant
agent.
Parts marked 134°C are autoclavable. All parts in the breathing
system, except the circuit O2 cell, flow sensors and bellows, can be
washed in an automated washer using mild detergent.
Only surface clean the circuit O2 cell. Do not autoclave the circuit O2
cell. Do not wash the circuit O2 cell, flow sensors or bellows in an
automated washer.
A Cleaning Cassette kit is available to assist in the cleaning of the
breathing system when using an automatic washer.
Always perform a preoperative test after reassembly and prior to
use. The anesthesia system User’s Reference manual contains a
preoperative checkout list and preoperative test instructions. Inspect
and replace any damaged parts in the breathing system, if the test
failed.

WARNING Obey applicable safety precautions:

• Read and understand the material safety data sheet
for each cleaning agent.
• Read and understand the manual for all sterilization
equipment to be used.
• Follow facility guidelines for use of personal
protective equipment during cleaning and sterilization
procedures.
• A damaged O2 cell can leak and cause burns. Do not
breathe the fumes.

• Failure to properly install and use bacterial or viral filters


in a way that protects the inspiratory and expiratory ports
will result in additional required cleaning and sterilization
procedures. If bacterial or viral filters are not used,
perform the cleaning and sterilization procedures
specified in this guide after each patient to avoid cross-
contamination between patients.
• To help prevent fires:

• All covers used on the system must be made from


antistatic (conductive) materials. Static electricity can
cause fires.
• Desiccated (dehydrated) absorbent material may
produce dangerous chemical reactions when
exposed to inhalation anesthetics. Adequate
precautions should be taken to ensure that absorbent
does not dry out. Turn off all gases when finished
using the system.

• Obey infection control and safety procedures. Used


equipment may contain blood and body fluids.
• Moveable parts and removable components may present
a pinch or a crush hazard. Use care when moving or
replacing system parts and components.
CAUTION To help prevent damage:

• Refer to the manufacturer’s data if you have


questions about a cleaning agent.
• Follow hospital procedures for cleaning areas of the
machine not specified in this section.
• Do not wipe the surfaces or clean the system using
organic, halogenated, or petroleum-based solvents,
anesthetic agents, glass cleaners, acetone, or other

harsh cleaning agents. Doing so can damage the
labels or the system.
• Do not use abrasive cleaning agents (such as steel
wool, silver polish or cleaner).
• Keep all liquids away from electronic parts.
• Do not permit liquid to go into the equipment
housings.
• Do not soak parts for more than 15 minutes. Swelling
or accelerated aging can occur.
• Only autoclave parts marked 134°C. Do not exceed
134°C.
• Cleaning solutions must have a pH of 7.0 to 10.5.
Organic, halogenated or petroleum-based solvents,
anesthetic agents, glass cleaners, acetone, and other
harsh cleaning agents should not be used.

Cleaning and maintenance schedule
Refer to the anesthesia system’s User’s Reference manual for
complete maintenance instructions regarding the anesthesia system.

Minimum Frequency

Breathing system removal
The breathing system patient circuit, bag hose, and canister must be
removed before removing and disassembling the breathing system.

the breathing system.

The breathing system assembly can be disassembled for cleaning,
sterilization, and part replacement.

1. Push the patient port assembly tab and pull the assembly from
the breathing system.

Cleaning
Use the recommended method to clean the breathing system parts.
See "Compatible chemical agents" for a list of cleaning agents that
have been tested and found to cause no harm to breathing system
parts.

Compatible chemical agents

The compatible chemical agents listed have been tested and found
not to cause harm to the anesthesia system parts. Read the material
safety data sheet (MSDS) for each chemical agent. Use of these
chemicals will not damage the components when used as
suggested, but efficacy of cleaning or disinfection has not been
demonstrated in all cases.
The compatible chemical agents listed may not be available or
approved for use in all countries. Follow hospital guidelines for
cleaning and chemical agent use.

CAUTION Cleaning solutions not shown in the chemical agents list


must have a pH of 7.0 to 10.5. Organic, halogenated or
petroleum-based solvents, anesthetic agents, glass
cleaners, acetone, and other harsh chemical agents are
not recommended.
• Use of any chemical agents other than listed in this table
may degrade system performance.

Cleaning cassette kit

The cleaning cassette kit is intended to enhance the cleaning of the


breathing system. Using the cleaning cassette and automated
washer plugs provides effective particulate removal using optimal
water flow management through the breathing system parts.
Disassemble the breathing system to the level shown in the
"Breathing system removal" and "Disassemble the breathing system"
procedures.
Plug the unused washer ports before starting the automatic washer,
using the cleaning cassette. After washing, remove the parts from
the washer and drain any water. Allow parts to cool and dry
completely before reassembling the breathing system.

Attaching the cleaning cassette kit

1. Remove the patient port, flow sensors, and APL valve


diaphragm assembly from the breathing system as described in
"Breathing system disassembly".
2. Attach the cleaning kit to the breathing system as shown.

• With the Bag/Vent switch set in the middle position, slide


the Bag/Vent switch assembly over the Bag/Vent switch.
• Press the APL plug onto the APL base.
• Press the flow sensor blank assembly into the inspiratory
and expiratory ports.
• Re-insert the patient port into the flow sensor assembly.
• If the machine has an O2 cell, use the O2 cell plug to seal
the O2 cell base. If there is no O2 cell, do not connect the
O2 cell plug.
• Plug the inspiratory port with the inspiratory port plug.

washer. Keep the breathing system upright and put it into the
automated washer.

Automated washer
The breathing system components can be cleaned using an
automated washer. Remove all visible contaminants from the parts
prior to automated washing.
Read and understand the manufacturer’s instruction manual for the
automated washing equipment prior to use.
Cleaning cassette kits are available to enhance the cleaning of the
breathing system parts in an automated washer.

CAUTION Do not clean the flow sensors and circuit O2 cell in an


automated washer.
The following parts can be cleaned using an automated washer:
• Bottom breathing system
• Patient port assembly
• Reusable absorber canister
• Cleaning cassette kit

The automated washing process consists of first cleaning the


breathing system and the cleaning cassette separately in the
automated washer. Then, attaching the cleaning cassette to the
breathing system and running it through the automated washer. The
parts then need to be thoroughly dried and autoclaved.

Automated washing without cleaning cassette


1. Disassemble the breathing system. See the "Breathing system
removal" and the "Disassemble the breathing system"
procedures.
2. Empty the reusable absorber canister.

3. Place the components in the washer tray and load into an
automatic washer. Place the components with the cavity side
facing downwards to easily drain the cleaning water.
4. Use Prolystica Ultra Concentrate Neutral Detergent as the
cleaning agent. Set the dispense rate to 0.8 ml per liter of water.
5. Follow the automated washer operation procedure.

• Prewash I with cold potable tap water. Set Hold time to 4


minutes.
• Drain the water.
• Prewash II with cold potable tap water. Set Hold time to 4
minutes.
• Drain the water.
• Clean with warm water for 4 minutes at 45°C.
• Drain the water.
• Rinse for 5 minutes.
• Rinse an additional 5 minutes.
• Set the thermal disinfection time for 10 minutes at 90°C.
• Drain the water.
• Dry for 25 minutes with hot air at 110°C. (If components are
not dry after completion of the automatic washing program,
they should be left to dry in open air).

6. Inspect the parts for damage. Replace or repair as necessary.

Automated washing with cleaning cassette


1. Attach the cleaning cassette kit to the breathing system and
place it into the washer with the cavity side facing upwards. See
the "Cleaning cassette kit" section.
2. Use Prolystica Ultra Concentrate Neutral Detergent as the
cleaning agent. Set the dispense rate to 0.8 ml per liter of water.
3. Follow the automated washer procedure.

• Prewash I with cold potable tap water. Set Hold time to 4


minutes.
• Drain the water.
• Prewash II with cold potable tap water. Set Hold time to 4
minutes.
• Drain the water.
• Clean with warm water for 4 minutes at 45°C.
• Drain the water.
• Rinse for 5 minutes.
• Rinse for an additional 5 minutes.
• Set the thermal disinfection time for 10 minutes at 90°C.
• Drain the water.
• Dry for 25 minutes with hot air at 110°C. (If components are
not dry after completion of the automatic washing program,
they should be left to dry in open air).

4. Unload the parts from the automated washer.
5. Drain the water from the parts.
6. Allow the parts to cool and dry in room air.
Do not wipe the parts during the drying process.
7. After the parts have completely cooled and dried, check the
parts for damage and/or deterioration, such as deformation or
cracking. Replace or repair as necessary.

Manual cleaning

The following parts can be cleaned manually with the soaking


method:
• Flow sensors
• APL diaphragm

Soaking method
1. Mix a mild detergent solution.

• Dilute Prolystica 2x Neutral Detergent with distilled water


between 20°C and 40°C to obtain a solution of 4 ml per liter.
• Thoroughly mix the solution.

2. Wash and soak the parts in the mild detergent solution for a
minimum of 5 minutes in a clean soaking tray.

• Make sure that all the surfaces of the parts are submerged
and no air bubbles are visible.
• Gently agitate the parts to remove and dislodge any debris.
• Use a soft, plastic brush to remove stubborn debris from
external surfaces only.

CAUTION Do not use a brush, abrasives, sharp tools or


anything that could damage the inside or surface of
the parts.

• Do not touch the internal flow sensor flap, as it may


affect flow sensing performance.
--

3. Remove the parts from the from the detergent solution. Rinse
the parts in distilled water between 20°C to 25°C. Rinse the
parts again in distilled water between 25°C to 30°C. Allow the
parts to air dry.

• Parts must be fully dry before sterilization or reassembling


into the anesthesia machine.

• Do not wipe parts surfaces during the drying process.

4. Check the parts for deterioration such as deformation or


cracking.

Sterilization
The breathing system can be loaded into standard sterilization trays.
Do not exceed the maximum load of the sterilization tray. The
approximate weight of the breathing system is 2.5 kg (5.5 lb). All
parts should be cleaned prior to autoclaving. Do not block any ports
of the breathing system when placing parts in the sterilization tray.
Steam should be able to freely enter all components during
autoclaving.
Read and understand the manufacturer’s instruction manual for the
autoclave equipment prior to use.

CAUTION Only autoclave parts marked 134°C.


• Do not autoclave the circuit O2 cell or O2 cell cable.
• Do not autoclave disposable absorber canisters.

1. Remove and disassemble the breathing system. See the


"Breathing system removal" and "Disassemble the breathing
system" procedure.
2. Set the Bag/Vent switch to Vent.
3. Make sure all the parts are dry before using sterilization wrap.
4. Wrap the parts in light gauge sterilization wrap for autoclaving
(one-ply polyporpylene wrap Kimberly clark Kimguard KC600 or
equivalent).
5. Remove the bellows assembly from the breathing system. Put
the bellows with rim upside down into the housing and place it
on the sterilization tray, so that the bellows is extended during
sterilization.

6. Put the empty reusable absorber canister on the sterilization tray


with the cavity side facing down.
7. Put the remaining parts including the port caps, APL diaphragm,
flow sensors, and absorber canister cover on the sterilization
tray.
8. Autoclave at 132°C for minimum of 4 minutes using the pre-
vacuum dynamic air removal cycle. Do not exceed 134°C.

9. Dry the parts for 20 minutes before reassembly.

Note If intend to store breathing system after sterilization, increase drying


time to 60 minutes and place the bottom breathing system assembly
in vertical orientation, with the guide on top and bag-to-vent switch
on the bottom.

10. Allow the parts to cool and dry completely before reassembling
the breathing system.
11. In accordance with application requirements, store the sterilized
parts in the original sterilization wrap to guard against
contamination.
12. Remove the light gauge sterilization wrap prior to reassembly.
13. Visually inspect all parts for deterioration such as deformation
and cracking.
14. Inspect the parts for damage. Replace or repair as necessary.

Assemble the breathing system
Complete the "Assemble the bellows assembly" and "Breathing
system disassembly" procedures to reassemble the breathing
system.

Breathing system assembly

1. Install the APL diaphragm assembly. Press the APL diaphragm


down into place making sure it completely covers the port.

2. Confirm check valves are properly installed in both inspiratory
and expiratory side of breathing system.

3. Insert the flow sensors into the flow sensor holders. Make sure
the flow sensors are installed as shown in the illustration below.

Install the breathing system

1. Align the breathing system base with the guide pins, holding the
breathing system at approximately a 45 degree angle.

2. Push the breathing system into the anesthesia machine, holding
the breathing system at approximately a 45 degree angle, so it
may slide between the canister lifter and breathing system top
without interference.

3. Rotate up and push the breathing system base to lock it into


place.
A click sound will be heard when attached properly.
4. Close the breathing system door.

5. Install the absorber canister and lift handle to engage canister
with breathing system. Install bag hose.

Bellows assembly
The bellows assembly can be disassembled for sterilization and part
replacement.

Disassemble the bellows assembly

1. Turn the bellows housing counterclockwise and lift.

the rim.

WARNING Do not disassemble the pressure-relief valve. This can
damage the seat or diaphragm and cause injury to the
patient.

4. Lift up and remove the bellows base with the latch and seal.

Assemble the bellows assembly

1. Install the bellows base to the top housing. Align the vent port
and press down on the bellows base to make sure it is installed
correctly.

2. Install the pressure-relief valve.

3. Align the pressure-relief valve and attach the bellows with the
rim to the bellows base. Press down on the bellows base and a
double-click should be heard when the rim is installed. Pull up
on the bellows to make sure it is locked.

4. Lower the bellows housing and turn it clockwise to lock. Verify
the housing is secure and the guidelines are facing forward.

5. Perform the "Bellows assembly test" before completing the


assembly of the breathing system.

Bellows assembly test
Perform this test to make sure that the bellows assembly
components are correctly assembled. This test is not an alternative
to a complete system checkout. If the bellows assembly functions
correctly, continue with the assembly of the breathing system. If the
bellows assembly does not function correctly, verify proper
assembly, and look for and replace any damaged parts.

WARNING Objects in the breathing system can stop gas flow to the
patient. This can cause injury or death:

• Do not use a test plug that is small enough to fall into


the breathing system.
• Make sure that there are no test plugs or other
objects caught in the breathing system.

• The bellows assembly tests do not replace the


preoperative tests. Always complete the preoperative
tests specified in the anesthesia system User’s
Reference manual before using the system on a patient.

1. Unlock the bellows housing by turning it counterclockwise.


2. Lift the bellows housing and the attached bellows base
assembly from the breathing system.
3. Hold the bellows assembly vertical. Use test plugs to seal the
drive gas in (17.4 mm) and drive gas out (18.4 mm) ports.

4. Invert the bellows assembly. The bellows must not fall any
amount within one minute. If it falls, check for the following:

• The ports are not tightly sealed.


• The bellows is incorrectly installed.
• The seal inside the bellows is not correctly installed (with
the groove pointed up).
• Parts are damaged.

5. Remove the plugs from the ports. Permit the bellows to fully
extend.
6. Use the appropriate test plugs to seal the vent port (21.4 mm)
and sample gas port (4 mm).

7. Hold the bellows assembly upright. The bellows must not fall
past the top guideline within one minute. If it falls, check for the
following:

• The ports are not tightly sealed.


• The bellows or the pressure-relief valve is not correctly
installed.
• Parts are damaged.

8. If the bellows assembly functions correctly, continue with the


assembly of the breathing system.

Absorber canister
The absorber canister is available in two versions: disposable
absorber and reusable absorber. Only the reusable absorber
canister may be cleaned.
Discard the used absorbent and any liquid in the canister body
before cleaning and filling the absorber canister.

Filling the reusable absorber canister

1. To open the canister cover, lift the latch handle up.

2. Push the latch handle down to release the cover.

3. Pull the latch hook away from the canister and lift the cover off
the canister.

4. Remove and properly discard the absorbent and any water in
the canister body.

WARNING Be careful when emptying the absorber canister. The


liquid is caustic and may burn skin.

5. To clean and disinfect the canister, see the "Automated washer"


procedure for cleaning instructions and the "Sterilization"
procedure for autoclaving instructions.
6. Pour fresh absorbent into the canister.
7. Gently shake the canister from side to side to level the top of the
absorbent. Make sure the absorbent in the canister does not
exceed the fill line.
8. Wipe off any absorbent dust on the top cover wiper seal and the
canister sealing area.

Note The top cover wiper seal may discolor over time, however color
change alone has no effect on seal function.

9. Insert the cover into the groove on the canister handle so that
the arrows get covered by the black zone.

10. Attach the cover latch hook to the canister body. Push the latch
handle down until it is flush with the canister cover.

WARNING Failure to properly lock the canister cover could cause


canister malfunction and breathing circuit leaks. The
locking tab of the canister cover must be in the
downward position and flush with the cover.

11. Always perform a "Preoperative checkout" after reassembly


before using the anesthesia system.

Circuit O2 cell
Do not get the circuit O2 cell wet. Clean the circuit O2 cell by wiping
it with a damp cloth.

CAUTION Do not autoclave the circuit O2 cell.


• Do not put the circuit O2 cell in liquid.
WARNING The circuit O2 cell cannot be disinfected or sterilized. Be
aware that cross-contamination is possible.
