1. In relation to DepEd Order No. 02, series of 2015- “Results- baser Performance Management
System”.
2. The school is scheduled for the implementation of Phase III which is “Performance Review
and Evaluation”.
2.1 Evaluation of teachers based on evidences of competencies measure against performance
targets agreed during Phase 1.
2.2 School head will point out the teacher’s strengths and weakness and gives a tentative
rating.
School head will discuss performance data and agree on the final rating
3. This will be held on July 27, 2023 at Agapito Amado National High School.
