(Only for use by those who have been registered as a trainee)
The following must accompany this application form:

a.  1 Colour I.D photo which must be emailed as a JPEG Without this, no card can be printed!
b.  Check list confirming training successfully completed OR Statement of Results (SOR)
following the External Integrated Summative Assessment (EISA) if registered as a trainee
prior to 1st October 2022. Any person registered after this date must submit their SOR.

than that will be accepted.
d.  Proof of employment AS PER EXAMPLE ATTACHED.
e.  Copy of CURRENT letter of good standing re workmen’s compensation. This must be
current. Only if it is a very new (i.e. just opened) company, then we would require proof that this
had been applied for.
f.  Copy of CURRENT SANS 1475 Certification. However should this not be available, i.e.
should it have been cancelled or be a new company/workshop – in this instance, no
actual registration can be done, however a competent person letter will be issued which
does not entitle the technician to service and must be submitted to the relevant
Certification body either prior or at the time of the inspection of the workshop.
g.  Signed Code of Conduct

Please note that by filling in these forms and signing them you are making a legal declaration. If it
is found that any information is false or incorrect, the SAQCC (Fire) will apply such legal action as it
deems necessary. The forms should be emailed with all supporting documentation to the above
address. Failure to fulfil the above criteria will result in your application being rejected.

PAYMENT
An invoice will only be forwarded once all the above has been
received. Proof of payment for this invoice must be submitted in
order for the application to be processed.
IMPORTANT NOTE
The SAQCC (Fire) registers the individual technician and not the company he is
working for, therefore each application form must be completed in full and
submitted together with the above documentation. SHOULD DOCUMENTS BE
MISSING OR NOT COMPLETED CORRECTLY, THIS WILL LEAD TO A DELAY IN
COMPLETING THE REGISTRATION.
